[Continuous haemofiltration in encephalopathy associated with hepatic failure (author's transl)].

Thirty-one comatose patients (18 with cirrhosis of the liver, 13 with severe hepatitis) were treated with continuous haemofiltration on polyacrylonitrile membrane (AN 69). The mean duration of sessions was 45 +/- 37 h, during which 136 +/- 108 1 of ultrafiltrate were dialyzed. Sixteen patients emerged from coma, 14 (7 with cirrhosis, 7 with hepatitis) completely and 2 partially. Blood ammonium levels decreased by 47 +/- 21% during the first 24 hours. Aminoacid clearance ranged from 20 to 50 ml/min, but only non significant changes were observed in the branched-chain/aromatic aminoacid ratio. Provided the haemodynamic balance is preserved, the technique is well tolerated. The duration of dialysis depends upon the degree of thrombopenia induced. Since the long-term prognosis of both cirrhosis patients and severe hepatitis patients is unmodified, continuous haemofiltration can only be helpful in cases where hepatic regeneration is possible; failing this, liver transplantation should be considered.

Amino Acids

[Acute arsenious hydride intoxication. Four cases (author's transl)].

While engaged in the repair of a zinc furnace, 4 workers were accidentally exposed to arsenious hydride (AsH3) fumes. Acute intravascular haemolysis developed within a few hours. On admission, the patients immediately underwent exsanguino-transfusion; 8.2 to 10.2 l of blood were exchanged through a continuous perfusion pump at the rate of 1 l/hour. Two patients resumed diuresis during transfusion, but the other two required repeated haemodialysis. Between the 10th and 30th days, while renal function was gradually returning to normal, mildly megaloblastic anaemia developed. This was followed during the 3rd month by clinical and electric signs of polyneuritis of the lower and upper limbs, which subsequently regressed. Regular measurements of arsenic levels in the blood and urine were performed between and during exsanguino-transfusion and haemodialysis.

Acute Disease

Cholera-like illness due to Aeromonas sobria.

A Thai woman from Bangkok was admitted to a hospital in Paris for a cholera-like illness. A culture of her "rice-water" diarrhea was negative for Vibrio cholerae and enterotoxigenic Escherichia coli but was positive for Aeromonas sobria. This strain produced enterotoxin, cytolysin, proteolysin, hemolysin, and a cell-rounding factor. Acute -and convalescent-phase sera showed an increase in neutralizing antibodies to enterotoxin, cytolysin, and hemolysin. The enterotoxin, which was labile at 100 C, induced an accumulation of fluid in the rabbit ileal loop model and was not neutralized by antiserum to cholera toxin. Suckling mouse assays and rabbit permeability skin tests were negative, and the Y1 mouse adrenal cell assay produced not true cytotonic effect. This report, the first of an infection due to A. sobria alone, provides evidence that A. sobria is an enteric pathogen of humans that can cause a toxin-mediated, life-threatening illness.

Spinal vacuum phenomenon: CT diagnosis and significance.

Spinal vacuum phenomenon may be encountered in many circumstances, such as degeneration of the intervertebral disk, disk injury, vertebral metastases, ischemic vertebral collapse, Schmorl's nodes, and infectious processes. The high spatial and densitometric resolution of computed tomography (CT) makes it possible to show gas in tissues with high frequency than by conventional radiography. In certain circumstances, the CT findings may be the decisive diagnostic element.

Adult

Vertebral osteomyelitis: disk hypodensity on CT.

The importance and role of computed tomography (CT) are discussed on the basis of 36 cases of vertebral osteomyelitis. The bone images themselves, the detection of lumbar disk hypodensity, and the exploration of soft paraspinal regions in the search for an abscess are factors that contribute to the superiority of this method in difficult cases. In cases where the diagnosis is already known, CT offers an excellent method to assess the extent of the lesions. Its accuracy, coupled with its rapidity and noninvasive nature, affects the role of conventional tomography, a method that is incomplete and involves higher radiation doses. CT offers an excellent method for follow-up after treatment of vertebral osteomyelitis.

Abscess

Massive digitoxin intoxication. Treatment of Amberlite XAD 4 resin hemoperfusion.

Amberlite XAD 4 resin hemoperfusion was studied in the treatment of massive digitoxin overdosage. In vitro experiments using 4 hour resin hemoperfusion showed that digitoxin is removed 100 percent from an isotonic saline reservoir, 83 +/- 14 percent from plasma, and 42 +/- 7 percent from whole blood. In dogs, serum level reduction is fast: 50 +/- 12 percent in 15 minutes, 71 +/- 6 percent in 180 minutes; mean plasma clearance is 24 +/- 6.5 ml/min for a 150 ml/min plasma flow rate. Eleven patients with massive digitoxin overdosage recovered with resin hemoperfusion. Serum level reduction was 32 +/- 14 percent with a 325 g resin column, 59 +/- 6 percent with two 325 g resin columns in series as compared with a 4.5 +/- 1.7 percent reduction without perfusion. Hemoperfusion increased the rate of spontaneous removal of digitoxin 8.6 +/- 3.3 times with a 325 g resin column, and 14 +/- 7.4 times with two 325 g resin columns.

Adult

[Effect of an immunomodulator (P40) on Escherichia coli urinary infection in the rat].

The preventive effects of various doses of the immunomodulator P40 on the experimental E. coli infection of the upper and lower urinary tract have been investigated in the rat. Under our experimental conditions, P40 only weakly influenced the bacteriological picture of the infection in rats infected by the upper route. However, the development of nephropathia might be decreased. By contrast, P40 was found to prevent the development of infection in two thirds of cases infected by the lower route. The preventive effect of P40 on infections of the lower urinary tract seems to be mostly medicated by the stimulation of phagocytosis, although both humoral and cellular immunity are likely to be involved. In contrast, the defense is most probably to be ascribed to cellular immunity in infections of the upper urinary tract. For the future, conditions will have to be specified which enable P40 to express predominantly its stimulating effect on cellular immunity. Furthermore, the choice of the minimum active dose together with its fractionated administration will prevent side-effects (prostatitis) while resulting in equally pronounced protective action. Since P40 has already successfully been used in man for the treatment of both infectious and tumoral processes, it could as well be used for the prevention of relapses in recurrent urinary infections.

Modification of steroidogenesis in a mouse adrenal cell line (Y-1) transformed by simian adenovirus SA-7.

Transformation of a steroidogenic mouse adrenal cell line (Y-1) by simian adenovirus SA7 produced a cell line with low apparent steroidogenic activity. The effect of ACTH and cholera toxin on cyclic AMP production was similar in both not transformed and virus-transformed cells and activity of cyclic AMP-dependent protein kinase was also similar in both cells. In transformed cells, cholesterol was metabolized to delta 5-3 beta-hydroxysteroids, mainly 20 alpha-dihydropregnenolone while in not transformed cells, the major metabolites were delta 4-3 ketosteroids (20 alpha-dihydro- and 11 beta-hydroxy-20 alpha-dihydroprogesterone). In both cell lines ACTH increased the metabolism of cholesterol. Further studies with labelled pregnenolone and progesterone revealed a loss of delta 5-3 beta-hydroxysteroid dehydrogenase/isomerase and 11 beta-hydroxylase activity in the transformed cells.

[Cellular effects of anesthetics].

Anesthetic drugs have multiple effects on cellular structures and functions. Among these are membrane perturbations, cell division, DNA synthesis, disruption of microtubules and microfilaments, phagocytosis and cell locomotion. Some of these effects are reversible, and directly correlated to concentration and length of exposure; other, however, seriously compromise cellular integrity and vital functions. A review of the literature and current work are presented.

Anesthetics

Immunological studies in human ageing. I. In vitro functions of T cells and polymorphs.

T-cells were investigated by the E rosette assay, PHA stimulation, allogeneic MLC and CML responses, in a group of 100 healthy aged people over 70 years old, and compared to young control individuals less than 45 years old. No major difference was found between the two groups, except for PHA stimulation which was significantly reduced in old individuals when low mitogen concentration was used. These observations suggest that only some T cells subsets are affected by ageing, without modification of the total T lymphocyte number. Contrarily, the phagocytic activities of polymorphonuclear cells assessed in a group of 38 aged people were dramatically decreased, using either the NBT test or a direct phagocytosis assay.

Aged

Induction of delta 3(4) ketosteroid synthesis by interferon in mouse adrenal tumour cell cultures.

Interferon, like cholera toxin, induces in mouse adrenal YI cells the production of delta 3(4) ketosteroids. In parallel, cyclic AMP (cAMP) is activated. At high concentrations, mouse but not human interferon induces a cell rounding effect, similar to that obtained with cholera toxin. In YI cells transformed by simian adenovirus 7, production of cAMP is still increased by that of steroids is blocked. The role if interferon in these cellular events is discussed.

Adenoviridae

Acute intoxication with diisopyramide: clinical and experimental study by hemoperfusion an Amberlite XAD 4 resin.

Diisopyramide overdose induces an early and severe cardiogenic shock; hemoperfusion on resin (Amberlite XAD 4) is the most efficient treatment. In vitro, 92% of the drug is adsorbed in 240 min. In dog, the plasmatic epuration is 66% with a mean plasma clearance of 74% plasmatic flow. A woman who had absorbed 4.50 g diisopyramide showed an idioventricular rhythm. The cardiac electrical stimulation and positive inotrope drugs were inefficient. Hemoperfusion on resin Amberlite XAD 4 corrected hemodynamic and ECG disturbances in 4 h. The epuration was proved by the decrease of diisopyramide plasma level: 24.4 micrograms/mL before epuration, 1.9 micrograms/mL after epuration, and a mean plasma clearance of 107.8 mL/min (61% of the plasmatic flow). Hemoperfusion of resin (Amberlite XAD 4) is more efficient than hemodialysis (cuprophan) and hemofiltration (polyacrylonitrile). The system that we have used is well tolerated and of low cost.

Animals

[Production of O antigens and antibodies during urinary tract infections due to Escherichia coli (author's transl)].

In 138 adults with monomicrobial E. coli urinary tract infections, IgG coated bacteriuria (ACB) are found in 9/10 acute pyelonephritis, 21/24 chronic pyelonephritis, 2/5 acute prostatitis, and in only 6/99 lower UTI. These urinary antibodies are synthetised localy because IgA-S are found in 75%, in the kidney because humoral IgG antibodies are detected in only 40% of the ACB + patients. O6, O18, O22, O75, are the four most frequent O antigens (47,5%) and no difference were found in the distribution of O serotypes according to ACB production or clinical signs. But rough strains are significantly more frequent in pyelonephritis, suggesting a modification of the bacterial wall. The use of ACB test and O serotyping allows a better follow-up of patients, but relapses with ACB- and reinfection with ACB+ show the complexity of the relation host-E. coli.
