BACKGROUND: Ireland has been relatively free of sickle-cell disease (SCD) and a care policy for the disease has not been established. AIM: To determine the prevalence of childhood SCD in Ireland and to predict requirements for a comprehensive care and assessment programme. METHODS: We retrospectively analysed the data of children with SCD presenting with sickle-cell crisis to one institution from January 1999 to April 2001. We also determined the nature and severity of the presenting haemoglobinopathy phenotype. RESULTS: Ninety-two patients with haemoglobinopathy have been registered with the Paediatric Haematology Service. The majority are from Nigeria with a smaller number from Angola and the Congo. Sixty have sickle-cell trait, 23 SCD, four haemoglobin SC disease and two haemoglobin E (HbE). There have been 32 sickle-cell crises. The majority were haemolytic or splenic sequestration events with a smaller number of aplastic and vaso-occlusive events and one osteomyelitis. CONCLUSION: The increasing number of children presenting with SCD as a result of the increasing refugee numbers requires a comprehensive care approach similar to that required for paediatric haemophilia to ensure optimum care.
The metabolism of 17 alpha-[3H]hydroxyprogesterone was examined in female rat anterior pituitary and hypothalamic tissues. After reverse isotopic dilution analysis and purification to constant specific activity, the following 5 alpha-, 3 alpha- and 20 alpha-reduced products were detected in both tissues: 17 alpha-hydroxy-5 alpha-pregnane-3,20-dione; 3 alpha,17 alpha-dihydroxy-5 alpha-pregnan-20-one; 17 alpha,20 alpha-dihydroxy-4-pregnen-3-one and 5 alpha-pregnane-3 alpha,17 alpha,20 alpha-triol. While the metabolites formed were qualitatively the same, there were quantitative differences between the two tissues. The 3 alpha,5 alpha-reduced metabolite, 3 alpha,17 alpha-dihydroxy-5 alpha-pregnan-20-one, was the principal product in the anterior pituitary while the 5 alpha-reduced metabolite, 17 alpha-hydroxy-5 alpha-pregnane-3,20-dione, was produced in largest amount by the hypothalamus. With both tissues, the aforementioned four products plus starting substrate accounted for nearly all of the starting radioactivity. There was no evidence for the formation of C19 steroids (androgens) despite the presence of the 17 alpha-hydroxy group.
It is being increasingly recognised that nursing needs to be constructively and critically informed by a body of scientific knowledge, and that practitioners need to develop active and independent learning styles which enable them to access, use and evaluate such knowledge. This approach has led to many teachers reexamining their approaches to teaching and learning, with an increasing emphasis upon cultivating lifelong learning and in providing students with the skills to facilitate this process. It is against this backdrop that the authors developed a comprehensive information skills package for student nurses undertaking an ENB Diploma in Nursing Science/Registered General Nurse Course (ENB Pilot Scheme Course). The package was designed specifically for use within the Newcastle Polytechnic Library though with modifications, it could be used in other venues. This paper explores the rationale and structure of the package and its subsequent implementation and evaluation.

Evoked potentials were produced by anodal stimulation over the motor cortex in six dogs. Potentials were recovered from the cranial thoracic and caudal lumbar portions of the spinal cord, and the radial and sciatic nerves. Evoked potential averages were recorded every 1.5 minutes during 40 minutes of aortic occlusion and during 40 minutes of reperfusion. Mean amplitudes of evoked potentials recovered from the caudal lumbar spinal cord decreased to 50% of original values at minute 12.2. Upon release of occlusion, the evoked potentials returned to baseline levels and remained there throughout the period of reperfusion. Sciatic nerve amplitudes decreased to 50% of original values at minute 4.5. In no subject could wave forms be recovered after minute 9.0. Upon release of occlusion, the evoked potentials returned to baseline levels and above, then deteriorated to 29 +/- 12% after 40 minutes of reperfusion. We concluded that transcranially induced evoked potentials were highly sensitive to spinal cord ischemia. Evoked potentials recovered from the sciatic nerve were consistent with functional grey matter immediately upon reperfusion, but deteriorated during reperfusion.
There is a prevailing wisdom that the taking and recording of arterial blood pressure is a non-problematic nursing activity. However, closer scrutiny reveals that the practices surrounding blood pressure measurement are subject to rational and ritual actions that have considerable significance for nursing practice.
There is a high incidence of epilepsy among people with learning disabilities. However, there is no comprehensive strategy for epilepsy care and little research is carried out into health issues and learning disabilities.
Managed care has come under fire lately, and states and the federal government have stepped in to regulate some plans' deficiencies. Some say regulation is not enough; managed care is morally flawed. But the evils of managed care are the result of letting it be shaped solely by market-driven forces rather than mission-driven values. In the Catholic tradition, healthcare is part of the common good. Viewed in this light, managed care becomes more than just a way to control costs. For managed care to serve the common good, we will have to collaborate with other providers that demonstrate a commitment to human life and dignity that is similar to our own. Such collaborations may force us to negotiate (without compromising) our values, but this gives us the opportunity to recognize a hierarchy of goods to be pursued and evils to be avoided. Through our involvement in managed care, we can help shape the greater culture, as well as the culture of healthcare. But we must prioritize our commitments according to values and principles grounded in the Catholic moral tradition. Without these values to guide us, Catholic healthcare will lose its identity and fade away.
Duplicating the preoperative assessment kept the staff working parallel, instead of intersecting or collaborating. The process of promoting better communication via collaborative documentation required staff to reflect on their roles--not as members of the PACU or OR team, but as part of the group giving surgical care.
