The effect of polylactic acid granules on the incidence of alveolar osteitis after mandibular third molar surgery. A prospective randomized study.

OBJECTIVE: To evaluate the effect of polylactic acid granules placed in third molar sockets on the incidence of alveolar osteitis. STUDY DESIGN: A prospective randomized study with 161 patients (322 bilaterally impacted mandibular third molars). RESULTS: The sockets in which the polylactic acid granules were placed had a significantly higher rate of alveolar osteitis (23.6%) than did the control sockets (13.58%). In all subgroups analyzed, the incidence of alveolar osteitis was higher in the experimental (polylactic acid) group. CONCLUSION: On the basis of the findings of this study, polylactic acid granules do not reduce the incidence of alveolar osteitis and may actually contribute to a higher incidence of alveolar osteitis.

Surgical extractions.

The principles of exodontia as they pertain to surgical extractions are presented in this article. A system of presurgical analysis that will alert the dentist to potential problems is outlined. Also, classic techniques of surgical extractions and root retrieval are presented.

Oral mucosal biopsy procedures. Excisional and incisional.

Incisional and excisional biopsies of oral mucosal lesions are procedures the dental practitioner should employ for total patient care. Indications and contraindications for obtaining biopsy specimens are outlined in this article. Also, basic and adjunctive surgical techniques are described with an emphasis on a continuum of care until resolution.

Acetominophen toxicity. Report of two cases.

Both chronic low-dose use within accepted therapeutic limits and acute overdosage of acetominophen with as little as 7 Gm or 150 mg./kg. can result in irreversible centrilobular hepatic necrosis. Prompt and proper treatment with N-acetyl-cysteine in strict adherence to accepted protocol will prevent potentially fatal results. Clinicians who prescribe acetominophen must likewise be cognizant of other potential side effects, such as methemoglobinemia, hemolysis, and prothrombin suppression.

VECTAN II: A computer program for the spatial analysis of the vectorcardiogram.

This paper presents the operation of a digital computer program, VECTAN II, for the spatial analysis of the vectorcardiogram (VCG). The program incorporates a unique waveform recognition algorithm based on the spatial vector length which has been shown to perform better than previous algorithms. The waveform analysis employed by the program considers the vectorcardiogram as a three dimensional entity rather than as scalar or planar representations. VECTAN II is designed chiefly to measure and quantify the VCG response of normal subjects to a controlled stress by analyzing one VCG complex every five seconds throughout a long experiment. The program has been used to analyzeom the NASA Johnson Space Center Cardiovascular Laboratory, from the pre- and postflight medical examinations of the Apollo 15, 16 and 17 crewmen, and from onboard Skylab experiments.
