The prenatal ultrasonic diagnosis of urethral obstruction and diverticulum of the urinary bladder.

In a 28 year old gravida 2 para 1 an ultrasonogram at 35 weeks of gestation revealed a huge cystic abdominal tumor in fetus. Postpartum ultrasound, excretory urography and micturating cystourethrography showed a trabeculated bladder with a large diverticulum and hydronephrosis due to a posterior urethral valve. The prenatal diagnosis of this anomaly and immediate surgical intervention prevented additional severe damage in the newborn.

Retrospective evaluation of human menopausal gonadotropin and human chorionic gonadotropin induction of ovulation in galactorrheic and hyperprolactinemic women.

A retrospective study on the success of induction of ovulation with human menopausal gonadotropin and human chorionic gonadotropin (hMG/hCG) in 267 women was performed. Galactorrheic women had a higher pregnancy rate (55%) than nongalactorrheic women (22%). Galactorrhea was a far better indicator of the success of the treatment than hyperprolactinemia at present. The results obtained may suggest that in "bromocriptine failure" it is not mandatory to lower prolactin levels prior to induction of ovulation with hMG/hCG, and these women may be treated with menotropins alone.

Comparison of cornual transfer via laparotomy with utero-cervical transfer of cultured preimplantation rat embryos.

Most work on embryo transfer has used the cornual route via laparotomy for implantation. This method is time-consuming and costly in operating materials. Cervical transfer seems to offer a simpler route and the promise of time and cost saving. Technical difficulties, however, have prevented this method from becoming widely used. It was thought that mastery of this technique is essential to allow large-scale experiments in order to determine the optimal time for reimplantation, the optimal stage whether 8 cells, morulae, or blastocysts, and to compare culture media. All of these will have significant clinical applications. In this work Vickery's method of cervical transfer in mice was modified to include direct vision of the cervix and dilatation before implantation. With this modification equivalent results were found on cervical or cornual transfer, but a higher failure rate on cervical transfer. The results and implications are discussed.

Elective cervical suture of twin pregnancies diagnosed ultrasonically in the first trimester following induced ovulation.

The efficiency of elective cervical suture in preventing premature delivery thus reducing neonatal mortality was studied in a group of 50 twin pregnancies. All pregnancies occurred after induction of ovulation and twins were diagnosed early by ultrasound. 25 randomly selected patients underwent elective cervical suture. 22 sutured and 23 non-sutured patients were followed until delivery, while 5 patients aborted in the second trimester. The benefit of suturing on the duration of pregnancy and its outcome were assessed. Of the sutured patients, 10 (45.4%) delivered prematurely and the neonatal death rate was 18.2%. In the non-sutured patients, 11 (47.8%) delivered prematurely and the neonatal death rate was 15.2%. This study demonstrates that elective cervical suture was not effective in prolonging gestation or improving fetal outcome in twin pregnancies following induced ovulation.

Continuous sonographic monitoring of IUD extraction during pregnancy: preliminary report.

The significance of the location of intrauterine contraceptive device in relation to the gestational sac on pregnancy outcome was studied in 13 pregnant women after extraction of the device. Of the seven women (group 1) who had the intrauterine contraceptive device removed after initial localization by sonography, two (28.5%) miscarried. In both cases the device was located cephalad or lateral to the gestational sac. In six women (group 2) continuous sonographic scanning was undertaken to visualize the extraction procedure. Traction on an intrauterine device located laterally (two cases) or cephalad (one case) to the gestational sac was followed by distortion of the sac configuration. Gentle intermittent traction on the contraceptive device resulted in successful extraction procedure. These preliminary observations suggest that pregnancies with an intrauterine contraceptive device located laterally or cephalad to the gestational sac are at greater risk during extraction of the device. Sonographic monitoring of the extraction procedure during pregnancy may minimize the hazards of intrauterine contraceptive device failure.

Nonoperative management of ectopic pregnancy. A preliminary report.

The incidence of ectopic pregnancy is increasing throughout the Western world; at present it is uncertain how much of this increase is due to the disease and/or its antecedents and how much due to better means of diagnosis. That the treatment of the obvious or ruptured ectopic pregnancy should be surgical is beyond doubt. However, in view of the natural tendency of some ectopic pregnancies to terminate in tubal abortion or complete resorption, it is questionable whether surgery is always necessary in every early case or whether some patients can be monitored by means of rising or falling levels of beta subunits of human chorionic gonadotropin (HCG) until tubal abortion or resorption occurs. This may be the best means of preserving tubal function and fertility.

Laparoscopy in the management of uterine perforation.

The introduction of laparoscopy as a routine diagnostic tool for pelvic disorders has prompted its use in the management of uterine perforation as well. A retrospective review of 52 patients with uterine perforation secondary to curettage is presented. In 23 (44.2%) the curettage was discontinued, and the postperforation course was uneventful. Eight patients (15.4%) underwent immediate exploratory laparotomy without laparoscopy; in three of them no internal organ injury or bleeding was found. Eighteen patients (34.6%) underwent postperforation laparoscopy; seven of these women underwent subsequent laparotomy. The indications and objectives of postperforation laparoscopy are outlined. Its judicious use allows proper selection of patients for exploratory laparotomy.

Spontaneous pregnancy and its outcome after human menopausal gonadotropin/human chorionic gonadotropin-induced pregnancy.

The fertility in previously sterile women who conceived at least once following hMG/hCG-induced ovulation is investigated. The study comprises 141 women. The cumulative spontaneous pregnancy rate (CSPR) was calculated using life table analysis and was found to be 30.4% after 5 years. The CSPR for subsequent pregnancies reached 91.3% after 5 years. This figure is similar to that of normal parous women, although the study group (previously infertile women) requires a larger exposure period to attain the figure. The spontaneous abortion rate in the hMG/hCG-induced pregnancies was 29%; whereas in subsequent spontaneous pregnancies this rate was 8.8%. This difference in rate was found to be statistically significant, and the possible reasons are discussed.

Effect of the carbon dioxide laser on the oviducts of rabbits.

Oviducts of 35 mature, white New Zealand rabbits were injured by a laser beam, using various powers from 25 to 40 W, and by a series of pulses of 1/10, 1/20 or 1/100 s duration. The contralateral oviducts served as controls and were injured at similar areas by a scalpel. The injured areas of both tubes were histologically examined at different periods from the time of injury. Enzyme histochemical studies were also carried out. The site of the laser injury was bloodless. There was a delay in the healing process of the laser side. Vessels were sealed at the area where the spot of the laser hits the vessels, but beyond that point a regular thermal damage was noted with thrombus formation such as seen in regular diathermia or scalpel injuries. Enzymatic activity of the epithelium in the vicinity of the laser-injured areas was intact. The thermal damage due to laser injury found in this study is still significant and would necessitate the use of the laser by microtechniques so as to limit the damage of oviduct tissues.

Uterine anomalies in diethylstilbestrol-exposed women with fertility disorders.

Uterine anomalies were demonstrated by hysterosalpingography (HSG) in 4 of 5 diethylstilbestrol (DES) exposed young women with fertility disturbances. The anomalies included malformed uterine cavities and cervical incompetence. It is suggested that HSG in DES exposed women with fertility disturbances be performed early in the process of infertility investigation and cerclage should be considered when uterine anomalies are demonstrated. Tubal pregnancy should be ruled out by ultrasonography early during pregnancy in DES-exposed women.

Triplet pregnancy in uterus didelphys with delivery interval of 72 days.

A unique case of triplet pregnancy in a woman with uterus didelphys is described, with fetuses A and B in the right uterine horn and fetus C in the left horn. Missed abortion of fetus A was diagnosed in the 22nd week of gestation by real-time ultrasonography, but only in the 27th week of gestation did the right uterine horn contract, resulting in expulsion of fetus A. Fetus B was delivered by cesarean section 2 hours later. Fetus C remained in the left uterine horn until the 37th week of gestation, when a viable 2490-g female infant was delivered by cesarean section. Triplet pregnancy occurs with sufficient frequency to be unremarkable; however, triplet pregnancy in a patient with uterus didelphys and with 72 days' lapse between the delivery of the first 2 fetuses and delivery of the third is a rare occurrence.

Cumulative conception rates following gonadotropin therapy.

During the years 1963 to 1978, 515 patients were treated with HMG. They were divided into two groups depending upon the absence (Group I) or presence (Group II) of distinct endogenous estrogen activity. Group II patients were referred after failure of clomiphene therapy. None of the patients had mechanical or male infertility factors. The cumulative pregnancy rate (life table method) in Group I patients after six cycles of treatment was 91.2%. In 77 patients from Group I, further treatment was given for a second pregnancy after the first gonadotropin conception. In this group, the cumulative conception rate was 93.6% after eight cycles of treatment. In Group II the cumulative conception rate was 50% after 12 cycles of treatment. In both groups, the results were better in patients who were less than 35 years of age than those who were 35 or more when treated.

Prognosis in pre- and postmenopausal patients with endometrial adenocarcinoma.

The prognosis in 41 pre- and 143 postmenopausal patients with endometrial adenocarcimona during two time periods were compared. A significantly shorter interval from 1st symptom to diagnosis was found in postmenopausal patients. The percent of patients diagnosed in stage I did not differ significantly between the two groups, but there was a significant, although small, excess of postmenopausal patients diagnosed in stages III-IV. A significantly more favourable survival in premenopausal patients was found and the same trend was observed when the effect of age, stage at diagnosis and operability were taken into consideration. The possibility that this could be related to hormonal and immunological factors is raised. A significantly better survival of patients diagnosed during the second period was observed and is attributed to improved planning and methods of treatment.
