[Efficacy of low-dose albendazole therapy in hydatic liver disease: a case report].

Hydatid liver disease is the most common form of human echinococcosis in Poland. Epidemiological data show a rising number of cases recognised last years. It is probably because of easier availability of ultrasound examination and computed tomography in the diagnostic procedures. While surgery remains the only approach for a radical cure, the chemotherapy with albendazole has became a useful advance in the management of cystic echinococcosis for the problem patient (inoperable disease, widely disseminated cysts, high surgical risk, etc.). The authors describe a patient with hydatid liver disease successfully treated with albendazole as a alternative treatment to surgery. A cyclic treatment of 28 days with 2 week break in between for 9 months with low doses of albendazole (6 mg/kg) resulted in clinical improvement and involution of hepatic cysts.

[Use of plasmapheresis in patients with AIDS and peripheral polyneuropathy].

The plasmapheresis is procedure removal of plasma and simultaneously, together with plasma, toxins, incorrect metabolites, antigens and immunological complexes. In patients with HIV infection often appearance periphery polyneuropathy. This is connected with autoantibody action on the damage of neurone. The aim of this study was evaluation effectiveness of plasmapheresis in treatment patients with AIDS and polyneuropathy. Seven patients with AIDS--C3 and polyneuropathy coexistence were treatment by plasmapheresis. The symptoms of polyneuropathy decrease in all patients. Were don't influence on immunological system and side effect performance plasmapheresis.

Plasmapheresis in the treatment of tropical malaria.

A case of tropical malaria imported from Kenya was described. Plasmodium falciparum (an etiological agent of this disease) is often drug resistant, therefore pharmacological treatment of the patients was supplemented with plasmapheresis. Full recovery has been achieved. It is suggested that plasmapheresis may be recommended for a complex therapy of severe forms of tropical malaria.

Efficacy of interferon therapy on patients with HCV infection in dependence from the time of treatment.

Among 45 patients with chronic hepatitis C the efficiency of interferon alpha 2b treatment was evaluated. The efficiency of interferon therapy was determined after 6 months of treatment in the group of 28 patients (group I) treated for 6 months and 17 patients (group II) treated for 12 months. The side effects were investigated with respect to such a factor as time of treatment. 11 (39%) patients from group I had eliminated HCV RNA and 14 (50%) patients had normalized AlAT levels in the serum. Five (29%) patients from group II eliminated HCV RNA and 8 (47%) patients had normalized AlAT levels in the serum. The flu-like syndrome, thrombocytopenia, vision disorder, depression and somnolence were most often the side effects observed in treated patients. Proportionally to time of treatment vision disorder, depression and somnolence increased.

[Rubella: the problem of women in procreative age].

The only effective method of preventing rubella virus infection is by vaccinating women who lack the antibodies IgG-RUB, which have been proven to prevent rubella. In these study we analyze the natural seroconversion against rubella virus among women in the procreative ages of 23-30 with ELFA assay (enzymimmunofluorescence assay), using a mini-VIDAS instrument. 7.8% of 148 women tested lacked IgG-RUB. Such seronegative women should be vaccinated before pregnancy.

[Cryptosporidium parvum: an emerging threat to the man].

The current opinions on the biology, epidemiology and potential role of Cryptosporidium parvum as an emerging, highly infectious threat are presented. Available Polish data concerning the frequency of cryptosporidiosis were gathered and the diagnostic procedures, pathogenesis and treatment of cryptosporidiosis are described.

[Epidemiological factors of mushroom poisoning in the north-east of Poland].

Mushroom poisoning (m.p.) are still real and increasing problems in Poland. Epidemiological characteristics, clinical picture and the outcome of the therapy with and without plasmapheresis are reported in 83 patients with mushroom poisoning. M.p. are occurring every year, between June and December. The most often m.p. cases occurred as collective intoxication (89%). The most often m.p. cases occurred as a collective intoxication (89%). The most severe cases are caused by the consumption of Amantia phalloides (31 cases), and deaths were reported only after consumption of that mushroom (6 deaths). Cases were also seen eating of not properly prepared eatable mushrooms (neurotoxins arisen from choline). Symptoms seen in our patients concluded: nausea, vomiting, abdominal pain, headache and fever. Particularly severe course of the disease was seen in children following intoxication with Amanita phalloides. Plasmapheresis was an effective therapy only when applied within 48 hours following intoxication.

[What motivates people to get tested for HIV antibodies?].

The aim of the study was to evaluate reasons for anti-HIV testing among persons admitted to Outpatient Counseling Unit. Serum examination for antibodies against HIV were performed in 389 persons during 8 years of the Unit activity. The most frequently it was analysed in intravenous drug users. However there are still many persons who are urged for this analysis before invasive medical procedures. Knowledge on HIV/AIDS in Poland seems to remain on the very low level in public as well as in medical staff.

Western blot banding pattern in early Lyme borreliosis among patients from an endemic region of north-eastern Poland.

Aim of this study was evaluation of Western blot banding patterns in different clinical forms of early Lyme borreliosis diagnosed in patients from north-eastern Poland, recognized as endemic for tick-borne diseases. Study was performed on serum samples of 48 patients with Lyme borreliosis and 26 healthy volunteers, as controls. Samples tested routinely for total antibody with enzyme immunoassay were subsequently analysed for specific antibodies with Western blot based on antigen extract of European strain of Borrelia burgdorferi. In patients, IgM antibodies were the most frequently directed against 41 kDa and 58 kDa antigens, whereas in control group only antibodies against 45 kDa and 58 kDa were present. Similar response was observed in respect to IgG antibodies. Evaluation of banding pattern in respect to clinical form of the disease revealed the highest prevalence of IgM and IgG anti-41 kDa antibodies in patients with erythema migrans and Lyme arthritis, and anti-58 kDa in neuroborreliosis patients, who had no anti-21 kDa antibodies. Relatively high frequency of IgG antibodies against 21, 30 and 93 kDa antigens was typical for neuroborreliosis. Bands count was significantly higher in different clinical forms of the disease than in controls, and it was the highest in neuroborreliosis. Combined analysis of Western blot results (IgM/IgG) enabled to achieve higher sensitivity (84%) and specificity (100%) than available with the most recommended EIA kits.

Plasma iPGE2 and i6-keto PGF1 alpha in the course of liver cirrhosis.

Plasma iPGE2 and i6-keto PGF1 alpha were measured with an EIA assay in twenty patients with alcohol-related liver cirrhosis (ALC group) and 13 patients with hepatitis B virus as an etiologic factor of liver cirrhosis (HLC group). Significant increase of both prostanoids was observed irrespectively of the etiology of liver cirrhosis. Their levels increased depending on the degree of liver insufficiency with the highest values in patients classified as Child-Pugh C class. A significant, positive correlation with Child-Pugh score was found regarding PGE2 (r = 0.657; p < 0.001) as well as 6-keto PGF1 alpha (r = 0.736; p < 0.001). Correlation (r = 0.789; p < 0.001) was also observed between levels of both prostaglandins. In conclusion we have shown that plasma iPGE2 and i6-keto PGF1 alpha arise simultaneously with the degree of liver insufficiency, that can be a result of activation of non-parenchymal liver cells accompanying hepatic fibrosis.

[Snake bites].

Vipera berus is the only viper i.e. venomous (poisonous) snake that is found in Poland. Snakebites occur rarely und usually follow attempts of catching the viper or accidental treading on it. Appropriate treatment consists of administering specific antivenom serum (antivenin). Four cases of snakebites hospitalized in 1989-1995 in the Department of Infectious Diseases of the University Medical School in Białystok are reported.

Plasmapheresis in the treatment of trichinellosis.

Four patients with severe form of trichinellosis were treated with plasmapheresis. We noticed clinical improvement and favourable alterations in laboratory profiles (morphology and immune response parameters in the blood). The effect of plasmapheresis therapy in patients with trichinellosis was considered beneficial. No major complications of plasmapheresis were observed.

[Candida humicola infection of the central nervous system in an HIV-infected patient: a case report].

HIV infection and parenteral drug abuse are at increased risk of developing Candida infection. Central nervous system (CNS) infection is an exceptional finding. We present fatal CNS infection by Candida humicola in drug abuser at late stage of HIV disease. In MEDLINE we have found only 3 reports about pathogenicity Candida humicola in humans (ophthalmopathy, conjunctivitis, melanonychia). Our case report suggests, that spectrum of pathogens dangerous to persons with AIDS increases.

Lysozyme in ulcerative colitis.

Studies were carried out on the neutrophil activity in a course of ulcerative colitis. Activity of lysozyme in blood serum as a granulocyte-derived enzyme was assessed. The studies included 60 patients who had 82 relapses and 11 patients with remissions. They were classified into mild, moderate and severe relapse according to severity of disease. Increased activity of lysozyme in blood serum was shown. That may indirectly prove the role of functional state of neutrophils in the disease.

Nitroblue tetrazolium test in course of ulcerative colitis.

Studies were carried out on the neutrophil activity in a course of a ulcerative colitis (UC). Phagocytic ability of granulocytes was assessed in spontaneous and stimulated nitroblue tetrazolium (NBT) reduction test. The studies included 60 patients who had 82 relapses and 11 patients with remission. They were classified into mild, moderate and severe relapse according to severity of disease. Increased phagocytic ability of neutrophils in spontaneous and stimulated NBT reduction test was shown.
