Characterization of clinical mastitis records from one herd in a subtropical environment.

Patterns of occurrence and reoccurrence of clinical mastitis, duration of episodes, and number of quarters treated in a large north Florida dairy over 6.5 yr are described. Monthly incidence of milk disposal due to clinical mastitis varied within and between years and ranged from 1.3 to 4.7% of cow-days in milk. In first parity animals, 37.6% of lactations were treated for clinical mastitis, averaging 1.4 episodes per treated lactation. Almost half of these episodes occurred by d 35 of lactation. For second, third, and fourth or greater parities, 35.5, 48.1, and 56.6% of lactations were treated. These lactations averaged 2.1, 2.3, and 2.8 episodes. In parities 2, 3, and greater than or equal to 4, episodes initiated by d 35 of lactation were 28.2, 23.9, and 20.6% of parity episodes. Seventy-two percent of episodes were treated for less than or equal to 7 d. Sixty-seven percent of episodes were treated in one quarter and treatment in two, three, or four quarters accounted for 21.6, 5.5, and 5.9% of all episodes. When two quarters were treated simultaneously, udder halves (front, rear, right, left) were treated more frequently than were diagonals.

Antegrade and retrograde infusion of cardioplegia: assessment by thermovision.

The efficacy of two routes of cardioplegia infusion was examined by assessing the hypothermia induced in patients with critically obstructed or occluded major coronary arteries. The antegrade (through the aorta) and the retrograde (through the coronary sinus) methods of cardioplegia infusion were compared using myocardial thermograms. Patients (N = 8) were matched according to angiographic similarities of their coronary artery disease. Adequate myocardial cooling distal to a critical obstruction could frequently not be obtained with antegrade infusion of cardioplegic solution. With retrograde infusion, the desired uniform cooling of the myocardium, as determined by thermographic analysis of the surface temperatures of the heart, was obtained. We conclude that retrograde infusion of cardioplegic solution induces more effective and homogeneous hypothermia in patients with critically obstructive multivessel coronary disease, and may provide improved myocardial protection.

Occurrence and reoccurrence of clinical mastitis.

Clinical mastitis records for 6.5 yr (July 1977 through November 1983) from a large north Florida dairy were analyzed. Observed frequencies of clinical mastitis were calculated in 7240 Holstein and Jersey lactations. Least squares analyses used Holstein and Jersey lactations greater than 200 d and limited maximum parity to 5 (6732 lactations, 5738 episodes). Solutions for number of occurrences of clinical mastitis adjusted for month of parturition and breed effects were .47, .59, .94, 1.27, and 1.50 for parities 1 through 5. Solutions for number of occurrences across lactation in Holsteins was greater than for Jerseys (1.28 vs. .63). Solutions for occurrence of first episode in Holsteins increased from .35 in parity 1 to .71 in parity 5. Occurrence of episodes two through five increased as parity increased. Similar trends were found in Jerseys with the exception of parity 2. Correlation of residuals between specific episodes increased as episode increased. From episode 2 through 5 probability of another occurrence of clinical mastitis was greater than .75. Solutions for proportion of occurrences of clinical mastitis occurring during the first 35 d postcalving were .60, .36, .31, .30, and .28 for parities 1 through 5. Clinical mastitis in first parity cows occurred primarily during the first 35 d postcalving.

Factors affecting days of discarded milk due to clinical mastitis and subsequent cost of discarded milk.

Records of clinical mastitis for 6.5 yr from one Florida dairy with 1050 to 1350 cows milking per month were used to predict duration of occurrence of clinical mastitis and to estimate dollar value of discarded milk per lactation with occurrences of clinical mastitis. Only 6.1% of lactations accrued more than 28 d of discarded milk and were responsible for 52.7% of days of discarded milk. Accurate maintenance of herd mastitis records enables calculation of actual days of milk available for sale and provides a useful tool for culling and treating decisions. Duration of each occurrence of clinical mastitis was modeled by fixed effects of breed, season of occurrence, current lactation history of mastitis, number of quarters with clinical signs, and average maximum temperature-humidity index for the 2 d prior to the occurrence (to second order). These effects explained 1.8% of observed variation in duration of occurrence. Random effects of cow explained 5 to 6% of variation. Prediction of duration of occurrence had a mean value of 6.6 d. When economic status of individual animals is evaluated, comparison of days of milk production available for sale is preferable to total days of milk production. Estimated average cost of discarded milk for parities 1, 2, 3, 4, and 5 ranged from $29.73 to 223.98 per lactation.

Detection of pheromone biosynthetic and degradative enzymes in vitro.

Highly sensitive, luminescent assays have been developed to measure enzyme activities involved in the metabolism of a major class of insect pheromones which includes fatty aldehydes, alcohols, and their acetate esters. These assays have been applied to measure the in vitro biosynthesis and degradation of the sex pheromone (trans:cis-11-tetradecenal, 96:4) of the eastern spruce budworm, Choristoneura fumiferana. Three activities were detected on analyses of extracts of the female moths: (a) an esterase that hydrolyzes both the cis and trans isomers of 11-tetradecenyl acetate, (b) an oxidase that converts fatty alcohols to aldehydes in the presence of O2, and (c) an NAD-dependent aldehyde dehydrogenase. The coupled luminescent response of bacterial luciferase to long chain aldehydes was used to measure rates of reaction as low as 0.1 pmol/min since only low amounts of material can be analyzed. Specific activities of these enzymes were higher in the pheromone producing gland than in other parts of the moth, implicating these enzymes, and the oxidase in particular, in the pathway of pheromone biosynthesis. The pathway was supported in vivo by demonstrating that topical application of 3H-labeled tetradecanyl acetate onto the insect gland resulted in the formation of [3H]tetradecanol and [3H]tetradecanoic acid, thus providing evidence that all three enzymes were functional in the living insects.

Emergency guide wire pacing: new methods for rapid conversion of a cardiac catheter into a pacemaker.

We developed a new electrode to convert rapidly a previously inserted pulmonary artery or left ventricular catheter into a pacemaker. One method of doing this is by withdrawal of the pulmonary artery catheter from the pulmonary artery to the right ventricle by pressure control, and a Teflon-coated guide wire, stripped of 5 mm of insulation at its tip, is advanced through the catheter to contact the endocardium. In the second method, the pacing electrode is advanced through the distal lumen of the catheter while it is positioned within the pulmonary artery and withdrawn into the right ventricle while pacing. Finally, a third method involves advancement of the guide wire electrode into the left ventricle through a pigtail catheter. To pace, the guide wire electrode is connected to the cathode of a pacemaker referenced to a skin electrode. We paced 10 of 10 right heart cardiac catheterization, intra- and postoperative surgery patients by methods 1 and 2, and 4 of 4 left heart catheterization patients by method 3. Thresholds (mean +/- SEM) for guide wire pacing were: right ventricle 1.52 +/- 0.4 mA; left ventricle 1.33 +/- 0.1 mA. Guide wire pacing is rapid, reliable, and requires little operator skill. Our indications for guide wire pacing are: 1) emergency right ventricular pacing in operative or intensive care unit patients with unexpected bradyarrhythmias who have an indwelling pulmonary artery catheter; and 2) emergency left ventricular pacing in left heart cardiac catheterization patients with contrast-induced bradyarrhythmias.

Transvenous pacemaker insertion with a zero dislodgement rate.

A non-retractable transvenous screw-in lead which gives both stability of placement and low acute and chronic stimulation thresholds was used in 64 patients during the period April, 1979 to June, 1981. The average threshold at implant was 0.7 volts; the current threshold average was .86 milliamps with the pulse width at the standard setting for the pacemaker employed. Chronic thresholds were usually below the lower programmable limit of the pacer as tested at 3 months. In one patient, the fact that the screw was fixed in the exposed position caused trouble. The lead became knotted in the superior vena cava and was removed by thoracotomy. Although this experience with the Osypka lead was not entirely satisfactory, newer developments with retractable leads may make the principle more acceptable.

Microbiology of a major foodborne outbreak of gastroenteritis caused by Yersinia enterocolitica serogroup O:8.

Gastrointestinal disorders of varying severity were observed in 239 (53%) of 455 campers and staff members at a coed summer camp in Sullivan County, New York, during July 1981. Five of seven hospitalized patients had appendectomies before the disease was recognized as yersiniosis. Yersinia enterocolitica serogroup O:8 (American strain) was isolated from 37 (54%) of 69 persons examined, including the head cook and 3 others of the 11-person kitchen staff. Of 48 food, water, and environmental samples collected from the camp area, Y. enterocolitica isolates belonging to the same serogroup and biogroup as the human isolates were recovered from dissolved powdered milk, a milk dispenser, and turkey chow mein. This laboratory finding supported the epidemiological data indicating a correlation between consumption of these foods and illness. Y. enterocolitica isolates of the same biogroup as the O:8 isolates but belonging to serogroup O:34 were also isolated from six campers and two samples of dissolved powdered milk. Pathogenicity studies on the Yersinia isolates were performed with three in vitro tests (calcium dependency, autoagglutination, and HeLa cell infection) and one in vivo test (intraperitoneal challenge of mice). Most of the serogroup O:8 human isolates and the chow mein isolate were positive in all four tests. Milk isolates of serogroup O:8 were positive in the in vitro tests but were relatively avirulent in mice, whereas serogroup O:34 isolates, regardless of source, were negative in all four tests.

The effect of intravenous aprotinin (Trasylol) on intraperitoneal adhesion formation in the rat.

A randomized controlled trial to assess the effect of intravenous Trasylol on postoperative intraperitoneal adhesion formation has been performed in rats. Before undergoing a standard left colonic resection, animals received i.v. Trasylol 10 000 KIU. This dose was repeated 8-hourly for 3 days postoperatively. Control animals received saline placebo. The mean percentage circumference of the anastomosis covered with adhesions in the Trasylol group animals was 20.86 +/- 6.08 compared with 45.24 +/- 5.97 in the control group (P less than 0.01). Significantly more animals treated with Trasylol were completely free of adhesions than those receiving saline. Bursting pressures did not differ between the two groups. Intravenous Trasylol clearly inhibits intraperitoneal adhesion formation in this model.

Charge levels in programmable pacemakers.

The method of calculation of charge in one series of programmable pacemakers is described. Accurate charge tables are constructed and examples of their use are given. With the tables the margin between the threshold and the pacemaker setting can be more precisely determined and consequently, at least in some cases, it can be reduced for greater pacemaker longevity.

Longevity comparisons among lithium anode power cells for cardiac pacemakers.

Of over 10,000 lithium-powered pacemakers followed by a commercial transtelephone monitoring service, 148 have been explanted due to rate decline. A breakdown according to the power cell manufactures shows that the longevity is not uniform, indicating that significant differences exist among the power cell designs even though all have lithium anodes. One tenth of all lithium-powered pacemakers in the study were explanted for rate declines within 4 years.
