Antisperm antibodies and microorganisms in genital secretions--a clinically significant relationship?

In asymptomatic infertility patients, no significant relationship was found between the presence of antisperm antibodies (ASA) in serum and in semen samples (IgG and/or IgA ASA), differentiated with the mixed antiglobulin reaction (MAR), and the microbial colonization of ejaculates covering a broad spectrum of microorganisms. Likewise, there was no significant association of ASA with microbial findings in patients' female partners, who also presented without symptoms of genital tract infection and were screened at the same time. Furthermore, ASA in semen (IgG and IgA) were not significantly related to several potential markers of subclinical male sexual gland infection or inflammation (leukocytes, PMN elastase, albumin, C3c) evaluated in aliquots of the same ejaculates used for immunological testing.

[Granuloma pyogenicum--removal with the CO2 laser].

13 patients with pyogenic granuloma were treated with a CO2 laser using the continuous wave laser in all cases and using additionally the ultrapulsed laser in 7 patients. The pyogenic granuloma was removed in all patients in one session. Side effects included transient erythema in 8 patients and scars in 2 cases. There was no hypo- or hyperpigmentation. The use of the CO2 laser is a fast and bloodless way to treat pyogenic granuloma with slight side effects and is an elegant alternative to standard approaches.

[The pulsed dye laser--possibilities and limits].

The pulsed dye laser (wavelength 585 nm, pulse duration 0.3-0.45 ms) is very effective in the treatment of port-wine stains, beginning hemangiomas, telangiectasias, spider nevi, xanthelasma and red hypertrophic scarring. It is the laser of choice in the therapy of young patients with port-wine stains. The pulsed dye laser is limited by its relatively superficial penetration into the skin with a maximum of 1.5 mm.

Chlamydial serology in 1303 asymptomatic subfertile couples.

The clinical significance of antichlamydial antibodies (Chlam Ab) was determined in a total of 1303 subfertile couples consulting for infertility investigation and treatment. Median age of the women was 30 (range 22-44) years and of the men 33 (range 21-53) years. The median duration of infertility was 4 (range 1-21) years. All patients were asymptomatic for genital tract infection. A comprehensive infertility investigation included examination of the endocrine, cervical, and tubal factor, and semen analysis, antisperm antibody (ASA) testing, sperm-mucus interaction testing in vitro using a standardized protocol, and post-coital testing (PCT). Screening for Chlam IgG Ab was performed in serum of both partners, obtained at the same time. Simultaneous microbial cultures in genital secretions of both partners included a broad spectrum of potentially pathogenic bacteria. Elevated titres of Chlam IgG Ab as seromarker for previous infection were found in 20.8% of all women, and in 12.6% of men. Chlam Ab were significantly more frequent in partners of seropositive patients (in 51.8% of women with a Chlam Ab positive partner, compared to 15.8% of the other women). Microbial screening outcome was not significantly related to results of chlamydial serology in both partners. In women, elevated titres of Chlam Ab were significantly associated with a tubal factor, but were not related to reduced quality of the endocervical mucus (CM), including the in-vitro penetrability of the CM (using partners' or donors' spermatozoa). In males, Chlam Ab were not significantly related to the outcome of semen analysis, including screening for ASA (IgG and/or IgA) in semen, and several parameters of sperm functional capacity. After exclusion of couples with tubal disease, subsequent male fertility did not significantly differ in males with or without Chlam Ab. The results suggest that during basic infertility investigation, positive chlamydial serology as an easy screening procedure indicates a higher risk for a tubal infertility factor. However, in asymptomatic patients, Chlam IgG Ab in serum are not associated with a cervical factor or with the male factor, using several determinants for evaluation of semen quality including subsequent fertilizing capacity.

[Lentigo benigna. Removal with the Q-switched ruby laser].

The Q-switched ruby laser (wave-length 694 nm, pulse duration 25 ns or 40 ns) causes selective damage to natural or artificial pigment. This laser has produced very good results in the treatment of amateur, professional and traumatic tattoos and of a number of benign pigmented lesions. We report on two patients with lentigines treated effectively with a Q-switched ruby laser.

[Removal of wrinkles and acne scars with the ultra-pulse CO2 laser].

The introduction of the UltraPulse Co2 laser has made it possible to remove wrinkles without scarring, especially in the perioral and periorbital area. Often only a single treatment session is required. The very short impulse duration of the laser is less than the thermal relaxation time of the skin, so that thermal damage and scanning of adjacent skin is almost non-existent. We report on a patient with perioral wrinkles who was treated by UltraPulse CO2 laser in a single sitting with a cosmetically excellent result. Another patient with acne scarring, and a third patient with wrinkles and acne scarring were also treated successfully in a single session.

[Evaluating polymerase chain reaction for detection of C. trachomatis in urogenital smears].

A total of 222 urogenital specimens were investigated with a commercially available polymerase chain reaction (Amplicor test) for the direct detection of Chlamydia trachomatis. The results were compared with those yielded by the conventional cell culture technique. Using cell culture a urogenital C. trachomatis infection could be detected in 72 of 222 patients. The Amplicor test yielded a positive result in 83. Referred to the detection rate of the Amplicor test and that of the cell culture, sensitivity was 91.2% for the test sensitivity and 79.1% for the cell culture. The specificity of both techniques was 100% when the specimens for which neither both nor either one of the tests gave positive results were considered. In accordance with other studies, this study suggests that tests based on nucleic acid amplification will supersede cell cultures as the gold standard for the detection of C. trachomatis and also become the method of choice in routine diagnosis of urogenital chamydial infections.

[Hyalinosis cutis et mucosae and Ehlers-Danlos syndrome. A rare combination of syndromes].

We report on a 7-year-old boy suffering from two rare genetic diseases, namely hyalinosis cutis et mucosae and Ehlers-Danlos syndrome. The diagnosis was finally established after 7 years, by means of light and electron microscopy and immunohistology. Therefore, this report deals with the typical clinical and morphological features of both genetic disorders.

[Diagnosis, therapy and prognosis of atypical mycobacterial infections--results of a retrospective study].

The most important mycobacteria causing disease in humans are Mycobacterium tuberculosis and Mycobacterium leprae. These germs contrast the so-called atypical mycobacteria. The importance of the atypical mycobacteria was recognized in the fifties. Even if the quantity of atypical mycobacterial disease has increased during the last decades in Germany, it is still a rare disease today, but it is seen in patients with acquired immunodeficiency syndrome more often nowadays. In the period from 1st January 1986 til 31st December 1992 31 HIV-negative patients with a diagnosis of atypical mycobacterial disease have been seen in the department for lung diseases in the Thoraxklinik Heidelberg-Rohrbach. During the same period an atypical mycobacterial disease was diagnosed in 12 out of 413 HIV-positive patients (2.9%) of the AIDS-ambulance of the skin hospital of the University of Heidelberg. Most of the HIV-negative patients showed additional diseases which reduce the immunological resistance. In HIV-positive patients an atypical mycobacteriosis heralds a severe immunodeficiency. Because it is a rare disease and an exact diagnosis is difficult to establish there is a lack of controlled clinical trials and therefore detailed therapeutical guidelines do not exist. A therapeutical approach is also complicated by a lack of effective drugs. With disseminated disease in AIDS-patients, which is mostly caused by Mycobacterium avium-intracellulare, the therapy should be stopped, if there are any severe side-effects. The present results of therapy are still disappointing. In the future the management of atypical mycobacterial disease will be more important, because there is an increasing number of patients with acquired immunodeficiency.

[Erythrokeratolysis hiemalis. Erythematosquamous genetic dermatosis with seasonal manifestation].

Erythrokeratolysis hiemalis, keratolytic winter erythema or Oudtshoorn skin has been reported from the South African district of Oudtshoorn as a dominantly inherited dermatosis beginning in early childhood, in some cases with circinar scaling erythemas. Seasonal manifestation in winter-time and a characteristic multi-form histology distinguish this dermatosis from other childhood scaling erythemas. We present clinical, histological and preliminary immunohistological data of a 4-year-old girl with the attributes of erythrokeratolysis hiemalis. No ancestors from the endemic region were traced. The lack of further cases in the family is interpreted as indicative of a spontaneous dominant new mutation.

[Duplex ultrasound of post-thrombotic syndrome--quantification of venous flow parameters].

In the last years Duplex-Sonography established itself in the diagnosis of venous diseases. This noninvasive method combines Doppler-ultrasound and real-time sonography. In this way an exact quantitative interpretation of the Dopplersounds in relation to the examined blood vessel is possible. Especially the venous bloodflow of postthrombotic veins will be explored. It will be discussed whether the data measured by Duplex-sonography allow a staging of venous diseases.
