Data on the prevalence of Trypanosoma cruzi infection is presented for the province of Salta, Argentina. Special emphasis is given to the detection of congenital transmission and to the economic benefits of preventing Chagas' disease. Seroepidemiological data obtained from 20 year old army draftees revealed a reduction, from 22.7 to 11.11% between 1964 and 1985. In university students, a rate of 0.96% was found in 1998. Surveys carried out during 1996 showed that more than 15% of the pregnant women analyzed carried T. cruzi infection, particularly in the north of the province. This situation brings about a high risk of appearance of congenital cases and represents an opportunity to test the most adequate strategies for detection. By applying systematically microhematocrit, hemoculture and PCR methods, to umbilical chord blood, an increase in the early detection of congenitally infected babies is being achieved. In 1992-94, very high seroprevalence rates of infection were found among indians of the Chaco region of Salta. The overall rate was 37%, but there were 5 localities where more than 54% of the population was infected. These numbers indicate that, in vast areas of the provincial territory, fight against vector bugs must not merely consist of surveilance activities, but rather of renewed spraying attacks. The fight must include control of pregnant women and blood banks. An economic analysis of the economic return, calculated only for spraying activities and for the Department of Anta (Salta), indicated a net present value of over 7 million dollars and an internal rate of return exceeding 60%.
A challenge for pacemaker therapists is whether a patient working in an environment with the potential for electromagnetic interference (EMI) can return to their work after a pacemaker has been implanted. Common practice has been to prohibit pacemaker patients from using electric welding machines. Twelve work environments and a new method for monitoring the pacemaker rhythm in the presence of EMI were tested. The new method uses a special memory called the event record found in several Siemens Pacesetter pacemaker models. Surface ECGs with a marking system, intracardiac electrograms, and a digital monitor were used to verify the results with event records. The results from several sources of EMI are reported. Twenty-one in vivo and in vitro tests were performed in the work environments of 12 patients. Event records were useful and accurate both in vivo and in vitro. Electric are welding machines up to 225 A did not affect these pacemakers. Arc welding machines using 1,000 A or more inhibited the in vitro test system within 1 or 2 meters of the weld or power generator. Electric welding machines with high frequency voltage superimposed on the welding current affected the pacemaker when it was within 2 meters of the power unit and 1 meter of the weld. Very large industrial degaussing coils affected pacemakers within 2 meters. The test method using event records was found to be an effective addition to monitoring the pacemaker. These results are specific for the pacemaker models tested. Such testing allows the physician to make a knowledgeable decision regarding return to work for the pacemaker patient in a high EMI environment.
AIM: Presentation of 3 cases of spontaneous hamatoma of the sheath of the rectus muscles in the abdomen. DESIGN: Retrospective clinical observations. LOCATION: Primary Care. PATIENTS: Two males (23 and 28 years old) and a woman (64 years old) who were clinically examined because the reported an acute abdominal pain. RESULTS: Clinical observations. CONCLUSIONS: We should consider the diagnosis of spontaneous hematoma of the sheath of the rectus muscles in the abdomen, differentiating it from a diagnosis of the acute abdomen.

PURPOSE: The purpose of this study was to evaluate the effect of different surgical variables, such as perfusion duration and number of grafts, on resistance and reactance, in heart surgery patients. METHODS: 77 patients submitted to coronary artery bypass were studied. The variable concerning time of extracorporeal circulation was classified in four progressive degrees. Resistance and reactance measurements were performed and compared to time of extracorporeal circulation and to number of grafts. Ten measurements were performed by bioelectrical impedance equipment. RESULTS: The comparison of reactance before surgery to the first measurement at the first postoperative day, for the different times of extracorporeal circulation, showed decrease (p = 0.01). Regarding gender, resistance and reactance showed significant differences (p < 0.001). There was also significant difference (p = 0.001) for the number of grafts. CONCLUSION: The decrease of resistance and reactance is related not only with surgery procedure, but also with the number of grafts and time of extracorporeal circulation.
