Flumazenil in ketamine and midazolam anaesthesia.

A double-blind, parallel group study using flumazenil and placebo was carried out to determine whether patients who received flumazenil would awake more quickly and whether this drug would reverse the protection conferred by midazolam on the psychic sequelae of ketamine. Fifty female patients were studied. The results showed that there was a significant reduction in awakening time (p = 0.02) and a very significant increase (p = 0.001) in the incidence of dreams in the flumazenil group.

Paediatric hand injuries and the galvanized-iron fence.

A review of all cases of operative hand surgery at The Adelaide Children's Hospital during the period January 1-June 30, 1988 yielded 38 cases that resulted from trauma (burns and fractures excluded). Lacerations from uncapped galvanized-iron fences occurred in 10 patients. Of these patients, two patients required skin grafts, seven patients had flexor-tendon injuries and three patients suffered digital-nerve lacerations. Twenty-five fingers were damaged. The pattern of injury involves multiple digits at different levels, with distally-based skin flaps, frequent divisions of flexor tendons and less-frequent nerve damage. This serious injury is common in South Australia but could be prevented easily by capping all iron fences.

Cost-effectiveness of free tissue transfer.

In a review of 61 patients with compound injuries to the upper and lower extremities. Operating Theatre costs were added to the costs of inpatient hospitalization after commencement of reconstruction with either a pedicled flap or free tissue transfer. The cost of pedicled groin flap to the upper limb and cross leg flap groups was double that of comparable free tissue transfer groups. The cost of abdominal tube pedicle to the lower limb was three times greater than for the free flap technique. The conclusion is drawn that apart from general advantages to the patient, the cost to the community is significantly reduced.

Critical flicker-fusion of solid and annular stimuli.

Critical flicker-fusion thresholds were measured with both solid disks and annular stimuli. The former subtended 1, 4, and 8 degrees of visual angle, and the latter subtended 4 and 8 degrees. The illuminated region of the 4-degree annulus provided the same area of stimulation as that of the 1-degree disk. The illuminated region of the 8-degree annulus provided the same area of stimulation as the 4-degree disk. Annular stimuli significantly reduced critical flicker-fusion thresholds relative to those obtained with disk stimuli of the same area. The effects are not explicable in terms of heterogeneous retinal sensitivity. Laws relating critical flicker-fusion to area seem to be valid only under the restricted condition of homogeneous stimulation.
