[Action of trimethyl 3,3-N-hexene-5-lactam on the mechanical properties of the longitudinal fibers of the rat colon. An in vitro study].

The effects of trimethyl 3,3-N-hexene-5-lactam (T. H. L.) on the mechanical properties of longitudinal muscle strips of the distal rat colon were investigated through stretching, electrical stimulation and addition of pharmacologic agents. T. H. L. abolished the contraction induced by electrical stimulation and decreased amplitude of contractions induced by carbamoylcholine and serotonin. It further reduced the relaxation induced by adrenaline.

The cradle of pediatric surgery.

The nearly two hundred years history of the Hôpital des Enfants-Malades in Paris is described, with special emphasis on the development of pediatric surgery in this hospital. Described is also the work of the various surgeons-in-chief, who since 1899 were also appointed professors of pediatric surgery. Progress was slow during the first half of the nineteenth century, but after Guersant took over in 1844 the hospital became a center for this specialty.

Total colonic aganglionosis (with or without ileal involvement): a review of 27 cases.

From 1960 to 1984, 27 cases of total colonic aganglionosis were treated at the Sick Children's Hospital in Paris; in 19 cases there was ileal involvement, 16 of them extending more than 15 cm above the ileocecal valve. Five had a family history of Hirschsprung's disease. Nine infants died without having had definitive surgery, because of delayed diagnosis, or intractable malabsorption in extensive ileal aganglionosis. Two cases were diagnosed only at the ages of 6 and 13 years. Eleven children had Martin's modification of the Duhamel operation, the oldest of these being now 13 years old; and one girl aged 13 underwent a Swenson operation with ileoanal anastomosis, and one child has had a Kimura procedure. Four infants still have a diverting ileostomy. One out of the 14 operated children died 3 years after operation with fulminating enterocolitis. Late surgical nutritional results are analyzed with regard to the length of the side-to-side ileocolorectal anastomosis, and to the size of the ileorectal anastomosis, on which adequate pouch emptying depends. The essential problem in total colonic aganglionosis is not the surgical management of the condition, but rather its prompt diagnosis and the handling of the neonatal intestinal obstruction.

[Cryptorchism and surgery].

Many different anatomical situations are observed by pediatric surgeons during the surgical procedure for cryptorchidism at any age. The authors propose an easy anatomical classification: type I seems to be a small trouble of the process of migration and to be more a disturbed fixation. Type III results from a big disturbed wolfian duct process. Type II is intermediary. A series of 102 cryptorchidism operated upon at the Pediatric Surgical depart. Children's Hospital Paris, were studied according to this classification and collorated with histopathological study. The correlation is significative. The authors suggest that this protocol of surgical description could be agreed by a lot of Pediatric surgeons, whatever be the age of patient at operation, in the purpose to allow a longitudinal study from initial anatomical disorders to spermogram and at last to bring a response to the unanswered question: what, when and how, a cryptorchidism has to be cured.

Measuring the anorectal angle.

Lateral radiographs of the rectum and anus were taken in 90 children with chronic idiopathic constipation. A computer program was developed to obtain an objective quantitative assessment of the anorectal angle; it averaged 120 degrees at rest, decreased during forceful anal contraction and increased during defecation. It was noted that 24% of the children, when asked to defecate, decreased their anorectal angle as normally observed during forceful anal contraction and, that 21% when asked to contract the anus, increased the angle as if they were defecating. This 'sphincteric disobedience' is probably of major clinical relevance to an understanding of the symptoms of these patients.

Anorectal motility after surgery for spina bifida.

Anorectal motility was studied in 93 children (aged 15 days to 16 1/2 years) who had undergone surgery for myelocele or meningomyelocele, and 80 controls, matched for age and sex. Mean resting pressure in the anal canal of patients was markedly decreased, particularly in the presence of incontinence, and unstable. The recording of a rectoanal inhibitory reflex both in the upper anal canal and at the anal margin was associated with the presence of fecal incontinence and motor disorders of the lower extremities. Patients could be divided into four groups according to manometric pattern: 1) those with normal recordings (incontinence in 63 per cent), 2) those with spontaneous intermittent relaxations of the upper anal canal and simultaneous rectal contractions (incontinence in 91 per cent), 3) those with normal manometry except for unstable upper anal canal pressure (incontinence in 92 per cent), and 4) those with absence of the rectoanal inhibitory reflex and hypotonic anal canal (patients too young to appraise continence, all under 2 years).

[Abdominal complications of ventriculo-peritoneal shunts in children. 65 cases].

From 1971 to 1982, among 1 200 children with hydrocephalus cured by ventriculo-peritoneal shunt, 65 have presented an abdominal complication. Some of them are rare and easily explainable: 2 intra-operative viscus perforations, 1 intestinal obstruction, 3 extrusions of the catheter into an inguinal hernia. Also rare, C.S.F. ascites (5 cases) et C.S.F. pseudocysts (3 cases) presented with typical symptomatology easily diagnosed, but their etiology is not clear. The main complications are peritonitis without perforation (32 cases) and migrations of the catheter in the colon (19 cases): their etiology is an intra-operative bacterial contamination, they happen during the following weeks or months after the shunt procedure. They are associated with 8 among 9 deaths of our data by gram negative germ meningitis. Removal of the peritoneal catheter and temporary external shunt are their main treatment.

[Retroperitoneal hamartoma. Apropos of 6 cases].

Retroperitoneal hamartoma is an exceptional, benign, malformative tumor (1, 2% among retroperitoneal tumors in children). The authors report 6 cases which have been observed at the C.C.I. (Hôpital des Enfants Malades. Paris) for a 15 years period. This tumor has been revealed, generally in 3 years old children, by various clinical pictures. Echography can confirm the liquid contents of the tumor and her retroperitoneal sit. Surgical but non aggressive complete remove leads to recovery without any sequela.
