Serratia marcescens in a general hospital.

Fourteen patients with infections caused by Serratia marcescens were seen over an eight-month period in a large general hospital. Predisposing factors suggested an 'opportunistic' pattern similar to that previously described in the United States. S. marcescens is an important pathogen which may be increasing in significance in the United Kingdom. Multiple resistance of the organism to antibiotics other than gentamicin makes the finding of apparent sensitivity to co-trimoxazole of potential therapeutic value.

Bacterial endocarditis in England in the 1970's: a review of 70 patients.

The features of 70 cases of bacterial endocarditis are reported. Streptococcus viridans was the cause in 45 per cent, staphylococci in 27 per cent, and enterococci in 7 per cent. Rheumatic heart disease was the predisposing factor in less than one quarter of patients and in almost half there was no obvious pre-existing cardiac disease. Apart from cardiac murmurs and pyrexia, the classical features of infective endocarditis were uncommon, haematuria being observed in less than one-third. Most patients were over the age of 30, one-third more than 60 years of age and the peak incidence occurred in the seventh decade. The mortality was 34 per cent, being highest in infection caused by Staphylococcus aureus. Adverse prognostic features included cardiac failure, advanced age and peripheral embolization. One-third of infections were caused by bacteria resistant to penicillin.

Mecillinam: a new antibiotic for enteric fever.

Mecillinam is a new antibiotic related to the penicillins but more active than ampicillin against salmonellae, including Salmonella typhi. Mecillinam must be administered parenterally, but the ester, pivmecillinam, is absorbed from the gut. Eight patients suffering from typhoid fever and one suffering from paratyphoid fever were treated with the antibiotic, and seven responded satisfactorily. One patient could not tolerate pivmecillinam because of vomiting but there were no other adverse reactions. Serum and bile levels of mecillinam were many times the minimum inhibitory concentrations for most salmonellae. The antibiotic is a promising addition to the agents available for treating typhoid.

Grouping of beta-haemolytic streptococci with group-specific antibodies absorbed to staphylococcal protein A.

A total of 245 isolates of beta-haemolytic streptococci was grouped by the counter current immunoelectrophoresis method and by co-agglutination with antibody-coated staphylococci. Co-agglutination was found to be simple and accurate, and many commercially available grouping sera were suitable for use in this test, though some of them had to be absorbed to remove cross-reactions.

Stimulation of growth on a deficient medium of a Klebsiella aerogenes species by sulphonamide, para-amino benzoic acid, nitrofurantoin, and some other substances.

A strain of Klebsiella aerogenes is described which was unable to grow from light inocula on 5% lysed horse blood diagnostic sensitivity test (DST) agar. It was able to grow on DST agar without added horse cells and MacConkey and nutrient agar. Growth on a deficient medium was promoted by sulphonamides and nitrofurantoin as well as by para-amino benzoic acid, para-amino salicylic acid, procaine hydrochloride, thymine, and uracil.
