Control of masseter muscle tension during sleep.

This study evaluated whether muscle tension could be controlled during sleep. Twelve volunteers were assigned to an experimental condition or to a no treatment control group. Experimental subjects were instructed to reduce the tension in their masseter (jaw) muscle when signalled with an auditory tone. These subjects demonstrated cue-controlled tension reductions during awake training sessions. Although both groups evidenced muscle tension increases in response to the tones presented during sleep, experimental subjects produced significantly smaller increases in response to tone stimuli than did control subjects. More importantly, experimental subjects sustained lower tonic levels of muscle tension throughout the sleep sessions than did control subjects. Sleep was moderately disrupted for subjects in both groups. However, experimental subjects evidenced the greatest tension reductions during those responses in which they shifted to a lighter stage of sleep rather than fully awakening. These data suggest that muscle tension can be moderated during sleep.

Power of the affected-sib-pair method to defect disease susceptibility loci of small effect: an application to multiple sclerosis.

The distribution of marker locus identity-by-descent scores in affected sib pairs provides a powerful tool for detecting the presence of a linked non-Mendelian disease susceptibility locus. This basic approach is here extended to include a trio of sibs. A special type of sib trio consisting of two affected and one unaffected sib is investigated. It is shown that compared to affected-sib-pairs, trios with the above configuration are less efficient in detecting the presence of a linked disease susceptibility locus. When the generalized two-allele single locus model is fitted to sib pairs affected with multiple sclerosis, an estimate of the recombination fraction of 0.21 between the putative disease susceptibility locus and the HLA complex is obtained. However, this transmission model is deemed inadequate since a recombination fraction this large is inconsistent with the variety of HLA associations observed at the population level.

Head position, intracranial pressure, and compliance.

The effect of head position on intracranial pressure (ICP) and intracranial compliance was determined in 19 consecutive ICU patients. Ten had lower ICPs with the head raised 60 degrees, two were lower at 0 degrees, and seven were unchanged. Compliance improved with head elevation in five patients, improved with head lowering in four, and was unchanged in 10. The use of subarachnoid screw devices for compliance measurements was validated by simultaneously recording intraventricular and subarachnoid pressures in four patients. Optimal head positioning for patients with raised ICP should be established individually rather than routinely caring for patients with the head elevated.

Development and evaluation of an ultrasonic ground water seepage meter.

Submarine ground water discharge can influence significantly the near-shore transport and flux of chemicals into the oceans. Quantification of the sources and rates of such discharge requires a ground water seepage meter that provides continuous measurements at high resolution over an extended period of time. An ultrasonic flowmeter has been adapted for such measurements in the submarine environment. Connected to a steel collection funnel, the meter houses two piezoelectric transducers mounted at opposite ends of a cylindrical flow tube. By monitoring the perturbations of fluid flow on the propagation of sound waves inside the flow tube, the ultrasonic meter can measure both forward and reverse fluid flows in real time. Laboratory and field calibrations show that the ultrasonic meter can resolve ground water discharges on the order of 0.1 microm/sec, and it is sufficiently robust for deployment in the field for several days. Data from West Neck Bay, Shelter Island, New York, elucidate the temporal and spatial heterogeneity of submarine ground water discharge and its interplay with tidal loading. A negative correlation between the discharge and tidal elevation was generally observed. A methodology was also developed whereby data for the sound velocity as a function of temperature can be used to infer the salinity and source of the submarine discharge. Independent measurements of electrical conductance were performed to validate this methodology.

Competency assessment and proficiency testing.

Competency assessment is an ongoing, continuous process of monitoring individuals' abilities to perform their specific job functions. A variety of methods are useful in monitoring cytology competency, including rescreening studies, descriptive monitors (abnormality rates), discrepancy rates, workload patterns, competency-based educational programs and programs using unknown slide challenges. The goal of proficiency testing (PT) is to ascertain and assess the ability of individuals beyond the particular items or challenges presented. However, cytology PT faces many challenges for implementation as it cannot duplicate normal working conditions, and there is often no gold standard to define the truth. PT is just one measure of performance and should be considered in conjunction with other quality assessment monitors. There is no consensus on the value or validity of a large-scale regulatory PT program. Any regulatory PT program should be field tested prior to implementation, and the grading system should be scientifically defensible. Scoring of performance on PT should occur in a timely fashion, and there should be an opportunity for educational feedback. The ultimate aim of both competency assessment and PT is to positively affect laboratory procedures and improve the cervical cancer screening process.
