Multiple bladder diverticula in Williams "Elfin-Facies" syndrome.

Twenty-three children with Williams "elfin-facies" syndrome were reviewed. A striking incidence of multiple bladder diverticula was found (8 children of 14 who had voiding cystourethrograms). The diverticula were usually not identifiable on excretory urography. Children generally have a low incidence of bladder diverticula (excluding "Hutch" diverticula), especially multiple diverticula; thus one should strongly consider the diagnosis of Williams syndrome and other connective tissue diseases when multiple bladder diverticula are identified.

Anatomic observations and etiologic and surgical considerations in choledochal cyst.

Twenty-five operative and postoperative cholangiograms in children with choledochal cysts indicated an intimate relationship between these cysts and anomalous pancreatico-biliary ductal junction. In all of these patients the distal main pancreatic duct (MPD) is excluded from the sphinctor of Oddi mechanism and free reflux of pancreatic juice up the biliary tree occurs. This leads to chronic cholangitis, which is presumed to be responsible for the formation of the choledochal cysts. Abrupt and intense exposure of the common bile duct (CBD) to pancreatic reflux may cause perforation and bile peritonitis. Long-standing exposure may induce malignant transformation in the choledochal cyst. Total cyst excision and separation of the biliary system from exposure to pancreatic reflux is suggested as the procedure of choice. Long-term results and metabolic and digestive consequences of this operation are yet to be determined.

99mTc-methylene diphosphonate imaging in neural crest tumors.

Thirteen patients with neural crest tumors were each studied with a radionuclide skeletal survey, a radiographic skeletal survey, and an iliac crest bone marrow aspiration. The RN skeletal survey proved to be more accurate that the radiographic skeletal survey in: 1) detecting the primary tumor; 2) demonstrating bony metastatic disease; and 3) evaluating response to therapy. The overall accuracy of the RN skeletal survey in detecting metastatic disease correlated with the results of bone marrow aspiration both in the initial detection of metastatic disease and the evaluation of response during therapy.

99M Tc-PIPIDA biliary imaging in children.

A report of 99mTc-PIPIDA as a diagnostic biliary agent in children is presented. The laboratory values and radiologic examinations are included to demonstrate the clinical value of the agent in evaluating both congenital and acquired pediatric biliary tract abnormalities. The role of 99mTc biliary tract imaging in children is discussed.

Pediatric radionuclide ventriculography.

Computed tomography is the standard diagnostic examination employed to identify the morbid anatomy of cerebrospinal fluid spaces in children. This noninvasive technique provides excellent anatomic information. However, CT only indirectly characterizes the cerebral spinal fluid pathways. When it is necessary to determine precise bulk cerebrospinal fluid flow for surgical correction, then radionuclide ventriculography (RNV) is a useful adjunctive procedure.

Tc-99m-IDA hepatobiliary imaging: a fractured liver.

The authors describe the scintigraphic appearance of a fractured liver using a Tc-99m-IDA derivative. Tc-99m-IDA imaging provides both functional and morphologic information which is necessary in evaluating biliary tract trauma.
