[The cardiac manifestations in hyperthyroidism. The surgical implications].

In a statistics including 954 patients with hyperthyroidism [correction of Huprotoncoses] treated between 1966-1989, the authors found 522 cases (54) with various associated cardiac disorders. Of these cases, 199 presented rhythm disturbances: extrasystolic arrhythmia, auricular fibrillation and flutter to which 34 postoperative arrhythmias are added. Cardiac insufficiency present in 46 cases was the main complication and end point of the various myocardial conditions. Ischemic cardiopathy (181 cases), arterial hypertension (98 cases) and rheumatic valvulopathies (9 cases), either isolated or dominating the clinical picture, complete the nosological spectrum of these disturbances. The frequency of associated conditions and the absence of some specific morphologic lesions suggest that thyrotoxicosis is rather an aggravating factor although in many cases the presence of a previous cardiac disease is excluded. The two objectives in the management of thyrocardiac diseases are the amelioration of cardiac condition and an endocrine balance. In the conditions of a careful selection and preoperative preparation, surgery gave good results consisting, in this series, in over 70% cures and ameliorations.

Hydroelectrolytic changes caused by prolonged clinical recumbency.

Investigations effected in 16 subjects of 20-69 years at bed rest after bone fractures demonstrated, in an interval of 10-27 days after surgery, normal plasma sodium concentration and a decreased urinary elimination, an increased plasma potassium concentration but a decreased urinary output. The osmotic changes were not significant. The plasma vasopressin activity was increased. Three urinary aldosterone determinations showed an increased output. The serum calcium level was within normal limits but its urinary output was initially slightly decreased, contrary to its augmented elimination after simple voluntary bed rest, as results from literature. The hydroxyproline elimination was increased, demonstrating an active collagen metabolism.

Humoral immune response in human pulmonary tuberculosis: antibodies against Mycobacterium tuberculosis polysaccharide, protein and glycolipid antigens.

Sera from 67 tuberculosis patients and 30 healthy subjects have been analyzed for the presence of specific antibodies against polysaccharide, protein and glycolipid antigens from H37Rv strain of Mycobacterium tuberculosis. The results proved the advantages of using the glycolipid antigens in diagnosing tuberculosis as well as the relationship between the antibodies level and the extension of the pulmonary tuberculous lesions.

Clinical usefulness of rapid serodiagnosis in pulmonary tuberculosis by ELISA with glycolipidic antigens extracted from Mycobacterium tuberculosis and with whole BCG suspension.

Antibodies against Mycobacterium tuberculosis glycolipids and whole BCG suspension were determined by ELISA on 58 sera from hospitalized patients with presumptive pulmonary tuberculosis and on 127 sera from control subjects. The experimental results demonstrated that the glycolipids are more adequate to be used as antigens than whole BCG suspension, as high sensitivity of the test was obtained. By using only one antigen, ELISA becomes more efficient in rapid diagnosis. ELISA with glycolipidic antigens should be used as a supporting test for direct smear examination in the diagnosis of pulmonary tuberculosis, especially in cases when repeated bacterial methods failed to prove the presence of M. tuberculosis.

Serodiagnosis of tuberculous meningitis by enzyme-linked immunosorbent assay (ELISA).

IgG antibodies against glycolipids and proteins isolated from M. tuberculosis and BCG suspension were determined by ELISA in sera, in CSFs and in serum and CSF paired samples, from patients with tuberculous meningitis and from healthy control subjects. With specificities between 90 and 94% for the antigens used, we obtained senitivities of 75% for Pr-ELISA, 60% for G1-ELISA and 35% for BCG-ELISA. As specific antibodies were detected in serum and CSFs, only one sample is enough to perform the test. We concluded that Pr-ELISA and G1-ELISA could be used as a supporting test in TBM diagnosis, especially when repeated bacteriological methods failed to prove the presence of tubercle bacilli and in cases without evidence of pulmonary tuberculosis.

[The treatment of rectal cancer--the optimizing possibilities].

The authors studies 280 rectal cancers treated in the first surgical clinic of Iaşi in the period 1981-1994. Although surgery is the main treatment of these tumors, it cannot be and it must be not used alone. Surgery must be associated with radio- and chemotherapy, because is the only way possible to increase the number of resections for rectal tumors and the survival rate. For the 263 tumors operated the resectability was 73, 5%: Milles's operation 101 (36, 7%), Dixon's operation 81 (30, 79%), other operation 91 (33, 11%). We consider that Dixon's operation is the best choice for the tumors in the stages A and B of Dukes classifications. Preoperative radiotherapy can influence the tumor volume (downstaging) and decrease local recurrences to about 10-15%. Surgery alone must be practiced only in emergencies. Postoperative chemotherapy is mandatory and it reduces local recurrences and delays metastasis evolution.

Application of ELISA with antigenic glycolipids in early diagnosis of pulmonary tuberculosis.

Antibodies against M. tuberculosis antigenic glycolipids were determined by enzyme-linked immunosorbent assay (ELISA) in 80 sera from patients suspected of pulmonary tuberculosis, with suggestive clinical signs and radiological abnormalities, but smears negative. The test was also performed on 68 control sera from patients with bacteriologically confirmed pulmonary tuberculosis, in different stages of disease. We obtained the test sensitivity of 89.5% for the confirmed tuberculosis patient sera. Low levels of specific antibodies and the sensitivity of 75% were found when using ELISA in smears negative patients at the onset of clinical tuberculosis. The experimental results showed that ELISA could be used as a supporting test in early diagnosis of pulmonary tuberculosis.
