A simple and rapid analysis of triplet repeat diseases by expand long PCR.

It is now known that 15 monogenic, mostly neurological, disorders are caused by the same type of mutations that occur in trinucleotide repeat sequences in certain genes. Since they share a nonspecific and variable phenotype, the accurate diagnosis could be made only by DNA analysis. We developed an Expand Long PCR assay that provides more reliable molecular diagnosis of such disorders. Its main characteristics are robust amplification of expanded alleles, simplicity, low cost and speed. We suggest the use of Expand Long PCR for routine molecular diagnosis of triplet repeat diseases, and present such analysis of the fragile X syndrome, myotonic dystrophy and Huntington's disease.

Genetic epidemiology of myotonic dystrophy in Istria, Croatia.

OBJECTIVES: We evaluated epidemiology of myotonic dystrophy in Istria, Croatia including direct mutation analysis as an additional, specific diagnostic criterion. MATERIAL AND METHODS: Patients were ascertained in the period 1980-1994 from multiple sources under established clinical criteria with a special reference to congenital and minimal forms of the disease. Additionally, patients and their relatives were evaluated by direct mutation analysis. The prevalence, corrected for underascertainment, was estimated on July 1, 1989. RESULTS: A total of 33 DM patients from nine families were ascertained. In all families the diagnosis was confirmed by mutation analysis of the DM gene. After correction for underascertainment the prevalence of 18.1/100,000 was calculated. CONCLUSION: One of the highest prevalence estimates of DM in the populations without evidence of founder effect or genetic isolation was found. Our results imply the importance of ascertainment of patients with all forms of DM and utilization of specific diagnostic tests for estimation of genetic epidemiology in DM.

[Masticatory values of dental prostheses anchored to depulped teeth].

The progress of conservative and surgical treatment of pulpless teeth has increased the number of pulpless teeth within prothetic restorations. There is a dilemma about masticatory values of such teeth and about the maximum load they can bear, that is, what masticatory values the denture anchored to them will have. Tooth values were measured by gnatodynamometry. We used gnatodynamometer with electroresistant metal measurement strips whose deformity is proportional to the force which the tooth, namely, the denture, bears. Measurements were carried out on a sample of 33 pulpless teeth, that is, on the almost same number of dentures anchored to the teeth. The measured pulpless teeth showed lower values than control teeth. The control group comprised normal teeth of the same subject, the sam jaw, which neutralized the problem of individuality. After the prothetic reconstruction by which the tooth regained its function, we checked mastication values again, this time the values of the dentures anchored to the pulpless teeth. We can conclude that pulpless teeth have lower values than vital ones. The lower values resulted also from their dysfunction. Prothetic reconstruction increases mastication values of pulpless teeth, but they never reach the values of controls, namely, of vital teeth.

Irreversible structural changes in thylakoid membranes at high temperatures. Detection by luminescence and EPR.

The character of structural changes in thylakoid membranes caused by temperature variation was investigated. Experiments were performed on maize leaf segments in vivo and in a closed temperature cycle 24-50-24 degrees C. Two biophysical methods were used for detection: luminescence and EPR. Arrhenius plots of delayed fluorescence (DF) versus reciprocal temperature revealed two break points, T1 and T2. The MeFASL (10.3) spin probe monitoring properties close to membrane surface detected only T2 transition temperature. The results were interpreted in terms of a fluidity change which starts in a membrane centre at T1 and gradually displaces toward the surface at T2. The T1 and T2 transition temperatures are sensitive to pretreatment history of plants indicating that high temperature and drought-induced membrane alterations are irreversible. Activation energies E1, E2 and E3 were determined for temperature regions below T1 between T1 and T2, and above T2, respectively. The E1 and E3 activation energies showed greater sensitivity to stress than did E2. There are some indications that the DF method could be used to screen temperature sensitive and temperature resistant genotypes.

Implementation of a personal computer (PC) as a graphic station into the system MicroDELTA-MaxDELTA for processing of nuclear medicine images.

One of the possibilities to integrate PC into the system "MicroDELTA-MaxDELTA" for acquisition and nuclear medicine image processing is introduced. A 386 IBM compatible personal computer is connected into the local area network with MicroVAX II, which is the part of a "MicroDELTA-MaxDELTA" system. "NuMed" application, which enables simple and efficient manipulation of patient studies and also their archiving, processing and generating reports, is developed in WINDOWS environment. Considering the fact that existing system fully satisfies the acquisition requirements, the whole conception has the aim to upgrade the digital image processing, and initiate further development of the whole system.

[Surgical treatment of polytrauma].

This report is based on the review of 230 victims of multiple injuries, treated during 8 months period. There were 156 casualties with dominant Chest and Abdominal injuries. In the immediate treatment of these patients we applied modification of an original scheme by Schweiberer. We also introduced our own procedure of life-saving measures of high priorities. In evaluation of the most accident victims governed by priority of the severity of the multiple injuries, a complete, orderly examination by X-ray, Ultrasonic and CT scanning was undertaken.

[Reconstruction of the terminal aorta in the treatment of ischemia of the lower extremities].

The significance of the terminal aorta reconstruction in the treatment of lower extremities ischemia cases is described in this work. The indications for this type of reconstructive operations, as well as operating methods which were performed on 115 patients are described. To be able to perform any of the described methods, authors are of opinion that advantages, as well as disadvantages in their mutual relation should be well known. Authors believe that after reconstruction in the aorta-iliac area is carried out, some additional reconstructive operations in the peripheral parts of the femoral artery can be avoided, because they do not contribute to better results.

[Description of various features of injuries to the peripheral arteries and their surgical treatment].

Some of the peripheral arteries damages which were treated during the time period of five years are here presented. The cases of arterial--venous fistulas, as well as false aneurisms have been reported. It should be emphasized that successful treatment requires rapid recognition of lesions, and the reconstruction of the affected artery should be carried out to surgical principles.

[Chronic venous insufficiency of the lower extremities and a halt to absenteeism].

In this work authors experiences about the influence of operation on absentism decrease in 2450 surgically treated patients with chronic venous insufficiency signs and clinical manifestations of various intensity have been presented. In the introduction higher frequency of this disturbance and great financial loss which appear as consequence of this disease, as well as complications which affect the patients with syndrome of chronic venous insufficiency have been described. The authors particularly emphasize still intensive conservative attitude in the therapy of this illness, what leads to its deterioration. Further the authors describe the method of their investigation and the results obtained have been presented in tables. That the operative treatment protects the respective patient with such disturbances from more serious clinical manifestations as well as adequate complications of these disturbances has been emphasized in the conclusion. Further it has been pointed out that immediately applied operative treatment obviously decreases absentism.
