Financial modeling: Rx for financial success.

In an era of managed care, cost cutting and finding ways to increase revenue are key goals in the survival of group practices. Many practices find that they have to boost their revenue by a certain amount (for example, 20-30% within the next three years) to maintain viability in the health care marketplace. Understanding how to generate that revenue and influence short-term and long-term financial outcomes is a far trickier process. This article details how practice administrators can influence a practice's bottom line through a three-step process: (1) identify the components of the practice's financial performance and drivers of performance results, (2) diagnose the practice's current financial situation, and (3) pinpoint benchmarks and targets for success.

Epilepsy associated with hypocalcemia: description of a family.

The unusual occurrence of epilepsy and hypocalcemia in the same family is described. Epilepsy was present in three siblings (convulsive generalized in two females and partial in one male). All six family members had calcium ion, plasmatic phosphorus, plasmatic and erythrocytic potassium, urine calcium and phosphate concentration below the normal levels. Altered values of calcium-induced potassium membrane permeability were found in three subjects (two of them also affected by epilepsy). The assumption that alterations of membrane structure or function play an essential role in determining familial epilepsy and that the normal "set point" for calcium is genetically determined in familial hypocalcemia could explain the concomitance of both pathologies to some extent.

Effect of glutamine and nucleosides on prolactin secretion in the rat.

The effect of glutamine and some nucleosides (cytidine and uridine) on basal and stimulated prolactin release was investigated in vivo and in vitro. Acute injection of glutamine did not modify serum prolactin concentration, both in basal and stimulated conditions; conversely repeated administrations of the aminoacid significantly reduced (p less than 0.05) the prolactin surge which occurs at 04:00 p.m. in male rats as well as that induced by the injection of the dopamine antagonist sulpiride. The hypoprolactinemic effect of glutamine was significantly potentiated by uridine and cytidine, which per se were not effective. No significant change in prolactin release was observed when anterior pituitary glands were incubated in presence of the above mentioned drugs. However, repeated in vivo injections of glutamine, uridine and cytidine were able to potentiate the inhibitory effect of dopamine on prolactin release in vitro. These results suggest that both glutamine and the nucleosides cytidine and uridine may influence the activity of the hypothalamic-hypophyseal-prolactin axis, probably via an interference with endogenous neurotransmitters.
