[Differential diagnostic question: traumatic unconsciousness or amnestic episode?].

Traumatic twilight state on the one and amnestic episode (transient global amnesia) on the other side are as a rule easy to differentiate from the patient's age, his behavior during the acute state of disease and the kind of its improving. Contrary to other authors' opinion, differentiation is not just possible by diagnosing an injury or its absence. In minor skull traumata preceding the traumatic twilight states injuries are often not manifest and otherwise amnestic episodes occasionnally occur after minor traumata. In a 64 years old woman an obviously typical amnestic episode appeared in direct connection with a probable fall on the back of the head. A 28 years old man underwent a five-hour amnestic episode whereas the EEG showed normal curves. The episode occurred 15 minutes after striking hard upon the ground with the back of the head during a ground roll without following unconsciousness. In both cases traumatic twilight states were not involved. In account of the interval of 15 minutes the trauma of the second case seems not be be the direct cause for the manifestation of amnestic episode. It also shows that the aspect of the disease in the rare occurrence in younger or middle-aged persons is on principle not different from that in old ones. Since a special medical examination during the acute state is only possible as exception the patient's amnestically seizable behaviour obtains greatest importance in differential diagnosis. For the rare coincidence the trauma is given no pathogenetic relevance in amnestic episodes.

The binding of hexobarbital and aniline to cytochrome P-450 of liver microsomes from control and phenobarbital-treated rats of different ages.

The spectral changes due to the binding of hexobarbital and aniline to cytochrome P-450 of rat liver microsomes were investigated in 10-day- to 15-month-old rats. The Ks values for both substances and consequently the affinity for cytochrome P-450 do not change during ageing. Phenobarbital treatment does not alter the affinity of hexobarbital, but enhances the Ks value for aniline. The maximal spectral changes (delta A max) due to aniline are nearly equal in all age groups whereas delta A max due to hexobarbital is very small in young rats and increases considerably during ageing. The age-dependence of the hexobarbital-induced delta A max is similar to the development of drug-metabolizing reactions. delta A max due to hexobarbital and aniline is enhanced by phenobarbital treatment of the rats. The addition of aniline to microsomes enhances the Ks value and diminishes delta A max for hexobarbital.

[Clofibrate treatment of diabetics with hyperproteinemias (author's transl)].

The effect of a six weeks clofibrate treatment on lipid metabolism was investigated in a collective of 38 constantly maladjusted elderly diabetics with an average of 30% overweight and various forms of hyperlipoproteinemias. Clofibrate produced a significant lowering of the mean triglyceride and cholesterol levels, whereas no such effect was observed in an untreated control group. Related to the various types of hyperlipoproteinemia (IIa, IIb and IV), the group treated with clofibrate showed a rate of normalization of nearly 50%, while the rate of improvement in the untreated control group was 10%.

Nucleus anomaly test and chromosomal analysis of bone marrow cells of the Chinese hamster and dominant lethal test in male mice after treatment with fluorescent whitening agents.

Four fluorescent whitening agents (FWAs) were tested for mutagenic activity after oral administration in the following three different mammalian test systems: (1) Dominant lethal test in the male mouse; (2) Cytogenetic studies on metaphase chromosomes from the bone marrow of the Chinese hamster; (3) Nucleus anomaly test in somatic interphase cells (bone marrow) of the Chinese hamster. These investigations yielded no evidence of dominant lethal effects of any of the four compounds on the progeny of male mice. Furthermore, the data obtained from chromosome analyses and the nucleus anomaly test revealed no effects at all. Thus, the results obtained in all these tests gave no indication of mutagenic activity after the administration of these compounds in dosages up to approximately 1/3 or the LD50.

[The importance of ultra-sound cephalometry for the prediction of the date of delivery--a contribution to the assessment of duration of pregnancy (author's transl)].

On the basis of some statistical data ultrasound cephalometry for the prediction of term is discussed. These data should be considered when providing socalled test-graphs. The usefulness of the author's own standard graph is examined. Prediction with the biparietal diameter is not much less exact than after the rule of Naegele. Accuracy of prediction can be much improved by using both methods together, starting from the averages of both.

[Side-effects of lithium therapy].

Side-effects and complications of lithium therapy were examined by evaluation of data registered from 73 patients. Transitory subjective complaints would often occur during the initial phase of therapy, but they do not require any additional therapy. Occasional development of struma and sometimes high increase of body weight require regular controls of these values. Disposing factors concerning the possible occurrence of epileptic seizures and extra-pyramidal disorders under lithium therapy are Melipramin as well as organic cerebral lesions.

The NADPH-dependent cytochrome P-450 reduction in liver microsomes of rats of different ages with and without phenobarbital pretreatment.

The activity of NADPH-cytochrome P-450 reductase in liver microsomes of 10- to 60-day-old rats was determined. Neither the half life time of cytochrome P-450 reduction nor the absolute amount of cytochrome P-450 reduced per time unit depend on age. Phenobarbital pretreatment enhances the reduction rate in all age groups. The addition of hexobarbital or ethylmorphine to microsomal suspension accelerates the reduction of cytochrome P-450 in some age groups only. Age differences corresponding to developmental changes in drug-metabolizing activities are not detectable. The NADPH-cytochrome P-450 reductase seems to be not responsible for the age dependence of drug metabolism.

[Determination of F-wave conductivity in the normal N. ulnaris].

The methodical procedure for the determination of the F-wave conductivity in the ulnar nerve (in total - conductivity, afferent, conduction frequency forearm, afferent-efferent in the proximal segment of the peripheral motor neuron) and the results of a hundred examinations are presented. The results are compared with the motor nerve conduction velocity. No influences of sex, age or the site of lead could be discovered.

[Amnestic episodes. (Isolated episodes of confusion with amnesia, amnesic ictus, transient global amnesia)].

The author, by giving an outline of one particular case as well as a survey of the relevant literature on transient global amnesia, describes the characteristic clinical picture that is characterized by a sudden loss of what is known as the short-time memory, which usually lasts for several hours, as well as by retrograde amnesia which sometimes dates back to many years. The condition gradually fades away within a few hours or during a night's sleep, and what remains is lack or loss of memory for the episodic period or sometimes retrograde amnesia. Relapses were not observed in the majority of patients. The disease usually develops in people in their sixth to eighth decades of life. The cause of the disease is not yet fully understood. Accordingly, it is justified to use such terms as autochthonous or cryptogenic amnestic episodes. The disease is also discussed from a differential diagnosis point of view.

[The effect of symphathicotropic substances on liver microsomes in vivo and in vitro].

Ephedrine and propranolol shorten hexobarbital sleeping time and increase amidopyrine-N-demethylation activity. Pholedrine only scarcely acts on these parameters, dihydroergotamine is effective. In vitro, ephedrine, pholedrine, propranolol and above all dihydroergotamine proved to be inhibitors of amidopyrine-N-demethylation. Noradrenaline, cAMP and dibutyryli cAMP were ineffective. An influence of the sympathetic nervous system on biotransformation activity cannot be recognized.
