[Serum and urinary antibodies in pyelonephritis (author's transl)].

Pyelonephritis is accompanied by complex immunological reactions, the study of which is just now being undertaken. Study of 216 patients with a significant monomicrobial urinary infection showed that in more than 95 per cent of cases of Gram negative infection the urine contained IgG type antibodies, often associated with IgA. These antibodies may be easily detected using an immunofluorescent technique, which would appear to be the best method available at present for distinguishing between pyelonephritis and an infection of the lower urinary tract. They may also be found in the serum, though less frequently. Detection by immunofluorescence is markedly more sensitive than using the classical passive haemagglutination technique. The biological significance of these antibodies remains imprecise, and their protective or facilitating role may be discussed. Their specificity must be determined, in order that sequential study of these immunological reactions may be of help in the conduct of treatment.

Antibodies in Escherichia coli urinary tract infection.

In 105 adults with E.Coli urinary tract infections, IgG coated bacteriuria was found in 8/9 with acute pyelonephritis (PN), 17/20 with chronic PN, and in only 2/76 with lower UTI. IgA was present in 66% of PN, but IgA secretory piece in less than 10%. These urinary IgG antibodies were, at least in part, synthesised in the kidney because serum IgG antibodies were detected by indirect immunofluorescence in only half the patients. 06, 018, 022 E.Coli serotypes were the three most frequently found O groups, but their prevalence in PN is not significant. The immunology of urinary tract infection (UTI) is still a subject of little interest in adult nephrology; but antibody production is a well-characterised event in pyelonephritis (PN), the study of which seems to be the best indirect procedure for localising the site of UTI. We have analysed the production of urinary and humoral antibodies, and their correlation with E.Coli serotypes in patients with E.Coli UTI.

Detecting enteropathogenic Escherichia coli strains of porcine origin. 1. Correlations between O and K antigens and the enterotoxin production in strains isolated from the newborn piglet.

A study of enterotoxin production by means of biological tests (Y1 adrenal cells and suckling mice) using 67 Escherichia coli strains responsible for diarrhea in newborn piglets, confirmed the advantages of these methods and demonstrated the correlation between antigenic specificity and enterotoxin production. In particular the presence of capsular antigen K 88 is related to the thermolabile (LT) fraction of the enterotoxin in 73.9 % of the enteropathogenic strains studied.

Sclerosed liver hemangioma mimicking malignant tumor at MR imaging: pathologic correlation.

This case report illustrates atypical magnetic resonance (MR) imaging findings in a liver hemangioma mimicking a malignant lesion--lower signal intensity than cerebrospinal fluid on T2-weighted spin-echo images and lack of early enhancement on dynamic contrast material--enhanced gradient-echo images. Pathologic analysis demonstrated nearly total replacement of the vascular cavities by dense fibrous tissue. In this rare, sclerosed form, this lesion could not be defined as a hemangioma with MR imaging.

Hyperintense benign liver lesions on spin-echo T1-weighted MR images: pathologic correlations.

BACKGROUND: To determine the incidence of hyperintensity on T1-weighted spin echo (SE) images in benign liver lesions, value of fat-suppressed magnetic resonance (MR) imaging for the detection of fat within these lesions, and the causes of hyperintensity by correlation to pathologic examinations. METHODS: Five hundred forty-nine patients with 805 benign liver lesions including 585 hemangiomas, 188 focal nodular hyperplasias (FNHs), 14 hepatic adenomas (HAs), 14 focal fatty infiltrations (FFIs), two biliary cystadenomas, and two hemorrhagic cysts were examined by T2-weighted and T1-weighted SE MR imaging. For hyperintense lesions on T1-weighted SE images, fat-suppressed images were obtained by selective presaturation of fat. RESULTS: Thirty-two lesions (four FNHs, 10 HAs, 14 FFIs, two biliary cystadenomas, and two hemorrhagic cysts) appeared hyperintense on T1-weighted SE images; 21 of these became hypointense on the fat-suppressed T1 weighted SE images (one FNH, six HAs, and 14 FFIs) and contained fat at pathological examination. The other 11 lesions remained hyperintense on fat-suppressed T1-weighted SE images and had no fat deposition. Causes of hyperintensity in these cases were sinusoidal dilatation, copper deposition, hemorrhage, and high protein content. CONCLUSION: Among benign liver lesions, hyperintensity on T1-weighted SE images is rare (3.9%). Causes of this hyperintensity are fat deposition, copper accumulation, sinusoidal dilatation, bemorrhage, and high protein content. Fat-suppressed imaging can distinguish fat deposition from other causes of hyperintensity.

Determination by 1H NMR spectroscopy of paraquat in urine from acutely poisoned patients. Comparison with second-derivative spectroscopy method.

The application of 1H nuclear magnetic resonance (NMR) spectroscopy to characterize and quantitate paraquat in urine is described. Characterization was performed taking advantage of two NMR spectroscopy parameters: chemical shifts and coupling patterns. Without any pretreatment of the biological samples, herbicide was detected by its aromatic doublets at 8.49 and 9.02 ppm. Quantitation of the xenobiotic was realized by relative integration of the dipyridyl protons to an internal standard. After a validation step using control urine samples, quantitation was performed in urine obtained from two poisoned patients. On admission, mean paraquat concentrations were 985 (patient 1) and 500 (patient 2) micromol/L. Results are compared and found to be in good agreement, using a second-derivative spectroscopy method.

Tetrachloroethylene and trichloroethylene fatality: case report and simple headspace SPME-capillary gas chromatographic determination in tissues.

We describe a simple, precise, and sensitive assay of tetrachloroethylene and trichloroethylene in tissues, suitable both for emergency cases and forensic medicine. The method employs headspace solid phase microextraction-capillary gas chromatography and electron capture detection. The case is relative to a 45-year-old woman discovered unconscious in a laundry area. The concentrations of the solvents in tissues were determined and compared to other previously published fatalities.

Computed tomography of thoracic aortic dissection: accuracy and pitfalls.

During a 5 year period, 137 patients with suspected acute aortic dissection were screened by CT. The radiologic diagnosis of dissection was made in 54 cases and eliminated in 76. There were seven false-negative CT examinations. Surgical and angiographic correlations or clinical follow-up were obtained with all patients.

Postoperative CT follow-up of aortic dissection.

Computed tomography with contrast enhancement permits noninvasive follow-up of patients treated for aortic dissection. We used CT to examine 52 patients with aortic dissections for up to 14 years postoperatively. The mean interval between surgery and CT was 32 months. Computed tomography demonstrated the aortic graft in 45 patients. Persistent patency of a false lumen was observed in 40 cases. In one case, CT demonstrated extension of the dissection. No rupture or thoracic pseudoaneurysm was noted in our series.

Epithelioid hemangioendothelioma of the liver: MR and CT findings.

The MR imaging features in five patients with hepatic epithelioid hemangioendothelioma (EHE) were correlated with CT and pathologic findings. Two hemangioendotheliomas appeared as multiple nodular lesions with a predominantly peripheral location in the liver. In three more extensive cases, the tumors formed confluent peripheral lesions with macroscopic invasion of portal or hepatic veins (n = 3), signs of portal hypertension (n = 3), and nodular hypertrophy of uninvolved liver (n = 2). These findings, suggestive of EHE, were well demonstrated by MR imaging and CT. The internal architecture of the tumors was clearly depicted on T2-weighted MR images. Viable tumor peripheries appeared moderately hyperintense relative to liver. The center of the tumors consisted of one or several concentric zones. Hyperintense central zones were composed of loose, edematous connective tissue. Hypointense zones contained mainly coagulation necrosis, calcifications, and scattered hemorrhages. Except for the presence of calcifications, the internal architecture of EHE was better defined by MR imaging than by CT.

Castleman disease mimicking liver tumor: CT and MR features.

We report the CT and MR features of two localized abdominal forms of Castleman disease mimicking hypervascular liver tumors due to their unusual location in the porta hepatis and the portacaval space. The MR appearance of Castleman lymph nodes is emphasized, including their characteristics on dynamic turbo-fast low angle shot (FLASH) sequences after Gd-DOTA bolus injection. Our report suggests that the CT and MR features of Castleman lymph nodes in these locations cannot enable their differentiation from other hypervascular masses such as benign liver tumors.

MRI of intralesional hemolysis in focal nodular hyperplasia of the liver.

A case of Kasabach-Merritt syndrome caused by focal nodular hyperplasia of the liver is presented with atypical magnetic resonance findings due to intratumoral hemosiderin deposition. The high sensitivity of magnetic resonance imaging for iron served to identify the site of hemolysis in this patient with Kasabach-Merritt syndrome.

[Hyperbaric oxygenation therapy and cicatrization].

HOT is a very useful therapeutic addition in chronic wounds presenting a healing problem and for which usual treatment were unsuccessful. In order to be fully effective, HOT must be included in a strategy of care, taking into account, not only the wound itself but the underlying disease. Peripheral delivery of oxygen and induction of tissue hyperoxygenation with HOT may be verified by measurement of the TcPO2 under HOT. The predictive value of these measurements is helpful in the decision for treatment and its management.

[Minima corticoid eyedrops in inflammatory outbreak of herpetic keratitis].

Seven patients having repeated inflammatory relapses of herpes simplex keratitis have been treated by small dosages of steroid eyedrops (from 0.25% prednisolone once a week to 1% prednisolone once a day). In these patients these doses were almost always sufficient. There was no complications (especially there happened 7 intercurrent dendritic ulcers which were unaffected by the steroids).
