[Trend changes in the adenotonsillar surgery].

UNLABELLED: In order to evaluate possible trend changes in the epidemiological factors of adenotonsillar surgery, a total of 1,958 operations performed between 1992 and 2002 were reviewed. RESULTS: One third of the operations were to remove tonsils in comparison with the two thirds that were to remove adenoids. Three procedures were used: 59% adenoidectomies, 28% adenotonsillectomies and 13% tonsillectomies. During the study period, the percentage of adenotonsillectomies decreased, whereas the number of adenoidectomies increased. In general, this kind of surgery is mainly performed on males. CONCLUSIONS: Although we have not discovered variations regarding gender and average age for the operations, adenotonsillar surgery has increased over recent years and the percentage ratio of the various procedures has altered as a result of new indications.

[Childhood nasal sinus mucoceles. Review and report of a new case].

The appearance of mucoceles of paranasal sinuses during infancy is very rare. In relation with a patient, 11-year-old, enduring an ethmoidal mucocele treated in our Department we benefit it in order to realize a review of the published cases in medical literature. We also discuss on the etiopathogenesis, clinic and differential diagnostic aspects in pediatric age in comparison of those verified in grown-up.

[Giant antro-choanal polyp].

Antrochoanal polyps are, basically, pathologies of children and young adults. The case here reported is really exceptional because its size and its appearance at so unusual age. Etio-pathogenesis, clinical course and differential diagnosis, face to other pathologies, are perused and also with several varieties of choanal polyps and the treatments proposed as well.

[Neck schwannomas].

Given the rareness of cervical schwannomas, they are often mistaken for other kinds of lesions. We present 2 clinical cases, make comments on the clinical-pathological features involved and point out the essential elements that ensure the differential diagnosis of these tumours.

[Retro-inframandibular approach to tumors of the deep parotid lobe].

Two cases of pleomorphic adenoma of the parotid deep lobe are presented. Both of them were removed through back-submandibular approach. We favor this procedure as the best for tumors of the parapharyngeal space, whereas the mandibulotomies are indicated when dealing with strong adhesions, reoperations or malignancies.

[Oropharyngeal tuberculosis. Report of a case in a lingual tonsil].

Pharyngeal tuberculosis is very rare and normally appears as an seemingly malignant process. As a result of which it is essential to perform a differential diagnosis before beginning anticancer treatment. We present a clinical case of lingual tonsil tuberculosis. A review is made of the pathogenic mechanisms, the clinical manifestations and the diagnosis of pharyngeal tuberculosis, whose response to treatment with antituberculous drugs proves to be excellent.

[Malignant rhinosinusal inverted papilloma with orbital involvement].

Contribution with the case of a woman 55-year-old in whom was objectified the recurrence of an inverted rhinosinusal papilloma and, under the anatomopathological viewpoint, an epidermoid carcinoma on the inverted papilloma. Was treated with surgery and radiotherapy. Remarks on several clinical, pathological and therapeutic aspects about inverted rhinosinusal papillomata, underlying its uncertain course so characteristic of these neoformations.

[Malignant fibrohistiocytoma in otorhinolaryngology. Review of two cases].

Fibrohistiocytomas are soft-tissue tumors of histiocytic origin that have a variety of histological patterns. Although cases of malignant fibrohisticytoma (MFH) of the head and neck have been reported with increasing frequency in recent years, they can be considered rare. However, they probably are more frequent than thought because of the different designations that the disease has received and confusion among clinicians and pathologists regarding these tumors. We report two cases of MFH of the tonsil and larynx, respectively. The histopathological diagnostic criteria, therapeutic indications, and prognosis evaluation are described.

[Rhino-orbital-cerebral mucormycosis. Review, update and report of a new case].

Mucormycosis is a fungal infection that often is fatal. It usually affects diabetic or immunocompromised patients. The site of onset generally is the nasal mucosa and the disease extends rapidly to the palate, orbit and brain. Early diagnosis is important and is based on the clinical features (diabetes, black eschar in nose, palate or face) and demonstration of the characteristic hyphae in the biopsy of the nasal mucosa or oral lesions. Treatment is based on systemic antifungal therapy, radical surgery, and medical management of the underlying disease. A case of rhino-orbital-cerebral mucormycosis is presented and the world literature is reviewed.

[Angioneurotic edema secondary to antihypertensive therapy].

Secondary angioneurotic oedema due to antihypertensive drugs of the angiotensin-converting enzyme inhibitors type, is an uncommon entity which must be remembered when dealing with differential diagnosis of this type of oedema. We have reviewed the subject following 2 of our own cases.

[Tracheocele: a case report].

Report of one tracheocele case casually uncovered in a man 51-year-old. Starting from the difference between congenital and acquired cases of this pathology the AA. refer to the symptomatologic picture, to diagnostical and therapeutical measures as well, pinpointing to possible complications that may arise specially by infants during the lactation period.

[Retropharyngeal abscess due to atypical mycobacteria. Case report].

It is a rare clinico-pathological infection located in the retropharyngeal space. Our contribution refer to a young man suffering this complaint caused by atypical mycobacteria, being the aim of the paper to underline the exceptional placement. The difficulties encountered in establishing the bacteriological diagnosis as well the excellent response to a combined treatment with tuberculostatics and surgery.

[Syndrome of multiple nevoid basal cell carcinoma].

The paramount features of the multiple nevoid basal Carcinoma, the so-called Gorlin's syndrome, are the presence of multiple maxillary cysts (odontogenic keratocysts), several nevoid basal cells carcinomata and some skeletal anomalies. Since its first description, due to Jarish (1894), at least about 250 cases have been published. The odontogenic keratocysts present themselves with ENT symptoms, specially when arising on the upper maxillary bone or on the homonymous sinus. Owing to this reason, the AA. of the paper, recall the clinic aspects of the condition.

[Leiomyoblastoma of the tongue].

Leiomyoma is a benign tumor of smooth muscle origin that is rarely found in oral cavity. We report a case of leiomyoblastoma of the tongue. The interest of this lesion is supported by its location and by the difficulty to determine its biological behavior. About 25% of the oral smooth muscle tumors are malignant, and have been reported "leiomyoma" locally invasive.

[Adenoid cystic carcinoma of the larynx].

The adenoid cystic carcinoma represents less than 1% of all the malignant tumours located in the larynx. Only about 80 cases have been described so far in world literature. In contrast with the epidermoid carcinoma of the larynx, the adenoid cystic carcinoma is not apparently related to common risk factors such as smoking. However the prognosis is much worse, with survival rates of less than 20% after 15-20 years. Treatment is surgical, although radiotherapy may be of help. In this paper a new case of adenoid cystic carcinoma located in the supraglottis of a 26-years-old woman is reported and the world literature is reviewed.
