Diagnosis of recent rubella virus infection by demonstration of specific immunoglobulin M antibodies: comparison of solid-phase reverse immunosorbent test with sucrose density gradient centrifugation.

A solid-phase reverse immunosorbent test (SPRIST) based on the addition of an excess of rubella virus hemagglutinin was evaluated for the demonstration of rubella-specific immunoglobulin M (IgM), and the results were compared with those of the density gradient centrifugation technique. In a retrospective study in which 157 sera were tested, the two techniques yielded identical results (55 IgM-positive and 102 IgM-negative samples). In a prospective study, 592 sere were examined; 8 IgM-positive results by SPRIST corresponded to a recent rubella infection or vaccination. Neither rheumatoid factor nor heterophil antibody seemed to interfere with the results of SPRIST. This test would be a useful and rapid routine technique for demonstration of the presence of virus-specific IgM in serum samples, particularly for viruses with a hemagglutinin. Except for anti-human IgM, no more reagents are needed than for widely used hemagglutination inhibition procedures.

[Acute skin disease due to staphylococcal infection (author's transl)].

Sixteen children with "scalded skin" due to staphylococcal infections are described [six cases of staphylococcal scarlet fever, 6 cases of bullous impetigo and 4 cases of toxic epidermal necrolysis (Lyell's disease--of which Ritter's disease is only the neonatal manifestation)]. The clinical features of each of these conditions are described, the common feature being the severity of the pathological changes. The role of the exfoliating toxin secreted by the pathogenic staphylococci mostly belonging to phagegroup II, phagetype 3A, 3N, 3C, 55 or 71) is emphasized. Treatment should be aimed at reducing secondary infection by strict asepsis and by eradicating staphylococci with appropriate antibiotic therapy. Corticosteroids have no beneficial effect. The outcome is good if these principles are applied strictly.

[Estimation of normal body volumes in children by the measurement of total electrical impedance (author's transl)].

The theoretical fluid volume of 41 normal children (mean age 8 years 9 months) was estimated from anthropometric data: height, weight, wrist circumference, and body surface. The correlation between this method and the conventional methods of determining total body water using tritiated water or of extracellular fluid volume using stable bromide or bromide 82 is very good. The real fluid volumes have been measured using total body electrical impedance at low frequency (Z5 kHz) and high frequency (Z1 MHz). The correlation of these results with those obtained by anthropometry is very satisfactory (r = 0.89; p < 0,001).

Malignant boutonneuse fever and polymyalgia rheumatica: a coincidental association?

Mediterranean spotted fever is a tick-borne disease that is endemic in the Mediterranean basin from spring to autumn. Usually mild, the disease can be severe in some cases, especially when risk factors are encountered in patients or when treatment is delayed. The correlation between these malignant forms and patients' immunological disorders remains unclear, while the pathophysiology of the disease seems well known. A case of a malignant form of Mediterranean spotted fever is reported which occurred 2 months prior to the diagnosis of polymyalgia rheumatica. Evidence of immunological disorders consisted only in an antiphospholipid antibody associated with a transient lupus anticoagulant. No underlying risk factors other than the primary undiagnosed phase of polymyalgia rheumatica has been observed.

Detection of activated basophils using flow cytometry for diagnosis in atopic patients.

BACKGROUND: human basophils release mediators of allergy after cross-linking of IgE receptors by allergens. Specific activation of basophils is detectable through flow cytometry (FCM) using an anti-CD63 fluorescein-conjugated monoclonal antibody. OBJECTIVES: this study evaluate the detection of activated basophils by FCM in routine diagnosis of atopic diseases as regard to skin prick tests and specific immunoglobulin E antibodies. METHODS AND RESULTS: whole blood from twenty patients suspected of atopy was preincubated with interleukin-3 (IL-3), then incubated with specific allergens. After staining using anti-CD63 antibodies, activated basophils were detected through FCM. IL-3-preincubation increases the spontaneous expression of CD63 even at low concentrations (0.1 ng/ml) on the basophils of 2 patients out of 20. The sensitivity and specificity of FCM were respectively 0.56 +/- 0.17 (m +/- SD) and 1.0 +/- 0.0 for the detection of dust mite-activated basophils without IL-3 preincubation, and 0.73 +/- 0.13 and 1.0 +/- 0.0 for the detention of grass pollen-activated basophils. IL-3-preincubation increased the sensitivity in a dose-dependent manner but decreased the specificity fo FCM for detecting dust mite hypersensitivity. CONCLUSION: this method allow for rapid and easy detection of activated basophils from whole blood, and could be of interest for detecting allergies to non-conventional allergens such as pharmaceutical drugs.

[Value of the use of the oral route versus the injectable route for fluoroquinolones. Pharmacoeconomic incidence and bibliographical study].

The aim of this study was to research the optimal conditions to shift to oral from injectable administration route for the fluoroquinolone antibiotics and the pharmacoeconomic and therapeutic impact of such a shift. Two indicators were used: proportion of the two administration routes, and mean cost per administration. The published results of pharmacokinetic studies in healthy and diseased subjects, and the clinical and/or pharmacokinetic studies including the notion of a therapeutic shift from the parenteral route to the oral route have been selected. The bioequivalence pharmacokinetic parameters of oral and injectable forms and the major clinical data of the therapeutic shift have been listed. Literature analysis reveals that there are few studies covering the specific assessment of the switch. The financial consequences of oral administration early use show the importance of such studies.
