Outcome of treatment of combined orthopedic and arterial trauma to the lower extremity.

To ascertain morbidity and disability associated with vigorous attempts at limb salvage after lower extremity combined orthopedic and arterial injuries we reviewed more than 10,000 trauma admissions. Twenty-two patients (0.2%) suffered 14 blunt and eight penetrating lower extremity orthoarterial injuries. In all, more than 90 extremity operations were performed within 30 days of admission. The mechanism of injury, ISS, sequence of orthopedic and vascular procedures, use of temporary arterial shunts, the nature of the arterial reconstruction, length of ischemic time, and the presence of open fractures did not affect limb salvage or outcome. The presence of neurosensory/motor impairment and/or serious soft-tissue loss and injuries at or distal to the popliteal artery were associated with a high frequency of disability and amputation. Tremendous effort was expended to achieve limb salvage in the patients of this review. Patients who underwent amputation had the more uneventful hospitalizations and the lower rates of disability. The notion that limb salvage need to be obtained in all patients at all costs may often lead to the triumph of technique and technology over reason.

The relationship between women's attitudes about condoms and their use: implications for condom promotion programs.

A survey of 759 women attending contraceptive care clinics revealed that a majority of women endorsed condom use as an important way to reduce the spread of AIDS (acquired immunodeficiency syndrome); 82 per cent reported past use of condoms, but only 14 per cent reported using condoms in addition to another form of contraception to prevent infection with sexually transmitted pathogens, including HIV (human immunodeficiency virus). Acceptance of condom advertisement, perceived male and peer acceptance of condoms, and the effect of condom use on the enjoyment of sexual intercourse predicted condom use.

Clinical experience with the isotopic cardiac pacemaker.

Clinical experience with isotopic pacemakers in 59 patients is compared with that in 77 control patients having conventional chemical battery-powered pulse generators. The review covers a 51/2-year period. Statistical analysis of the two series is impossible because of the numerous variables such as age, type of disease, number of controls, types of test and control pulse generators, dates of insertion, and protocol regulations. However, there were pulse generator failures in the control group, but not in the test group. Though not proven in this study, the isotopic cardiac pacer is likely to last longer than conventional chemical battery-powered units, and could provide lifetime pacing for many patients. The risk of carcinogenesis is minimal and seems negligible in older patients. The isotopic cardiac pacer, in spite of restrictions of the Nuclear Regulatory Commission, should be considered for any patient with a life expectancy of 10 or more years. Paradoxically, it might be indicated in older rather than younger patients.

Temporary cardiac pacing: causes, recognition, and management of failure to pace.

Proper management of temporary cardiac pacing is based on the knowledge of the system's vulnerable points and the practice of preventive maintenance measures. This article has described emergency measures for and systematic analysis of pacing failure keyed by the presence or absence of pacing spikes, as well as the correction of such failures. The authors have addressed only one aspect of pacing failure, however. The reader is also encouraged to become familiar with sensing failure and the complications of insertion of pacing catheters.

Go-kart injuries at a fairground.

Twelve serious go-kart injuries were seen in three months. Almost all of these were caused by the steering wheel and all could have been prevented or their severity reduced by the use of a seat belt.
