Simultaneous bilateral pyeloplasties.

Eleven patients with bilateral pelviureteric junction obstruction are presented. The operative technique for simultaneous bilateral pyeloplasty is briefly described and the results discussed. The importance of adequate extraperitoneal loin drainage is stressed and the incidence of subsequent adhesion obstruction is recorded.

Childhood actinomycosis. Report of 3 recent cases.

Three cases of childhood actinomycosis are reported, 2 with the commonest presentation of cervicofacial abscess and the third with a rarely reported superficial chest wall abscess. The importance of prompt bacteriological diagnosis and adequate treatment with surgical drainage and chemotherapy is stressed. Though in adults males are affected more frequently than females, the sexes are probably equally affected in childhood.

Simultaneous bilateral pyeloplasties.

A technique is described which gives excellent results in the treatment of unilateral hydronephorsis and allows for simultaneous bilateral pyeloplasties. The advantages for the patients with bilateral hydronephrosis are a single admission to hospital and a single anaesthetic and operative procedure. The exposure is superior to that achieved by a loin incision, allowing the surgeon to perform the pyeloplasties more rapidly and with less traction on the renal vessels. Over a follow-up period of 1-7 years no subsequent pyeloplasty or nephrectomy has been required and all 11 patients are well and symptom free.

Surgical presentation of toxic shock syndrome.

Three infants with clinical features of sepsis, hypovolaemia and an acute abdomen were referred to a paediatric surgical unit. Subsequent clinical signs of diffuse macular erythema followed by desquamation and isolation of Staphylococcus aureus from nasal or umbilical swabs led to a diagnosis of staphylococcal toxic shock syndrome. Surgical intervention was not indicated.
