Prenatal diagnosis of the Lesch-Nyhan syndrome.

Amniocentesis provides the prenatal diagnosis of chromosomal abnormalities and many biochemical disorders. Many of the biochemical assays are not routinely performed in many institutions. Those institutions utilizing autoradiographic studies routinely can make a diagnosis of biochemical disorders satisfactorily by utilizing a combination of bank cells, amniotic fluid cells and autoradiographic techniques. An example is given of this technique used in a patient with a family history of Lesch-Nyhan syndrome.

Convulsive toxemia of pregnancy (eclampsia).

A retrospective study investigated the epidemlologic characteristics of eclamptic patients seen at the University of Mississippi Medical Center. Between January 1955 and June 1973, 194 patients were discharged from our hospital with the diagnosis of eclampsia. Although our incidence of severe forms of toxemia of pregnancy has decreased from 6.8 per 1,000 deliveries from 1955 to 1960 to 4.8 per 1,000 deliveries from 1970 to 1973, it is still three times higher than for the United States overall. Our patients generally were young, unmarried, and without adequate prenatal care. Primary therapy for severe preeclampsia and eclampsia consists of bed rest and administration of magnesium sulfate until stable, and then delivery. Therapy for the eclamptic patient includes control of the convulsions, administration of magnesium sulfate, and delivery when the patient's condition has stabilized. Antihypertensives and diuretics are used for cardiovascular indications.

Amniotic fluid glucose: a maternal, fetal, and neonatal correlation.

While it has been noted that amniotic fluid glucose is increased in patients withdiabetes mellitus, the exact significance of this finding is not known. Our study of amniotic fluid glucose in 53 amniotic fluid samples from 41 patients shows a high correlation of amniotic fluid glucose levels of 30 mg. per 100 ml. or greater with neonatal hypoglycemia. We have also found elevated amniotic fluid glucose levels in patients whose onlyother indication of abnormal glucose metabolism was recurrent glycosuria. Thepossible significance of these findings is discussed.
