Synthesis and binding of stable bisubstrate ligands for phosphoglycerate kinase.

Stable bisubstrate ligands of phosphoglycerate kinase (PGK) have been synthesized with AMP or ADP conjugated to hydrolytically-stable, symmetrical analogues of 1,3-bisphosphoglycerate and their binding to yeast PGK evaluated. Their Kds decrease with net negative charge, with a penta-anionic analogue 7 showing highest affinity-in accordance with its approximation to the transition state for the reaction catalysed by PGK.

A bisubstrate analog induces unexpected conformational changes in phosphoglycerate kinase from Trypanosoma brucei.

The glycolytic enzyme phosphoglycerate kinase (PGK) catalyzes phosphoryl transfer between 1,3-bis-phosphoglycerate and ADP to form 3-phosphoglycerate and ATP. During catalysis, a major hinge bending motion occurs which brings the N and C-terminal enzyme domains and their bound substrates together and in-line for phosphoryl transfer. We have crystallized Trypanosoma brucei PGK in the presence of the bisubstrate analog, adenylyl 1,1,5,5-tetrafluoropentane-1, 5-bisphosphonate, and solved the structure of this complex in two different crystal forms at 1.6 and 2.0 A resolution, obtained from PEG 8000 and ammonium phosphate solutions, respectively. These high resolution structures of PGK:inhibitor complexes are of particular interest for drug design since Trypanosoma brucei, the causative agent of African sleeping sickness, relies on glycolysis as its sole energy source. In both structures, the inhibitor is bound in a fully extended conformation with its adenosine moiety assuming exactly the same position as in ADP:PGK complexes and with its 5' phosphonate group occupying part of the 1,3-bis-phosphoglycerate binding site. The bisubstrate analog forces PGK to assume a novel, "inhibited" conformation, intermediate in hinge angle between the native structures of open and closed form PGK. These structures of enzyme-inhibitor complexes demonstrate that PGK has two distinct hinge points that can each be independently activated. In the "PEG" structure, the C-terminal hinge is partially activated while the N-terminal hinge point remains in an open state. In the "phosphate" structure, closure of the N-terminal hinge point is also evident. Finally and most unexpectedly, both complex structures also contain a 3 A shift of a helix that lies outside the flexible hinge region. We propose that a transient shift of this helix is a required element of PGK hinge closure during catalysis.

Quantitation of estrogen receptor mRNA copy numbers in breast cancer cell lines and tumors.

Several clinical studies have suggested that the content of estrogen receptor (ER) in breast tumors influences the survival, tumor recurrence, and response to antiestrogen therapies. Therefore, the ability to precisely quantitate the ER content in tumor tissues will be of significant benefit to women with breast cancer. Although immunohistochemical and polymerase chain reaction (PCR) methods have been described for the detection and semiquantitation of ER, none of them precisely quantitate ER copy numbers in tumor samples. In the present report we describe a molecular approach to accurately quantitate ER mRNA copy numbers using a reverse-transcription PCR (RT-PCR) template competition method. A competitor template was devised by inserting unrelated nucleic acid sequences into an ER cDNA clone. A template competitive RT-PCR analysis was then performed to determine the number of copies of ER mRNA. As a standard of reference for the ER mRNA copy numbers from various samples, the mRNA copy numbers of a constitutively expressed gene, glyceraldehyde-3-phosphate dehydrogenase (GAPDH), were also quantitated. The ER quantitations were performed in three positive cell lines, MCF-7, T47D, and ZR-75, and two positive tumor tissues by this approach. Our results described here show that among the cell lines studied, T47D expresses the highest copy numbers of ER. We also present here that ER as low as 10(3) copies per 10(5) copies of GAPDH can be detected and quantitated in tumor samples by the template competition method. In addition, the molecular approach can simultaneously detect, distinguish, and quantitate exon deletion variant copy numbers of ER. The results described in this report indicate that the ratios of exon 7 deletion variant to wild type in the tumor tissues are significantly higher than in the cell lines studied.

Comparative mutagenicities of N6-methoxy-2,6-diaminopurine and N6-methoxyaminopurine 2'-deoxyribonucleosides and their 5'-triphosphates.

The structure of the deoxyribonucleoside derived from N 6-methoxy-2, 6-diaminopurine (dK) was examined by NMR. The methoxyamino residue was found predominantly in the imino rather than the amino tautomer (ratio: 9:1 in DMSO). The nucleoside proved to be a potent transition mutagen in Escherichia coli , in contrast to the closely related nucleoside derived from the analogue N6-methoxyaminopurine (dZ), which was only weakly mutagenic. The 5'-triphosphate derivatives, dKTP and dZTP, were synthesized; Taq polymerase incorporated dKTP opposite both T and, less well, opposite dC in template DNA. Both analogue triphosphates produced transition mutations when added to PCR reactions. In each case, there was a large excess of AT-->GC compared to GC-->AT mutations (ratios were 15:1 for dKTP and 10:1 for dZTP). Polymerase extension times in each cycle had to be extended, consistent with a decreased rate of DNA synthesis in the presence of the analogues. This and the mutagenic ratios are discussed in terms of syn-anti inversion of the methoxyl group.

In vitro model of arterial stenosis: correlation of MR signal dephasing and trans-stenotic pressure gradients.

PURPOSE: Turbulent flow just distal to stenoses causes signal loss (dephasing) on magnetic resonance angiography (MRA). This study correlates dephasing with trans-stenotic pressure gradients in an in vitro model of arterial stenosis. MATERIALS AND METHODS: Three-dimensional (3D) phase contrast, 2D time-of-flight, and 3D spoiled gradient echo MRA with/without gadolinium and varied echo time were performed for a system consisting of a peristaltic perfusion pump and a silastic vessel with stenoses of varying caliber. Length and diameter of dephasing jets were measured, and volumes calculated at varying pressure gradients and echo times, then correlated with percentage cross-sectional area stenosis as measured by conventional angiography. RESULTS: Dephasing occurred in all sequences at pressure gradients of > or =4 mmHg (1 mmHg = 133 Pa) and stenoses of greater than 70%, and varied directly with pressure gradient. The dephasing was greatest for 3D phase contrast (PC). Gadolinium did not diminish dephasing. CONCLUSIONS: MRA signal dephasing at stenoses varies directly with pressure gradient. MRA may provide a non-invasive means for determining the hemodynamic significance of arterial stenoses.

Excessive oral amphetamine use as a possible cause of renal and splanchnic arterial aneurysms: a report of two cases.

INTRODUCTION: Multiple visceral aneurysms are uncommon and usually result from connective tissue diseases, systemic arteritis, or mycotic lesions. An association between multiple visceral aneurysms and excessive oral amphetamine use has not been reported. METHODS: The clinical features of 2 patients at the University of Michigan Medical Center for treatment of multiple visceral aneurysms and amphetamine use were reviewed. RESULTS: The patients had histories of excessive oral amphetamine use that ranged from 50 mg daily for 22 years to 200 mg daily for 2 years. No evidence was seen of systemic arteritis, connective tissue disorder, or an infectious process that may have caused the aneurysms. The arteriograms documented multiple splanchnic and renal artery aneurysms that involved both the large and the small arteries. The aneurysms of 1 patient were managed conservatively, and the patient has not had any clinical sequelae of the aneurysms during 14 years of follow-up. The second patient had hematobilia from a ruptured hepatic artery aneurysm that was treated with transcatheter embolic occlusion of the bleeding vessel. The patient had no recurrent gastrointestinal problems and continued to use amphetamines until his death from a cerebrovascular accident 6 years later. CONCLUSION: A possible association between excessive oral amphetamine use and multiple visceral aneurysms is reported for 2 patients in whom other risk factors were absent. The potential for chronic oral amphetamine use to cause multiple visceral aneurysms is an ill-defined but not unexpected complication of this substance that is known to contribute to arterial hypertension and to produce a form of necrotizing arteritis.

Induction of signalling anergy via the T-cell receptor in cultured Jurkat T cells by pre-exposure to a filarial nematode secreted product.

Filarial nematodes constitute major causes of morbidity in the Tropics. The worms have a life-span exceeding five years, a longevity which is considered to reflect at least in part, their ability to interfere with host lymphocyte responsiveness. To date the molecular mechanisms underlying this ability have not been defined but we now demonstrate that ES-62, a phosphorylcholine (PC)-containing glycoprotein released by the rodent filarial parasite Acanthocheilonema viteae, is able to render Jurkat T cells anergic to intracellular signalling via the antigen receptor (TCR). In particular, ES-62 acts by modulating activation of the tyrosine kinases Fyn, Lck and ZAP-70 leading to selective disruption of TCR coupling to the phospholipase D, protein kinase C, phosphoinositide-3-kinase and RasMAPkinase signalling cascades. These cascades are key elements in the transduction of transcriptional and proliferative signals following ligation of TCR. As PC-containing secreted products (PC-ES) are also released by human filarial parasites, our data suggest that PC-ES may play a role in the induction of T lymphocyte hyporesponsiveness observed during filarial infections.

Gadolinium-enhanced magnetic resonance venography of the portal venous system prior to transjugular intrahepatic portosystemic shunts and liver transplantation. Original investigation.

RATIONALE AND OBJECTIVES: The accuracy of gadolinium-enhanced magnetic resonance venography (GdMRV) in identifying visceral venous abnormalities was assessed in patients before they underwent transjugular intrahepatic portosystemic shunt (TIPS) or orthotopic liver transplantation (OLT). METHODS: Twenty-seven patients with portal hypertension underwent GdMRV and transcatheter venography prior to OLT or TIPS. The gadolinium dose was 0.5 mL/kg (0.25 mmol/kg), administered by rapid hand injection. Coronal 3D spoiled gradient-echo GdMRV was performed in a single breath-hold. Four blinded reviewers retrospectively evaluated coronal maximum intensity projection (MIP) images, while two reviewers evaluated the MIPs and multiplanar reconstructions. Abnormalities that could affect transjugular intrahepatic portosystemic shunt or transplantation were noted and compared with the results of corresponding catheter venograms read by a separate blinded reviewer. RESULTS: Abnormalities were identified by GdMRV with a sensitivity and specificity of 83% and 97% for the right hepatic vein, 86% and 100% for the main portal vein (MPV), 42% and 99% for the right portal vein, 54% and 94% for the left portal vein, 61% and 96% for the superior mesenteric vein, and 74% and 91% for the splenic vein. Varices and shunts were correctly identified with a sensitivity of 96%. Multiplanar reconstruction increased MPV sensitivity to 100%. CONCLUSION: Vascular abnormalities that affect TIPS and OLT can be identified by GdMRV. Multiplanar reconstruction increased the accuracy to 100% for the MPV.

Bromodeoxyuridine incorporation and Ki 67 expression in oral hairy leukoplakia.

OBJECTIVES: Oral hairy leukoplakia (HL) is an acanthotic, hyperparakeratotic lesion characterised by the presence of a replicative Epstein-Barr virus (EBV) infection in the superficial and adjoining layers of the epithelium. EBV or its gene products are capable of modifying epithelial differentiation. The aim of this study was to establish whether the presence of EBV was associated with an alteration in cell turnover by assessing bromodeoxyuridine (BrdU) incorporation and Ki 67 expression in lesional tissue and control mucosa. METHODS: Biopsies of HL together with age, site and sex matched controls (n = 7 and 8 respectively) were incubated in 200 microM BrdU in vitro, fixed in methacarn and processed to paraffin wax. Following acid hydrolysis, incorporated BrdU and Ki 67 were identified in serial 5 microns sections using a three-stage immunoperoxidase technique and cell density expressed as the number of positive cells per mm basement membrane length. RESULTS: Overall, there was no difference in the number of BrdU positive cells per mm basement membrane length between control and HL tissue. However, within HL alone, the presence of focal EBV replication was associated with a significant reduction in the number of basal cells incorporating BrdU compared to adjacent EBV free areas (P < 0.05). There was no significant difference between Ki 67 positive cells in control and HL tissue and no evidence of a reduction of Ki 67 positive cells in areas associated with EBV replication. CONCLUSIONS: These results suggest that there is no evidence of a generalised alteration of the proliferative capacity of basal cells in HL, although the focal reduction in BrdU incorporation may reflect subtle changes on cell turnover by EBV infection.

Building a research agenda: responding to the needs of Community and Migrant Health Centers.

This article summarizes the results of an invitational conference designed to establish a research agenda for collaborative projects involving university-based health services researchers and staff (administrative and clinical) from Community and Migrant Health Centers (C/MHCs). More research related to C/MHCs needs to be developed, preferably by collaborative teams of researchers and C/MHC personnel. Specific research ideas are summarized, and five more detailed research proposals are presented. This is an especially important area that needs work, given the changes taking place in health care finance and the impacts of those changes on C/MHCs.

A role for interleukin-6 in host defense against murine Chlamydia trachomatis infection.

Interleukin-6-deficient (IL-6(-/-)) knockout mice had significantly increased Chlamydia trachomatis levels in lung tissue and increased mortality compared to B6129F2/J controls early after intranasal infection. Gamma interferon production and chlamydia-specific antibody levels were consistent with a decreased but reversible Th1-like response in IL-6(-/-) mice. IL-6 is needed for an optimal early host response to this infection.

The mechanism of mutation induction by a hydrogen bond ambivalent, bicyclic N4-oxy-2'-deoxycytidine in Escherichia coli.

The triphosphate of the nucleoside deoxyribosyl dihydropyrimido[4,5-c][1,2]oxazin-7-one (dP) is known to be incorporated into DNA efficiently by Taq polymerase and is a useful tool for polymerase-mediated in vitro mutagenesis. It is shown here that dP is a potent mutagen in Escherichia coli and Salmonella typhimurium . In E.coli , this deoxycytidine analog induces both GC-->AT and AT-->GC transitions. No induced transversions are observed. It is highly mutagenic in wild-type E.coli, but this is much reduced in a strain lacking thymidine kinase. Mutagenesis induced by dP is efficiently inhibited by the addition of thymidine. Partially purified thymidine kinase from E.coli catalyzes phosphorylation of dP to its 5'-monophosphate. When E.coli was grown in the presence of dP, the nucleoside analog was incorporated into its DNA. The content of dP in DNA was dependent on the concentration of dP added to the medium. The incorporation characteristics of the 5'-triphosphate of dP (dPTP) were also studied using E.coli DNA polymerase I large fragment. The results confirm that this triphosphate can be incorporated opposite A and G in the template with similar efficiencies. This indicates that dP is metabolized as a thymidine analog and that the resulting triphosphate induces a high rate of mutagenesis through replicational errors.

Habitable moons around extrasolar giant planets.

Possible planetary objects have now been discovered orbiting nine different main-sequence stars. These companion objects (some of which might actually be brown dwarfs) all have a mass at least half that of Jupiter, and are therefore unlikely to be hospitable to Earth-like life: jovian planets and brown dwarfs support neither a solid nor a liquid surface near which organisms might dwell. Here we argue that rocky moons orbiting these companions could be habitable if the planet-moon system orbits the parent star within the so-called 'habitable zone', where life-supporting liquid water could be present. The companions to the stars 16 Cygni B and 47 Ursae Majoris might satisfy this criterion. Such a moon would, however, need to be large enough (>0.12 Earth masses) to retain a substantial and long-lived atmosphere, and would also need to possess a strong magnetic field in order to prevent its atmosphere from being sputtered away by the constant bombardment of energetic ions from the planet's magnetosphere.

Habitable planets with high obliquities.

Earth's obliquity would vary chaotically from 0 degrees to 85 degrees were it not for the presence of the Moon (J. Laskar, F. Joutel, and P. Robutel, 1993, Nature 361, 615-617). The Moon itself is thought to be an accident of accretion, formed by a glancing blow from a Mars-sized planetesimal. Hence, planets with similar moons and stable obliquities may be extremely rare. This has lead Laskar and colleagues to suggest that the number of Earth-like planets with high obliquities and temperate, life-supporting climates may be small. To test this proposition, we have used an energy-balance climate model to simulate Earth's climate at obliquities up to 90 degrees. We show that Earth's climate would become regionally severe in such circumstances, with large seasonal cycles and accompanying temperature extremes on middle- and high-latitude continents which might be damaging to many forms of life. The response of other, hypothetical, Earth-like planets to large obliquity fluctuations depends on their land-sea distribution and on their position within the habitable zone (HZ) around their star. Planets with several modest-sized continents or equatorial supercontinents are more climatically stable than those with polar supercontinents. Planets farther out in the HZ are less affected by high obliquities because their atmospheres should accumulate CO2 in response to the carbonate-silicate cycle. Dense, CO2-rich atmospheres transport heat very effectively and therefore limit the magnitude of both seasonal cycles and latitudinal temperature gradients. We conclude that a significant fraction of extrasolar Earth-like planets may still be habitable, even if they are subject to large obliquity fluctuations.

Surgical delay for acute type A dissection with malperfusion.

BACKGROUND: An acute type A aortic dissection is considered a surgical emergency. Review of the risk factors for a type A dissection showed that preoperative malperfusion was associated with a 22% (2/9) intraoperative mortality and an 89% (8/9) hospital mortality. Intraoperative deaths were secondary to pulmonary failure resulting from capillary leak; the remaining patients died of multiorgan failure resulting from reperfusion injury. METHODS: The surgical delay approach was adopted for malperfused patients, and treatment in these patients included percutaneous reperfusion, with aortic fenestration and branch stenting where appropriate. Twenty patients had a type A dissection and malperfusion shown by pulsed-wave Doppler echocardiography, transesophageal echocardiography, or spiral computed tomographic scanning. Malperfusion was documented by angiography. After reperfusion, all patients' conditions were stabilized in the intensive care unit; intravenous beta-blockers were administered to decrease the maximum rate of increase of left ventricular pressure. Once patients completely recovered from the consequences of malperfusion, surgical repair was performed. Statistical comparison of the non-delay and delay groups was performed using Fisher's exact test and Student's t test. Multiple logistic regression analysis was used to establish independent predictors for mortality. RESULTS: The mean delay to repair was 20 days (2 to 67 days). Four (31%) patients were discharged home and readmitted for operation. Three patients (15%) died preoperatively, 1 of retrograde dissection and rupture and 2 of reperfusion injury. Seventeen underwent surgical repair, with two deaths (12%); 15 (75%) were discharged, with an average follow-up of 16.8 months (p < 0.003). Delay was the only independent predictor of outcome. CONCLUSIONS: Patients with an acute type A dissection and malperfusion should undergo percutaneous reperfusion, and surgical repair should be delayed until the reperfusion injury resolves.

The effect of contralateral broad-band noise on acoustic distortion products from the human ear.

In the first part of the study, the effect of ipsilateral stimulus level on contralateral suppression of the 2f2-f2 distortion product by broad-band noise at 60 dB SPL RMS is examined. The levels of the primary stimuli were manipulated independently, giving f1 and f2 growth curves at four different f2 frequencies for four subjects. These typically bell-shaped, growth curves are shifted vertically to lower distortion levels and, in some cases, horizontally to higher primary stimulus levels. These results can be interpreted as an attenuation of both the primary stimuli and the distortion product and could be caused by simple acoustic attenuation produced by middle ear muscle activity, efferent activity or a combination of the two. In the second part of the study, the same contralateral stimulus was used while measuring both 3f1-2f2 and 2f1-f2 distortion products from the ipsilateral ear. The frequency separation of the primary tones was varied. This produced an approximately bandpass shape with the level peaking when the distortion frequency was approximately half an octave below f2, as previously described (Brown and Gaskill, 1990). This shape is thought to be linked with frequency selectivity in the cochlea. Contralateral broadband noise did not affect the tuning or the centre frequency of the bandpass shape or the mean group delay. It did reduce the size of the distortion peak and, in particular, it affected the peak-to-trough height of the 'fine structure' in the amplitude. Vector analysis revealed that the fine structure was due to a signal with substantial delay (probably from the distortion product 'place') which was summed with a larger, less delayed component (probably directly from the f2 'place'). The greater effect of contralateral stimulation on the more delayed component may reflect differences in efferent effect with complex (stimulus place), rather than simple (distortion product place) stimuli.

Ki-67 antigen in ameloblastomas: correlation with clinical and histological parameters in 54 cases from Kenya.

The aim of this study was to assess the cell proliferation in ameloblastomas and to correlate this with clinical features and histology. Immunohistochemistry with Ki-67 monoclonal antibody was performed on fresh tissue from 54 ameloblastomas. A labelling index (LI) was calculated by expressing the percentage of Ki-67 positive cells. There was no significant correlation between LI and clinical features: age, sex or tumour size. Follicular ameloblastomas had significantly higher LI (5.0 +/- 0.5; mean +/- SEM) than plexiform tumours (3.2 +/- 0.6; P < 0.05). Plexiform ameloblastomas from the anterior mandible had a significantly lower LI (1.8 +/- 0.5) than those from the posterior (3.9 +/- 0.8; P < 0.05). LI was higher in squamous arcades (6.4 +/- 3.1%) than in epithelial cords and cysts (1.4 +/- 1.3%; P < 0.001). These results suggest that LI correlates most closely with the histological pattern of the epithelium of ameloblastoma, both within and between different tumours.
