Hepatitis A antibody prevalence among young adults in Israel--the decline continues.

This study sought to determine whether the decline in prevalence of hepatitis A virus (HAV) antibodies detected in Israel in 1977, 1984, and 1987 has continued. The anti-HAV antibody prevalence of a systematic sample of 578 male and female recruits inducted into the Israel Defence Force in 1996 was 38.4%. The reduction in antibody prevalence from 1977 (64%) was highly significant (P < 0.001). There was a smaller decrease rate in recruits of European, North American, Australian and South African origin than from elsewhere. A 'strategy' that uses active immunization against hepatitis A (inactivated vaccine, instead of gamma globulin) should be considered, particularly in high risk groups such as field units during military service.

The epidemiologic trends of scabies among Israeli soldiers: a 28-year follow-up.

BACKGROUND: Scabies is not a notifiable disease in most countries. Thus, the reported rates of the disease in large populations are usually inaccurate and based on estimations. Scabies is usually reported only when large outbreaks occur. OBJECTIVE: This article describes the global epidemiology of scabies in the Israel Defense Force. The data used in this study are based on the routine and mandatory reporting of every individual case of scabies to the Army Health Branch Epidemiology Department since 1968. RESULTS: There was a period of 13 years of quiescence from the implementation of reporting in 1968 until 1981. This period was followed by an epidemic of 15 years, peaking in 1985 and 1986, and returning to the baseline rates of the quiescent period by 1996. This prospectively observed pattern of at least 13 years of quiescence followed by a peak of 15 years is consistent with other, mostly retrospective, reports. Although the peak may have been triggered by the "Lebanon" war of 1982, it is noticeable that no peak was observed during the "Yom Kippur" war (1973). CONCLUSIONS: Prospective, mandatory reporting of the kind described here should help to better understand the epidemiology of scabies.

Field treatment of snakebites in the Israel Defense Forces.

Eight species of venomous snakes capable of inflicting bites dangerous or fatal to humans inhabit the State of Israel. Soldiers in the Israel Defense Forces (IDF) routinely serve under field conditions, and are at greater risk for contact with venomous snakes than are their civilian counterparts. Every year scores of military personnel are bitten by poisonous snakes. The IDF has developed a protocol for field-level treatment of snakebite victims, which we present in this article. Employment of a number of simple therapeutic measures for early treatment of snakebite can prevent or significantly reduce venom-induced morbidity and mortality.

Is the coexistence of mutations in the genes of factor V and MTHFR a predisposing factor for massive skin necrosis due to loxoscelism?

A 28-year-old previously healthy man was diagnosed as having an extensive necrotic lesion of his calf due to loxoscelism. One year later he was diagnosed as having co-inheritance of mutations in factor V and methyl tetrahydrofolate reductase (MTHFR). This is the first report of a possible etiologic connection between loxoscelism necrotic lesions and thrombogenic diseases.

Occurrence of supernumerary nipples in children with kidney and urinary tract malformations.

The reported data on the association of kidney and urinary tract malformations with supernumerary nipples are contradictory. We examined 200 children, ages 1 month-16 years, who were being followed because of recurrent urinary tract infection for supernumerary nipples. The patients were divided into two groups: those who were found to have urinary tract malformations on radiographic studies (n=92) and those who were not (n=108). All children were examined for any abnormal pigmentation along the milk line, and the entire body was examined for ectopic supernumerary nipples. Two of the children with proved urinary tract pathology and two of the children with no urinary tract pathology had supernumerary nipples. The odds ratio for having supernumerary nipples among the first group was 1.18 (95% CI: 0.084-16.53, p=0.627). Our results indicate no association between kidney and urinary tract malformations and supernumerary nipples. We believe the message to the practicing physician is that there is no need for radiographic or ultrasonographic investigation of the urinary tract in asymptomatic children found to have supernumerary nipples on routine physical examination.

Wegener's granulomatosis with cardiac involvement masquerading as Lyme disease.

Cardiac conduction disease is an infrequent complication of Wegener's granulomatosis (WG). We describe a case demonstrating an association between WG and complete atrioventricular dissociation. This manifestation was initially interpreted as Lyme disease based on these cardiac findings, arthritis, myalgias and positive Lyme serology. The clinical overlap between these disorders and the appropriate use of respective serologies is discussed.

New insights into the autoantibody-mediated mechanisms of autoimmune bullous diseases and urticaria.

The skin is a common target of cellular and/or antibody mediated pathological immune responses. Pemphigoids, pemphigus vulgaris and dermatitis herpetiformis are bullous disease due to autoantibodies targeting specific proteins of the skin. The pemphigoid autoantigens are the BP180 and the BP230 antigens, two components of the epithelial basement membrane zone. Additional antigenic targets reported in a portion of patients are laminin 5, the alpha6 subunit of the hemidesmosomal integrin alpha6beta4 and a glycoprotein termed p200. The epidermal and mucosal epithelial cells detachment (acantholysis) characteristic of pemphigus vulgaris is induced by autoantibodies directed against the desmoglein 3 and 1. The desmogleins are desmosomal cadherins, which play a major role in the cell-to-cell adhesion. Dermatitis herpetiformis is regarded as cutaneous phenotype of coeliac disease. A novel autoimmune hypothesis of coeliac disease links wheat gliadin and tissue transglutaminase (TG2) in the gut, which leads to T cell response and IgA autoantibody formation. In dermatitis herpetiformis skin the target for IgA deposition seems to be epidermal TG3. Urticaria is a complex syndrome caused by both immune and non-immune mechanisms. In a subsets of patients with chronic urticaria mast cell degranulation is induced by autoantibodies directed against the a-subunit of the high-affinity IgE receptor, and/or the IgE.
