Inhibition of DNA synthesis in dermal tissue of merino sheep treated with depilatory doses of mouse epidermal growth factor.

Two groups of three Merino wethers were infused intravenously with either 0.12 mg mouse epidermal growth factor (mEGF)/kg fleece-free body weight or 0.9% (w/v) NaCl over 24 h. Sheep treated with mEGF rejected food during treatment but feed intake was kept equal for both groups. Wool growth and plasma concentrations of mEGF were measured during the experiment. Pieces of skin taken from the wool-growing regions of the body were incubated with radioactive thymidine in order to measure its rate of incorporation into DNA. The skin was then divided at about the level of the sebaceous glands into sections that contained the upper dermis and epidermis (E sections) and those containing the generative wool-follicle bulbs (D sections). No mEGF was detected in the controls whereas mean levels of about 35 micrograms mEGF/1 plasma were detected during the last 4 h of infusion in the protein-treated group. After infusion, wool growth was reduced by about 20% of the mean pretreatment level in the controls and no shedding of wool fibre was evident. In the mEGF-treated sheep, on the other hand, wool growth was depressed by 75-95% of the mean pretreatment level and the fleeces were almost completely cast in all three of the animals, leaving them nude on the wool-growing regions of the body. Wool growth was restored to its pretreatment level in this group about 1 month after infusion. The D sections of skin contributed 50-60% of skin wet weight in controls throughout the experiment.(ABSTRACT TRUNCATED AT 250 WORDS)

Batrachotoxin induced axonal necrosis followed by regeneration.

Batrachotoxin (BTX), when applied to peripheral nerve in concentrations sufficient to block impulse transmission and axonal transport, causes axonal necrosis. Quantitation of this phenomenon reveals reduction of both myelinated and unmyelinated fibers 7 days post-BTX injection. Evidence for regeneration correlates with the previously reported recovery of postsynaptic events.

Direct, indirect, and sham laser photocoagulation in the management of central serous chorioretinopathy.

The role of argon laser photocoagulation in the management of central serous choriorectinopathy was evaluated in a prospective randomized study of eyes with leaks smaller than 250 microns in diameter in the early frames of the angiogram. Eyes were assigned to Group A when the leak was located in the papillomacular bundle or within 500 microns of the capillary-free zone and to Group B when the leakage site was located outside the papillomacular bundle and more than 500 microns from the capillary-free zone. Thirty eyes in Group A underwent either a sham argon laser treatment or a real argon laser treatment directed to the pigment epithelium under the elevated retina at a site remote from the site of fluorescein dye leakage (indirect laser photocoagulation). Twelve eyes in Group B underwent either a laser treatment directed to the site of fluorescein dye leakage (direct laser photocoagulation) or indirect laser photocoagulation. Compared with indirect photocoagulation, direct laser photocoagulation shortened the duration of central serous chorioretinopathy by approximately two months, a statistically significant difference. When compared to sham treatment, indirect photocoagulation did not significantly alter the duration of central serous chorioretinopathy. During an 18-month interval, the recurrence rate in the eyes treated with sham and indirect laser photocoagulation was 34%, whereas no recurrences were observed in the eyes treated with direct photocoagulation. There were no complications from photocoagulation in any of the eyes.

Electrofocusing fractionation and characterization of pituitary follicle-stimulating hormone from male and female rats.

Extracts of male and female rat pituitaries were fractionated by electrofocusing on sucrose density gradients and the pH distribution of FSH determined by an FSH in vitro bioassay, rFSH radioimmunoassay (RIA) and FSH radioreceptor assay (RRA) method. FSH for both sexes was located in the pH range 3-6. Significantly more activity, as measured by all three methods, was found in the male in the pH range 3.4-4.0 (P less than 0.01), while conversely significantly more activity was found in the female in the pH range 4.8-6.0 (P less than 0.05). Ratios of the levels of FSH assayed by the three methods showed significant (P less than 0.05) differences between sexes in both the original pituitary extracts and electrofocusing fractions in the pH region 4.4-6.0. The acidic fractions, when fractionated by gel filtration (Sephadex G100SF), were larger in apparent molecular size than the more alkaline fractions found in pituitary extracts from either sex. Binding of electrofocusing fractions from male pituitaries to concanavalin A showed significant (P less than 0.01) differences between fractions, suggesting that the various FSH fractions may differ in their carbohydrate structure. It is concluded that the differences in apparent size and charge of FSH from male and female rat pituitaries can be attributed to the combination in different proportions of the various FSH species. However, differences in the ratio of FSH activity (in vitro bioassay, RRA and RIA) for FSH from the same pI region (pH 4.4-6.0) between sexes suggests that structural differences may also exist.

Optic nerve meningioma.

An obese white woman presented with a 10-month history of progressive loss of vision in the right eye associated with a moderate proptosis. Examination of the fundus revealed marked congestion of the retinal vasculature with dilation of both the arteries and the veins. Numerous blot hemorrhages were scattered throughout the retina. The retina was elevated and appeared to be displaced anteriorly by solid tissue, but B-scan ultrasonography demonstrated only a serous detachment of a thickened, markedly convoluted retina. A diagnosis of optic nerve meningioma was considered, but definitive treatment was deferred because of medical problems. One month later the patient returned with a blind, painful right eye associated with neovascular glaucoma. High resolution CT coronal sections of the orbit showed generalized thickening of the optic nerve. The blind eye was enucleated facilitating exposure of the optic nerve which was found to be diffusely thickened by meningioma. The orbital portion of the optic nerve was subsequently removed and meningioma was demonstrated along its entire length to the bony optic canal. A transfrontal craniotomy was later performed to rid the patient of residual tumor.

Extrascleral extension of choroidal melanoma diagnosed by ultrasound.

Four cases of choroidal melanoma with extrascleral extension are presented. In each case, contact B-scan ultrasonography identified extrascleral extension of choroidal melanoma as an area of relative echolucency immediately behind the globe. These ultrasonographic findings are contrasted with those demonstrated by computerized tomography. The importance of B-scan ultrasonography in the assessment of a patient with possible choroidal melanoma is discussed.

Hamartomatous adenoma of the nonpigmented ciliary epithelium arising in iris-ciliary body coloboma. Light and electron microscopic observations.

A 2 1/2-year-old boy had a slowly enlarging mass at the site of a typical iris-ciliary body coloboma for 2 years. The mass was excised partially by iridocyclectomy. By light microscopy, the tumor cells were embedded in a rich mucoid stroma that contained abundant hyaluronidase-sensitive acid mucopolysaccharides. By electron microscopy the tumor showed light and dark cells with interdigitating cell membranes, desmosomes, gap junctions, multilaminar basement membrane, and numerous extracellular collagen fibrils that resembled vitreous fibrils. We believe that the tumor represents a hamartomatous (congenital) adenoma of the nonpigmented ciliary epithelium rather than a conventional (acquired) adenoma, since it developed precisely within a colobomatous defect of the iris and ciliary body.

Tuberous sclerosis in an infant of 28 weeks gestational age.

A case of tuberous sclerosis diagnosed at autopsy in a neonate at 28 weeks gestation is described. This is believed to be the earliest case yet reported. The CNS lesions consist of cortical tuberosities, heterotopic white matter nodules and subependymal nodules. These nodules consist predominantly of giant astrocytes but a small undifferentiated cell component is present as well. Multiple cardiac rhabdomyomas are also present. An explanation for neuronal hypocellularity within the cortical tuberosities is suggested.

Paraneoplastic subacute necrotic myelopathy.

A 79 year old male acutely developed a Brown-Sequard syndrome two weeks following resection of renal cell carcinoma. The subsequent clinical course was marked by subacute progression of spinal cord signs. Pathologic studies showed extensive multifocal spinal cord necrosis that could not be attributed to vascular occlusion or to vasculitis. This case appears to be a rare example of paraneoplastic subacute necrotic myelopathy.

The regulation of testicular function.

From this review it is evident that our understanding of the control of testicular function has advanced greatly over the past decade. Many of the advances have depended on techniques to isolate and study the function of Sertoli cells and Leydig cells. Although this knowledge has been essential, it is important to proceed cautiously in the extrapolation of this information to the in vivo situation. The likely interdependence of the two compartments of the testis in normal function highlights the need to consider the testis as a whole, and future studies must give cognizance to this concept. Furthermore, the importance of blood and lymphatic flow and the evidence of hormonal influences on the vasculature of the testis add yet another dimension to the in vivo function of this organ.

Preliminary observations regarding the use of iodine-125 in the management of choroidal melanoma.

When compared with Cobalt-60, the physical properties of Iodine-125 make it an attractive alternative therapy for consideration in the management of selected choroidal melanomas. The low-energy gamma emission can be nearly completely shielded by a thin sheet of gold 0.4 mm thick. Iodine-125 contained in a gold-backed episcleral plaque may reduce radiation exposure to non-target tissues of the eye including the optic nerve and macula. Iodine-125 is readily available, is adaptable to specific tumour locations and size, and offers very little radiation hazard to handlers. The physical characteristics of Iodine-125 are suitable for treatment of relatively large ocular tumours. Preliminary experience at the Mayo Clinic with Iodine-125 in the management of 12 patients with malignant melanoma to the choroid who have been followed between six months and two and a quarter years, indicate the effectiveness of Iodine-125 in promoting tumour shrinkage. In 11 of the 12 treated eyes, there is evidence of decreased tumour thickness as judged by ultrasonography and photography.

Cervical myelopathy due to atlantoaxial and subaxial subluxation in rheumatoid arthritis.

Four patients aged 41 to 73 years, who had had rheumatoid arthritis for eight to 25 years, had signs and symptoms of cervical myelopathy and radiculopathy due to either atlantoaxial dislocation with herniation of the odontoid through the foramen magnum, or subluxation of the middle to lower cervical vertebrae. Spastic paraparesis or quadriparesis, severe nuchal immobility and pain, and flaccid paresis of the upper limbs necessitated anterior medullary decompression and posterior cervical fusion. Postmortem examination disclosed old ischemic necrosis, atrophy, and gliosis in the low medulla and cervical cord. Anterior and posterior gray horns and contiguous posterior and lateral funiculi bore the brunt of the damage. Ascending and descending wallerian degeneration and atrophy of the cervical nerve root were evident. In three cases, anterior spinal or radicular arteries demonstrated intimal fibrosis with moderate stenosis; two cases depicted chronic phlebitis or subarachnoid vessels. Previous reports have infrequently provided evidence of a vasculopathy.

The incidence of retinal detachment in Rochester, Minnesota, 1970-1978.

We studied the incidence of rhegmatogenous retinal detachment in Rochester, Minnesota, for the nine-year period from 1970 through 1978. The age-adjusted rate for idiopathic rhegmatogenous retinal detachment was 10.1 per 100,000 population per year (45 cases). Including 12 cases of aphakic retinal detachments, the age-adjusted incidence was 12.9 per 100,000 population per year. Not surprisingly, the incidence rate increased with age in both sexes. By extrapolation from the Rochester rates, the number of cases per year of idiopathic retinal detachment in the United States would be expected to be about 22,000, and the number of both aphakic and idiopathic phakic cases per year would be expected to approach 28,000.

Macroaneurysms of the retinal arteries.

In a study of the natural course and risk factors leading to visual impairment from retinal arterial macroaneurysms in 40 eyes (35 patients), we used a simple classification based on the anatomic location of the macroaneurysm and complicating factors. Group A included 17 eyes in which the exudate, edema, hemorrhage, or aneurysm (or a combination of these) was located within the vascular arcades and caused a decrease in central visual acuity. Group B included eight eyes in which the exudate, edema, hemorrhage, or aneurysm (or a combination of these) was located within the vascular arcades but did not cause a decrease in central visual acuity. Group C included 15 eyes in which the exudate, edema, hemorrhage, or aneurysm (or a combination of these) was located peripheral to the vascular arcades. Eyes in Group A with recent visual symptom had an uncertain prognosis and some were treated; eyes in Group B had a more favorable prognosis but required careful periodic examinations; and eyes in Group C had a favorable prognosis and almost always did well without treatment. Eyes with macroaneurysms hemorrhaging into the vitreous or under the inner limiting lamina also tended to recover.

The use of 51Cr for assessing cytotoxicity in an in vitro bioassay for inhibin.

A cytotoxicity test based on 51Cr release was investigated to assess the specificity of an in vitro bioassay for inhibin utilizing rat anterior pituitary cells in culture. The bioassay is based on a specific suppression of the pituitary cell content of FSH. A variety of substances both of known and unknown toxicity, were bioassayed for inhibin at serial dilutions and the 51Cr release, and FSH and LH cell content levels determined and morphological appearance of the cells evaluated. Results indicated that the 51Cr release procedure was more sensitive and specific in assessing cytotoxicity than either morphological evaluation or LH cell content procedures. In addition, since the 51Cr release was largely independent of the 51Cr concentration, the cell concentration and the age of the culture (2-6 days), this method could be used to assess the quality of, or differences between individual cultures. In conclusion, the 51Cr release procedure provides an additional index for assessing the specificity of the inhibin bioassay as well as an index of the quality of individual cultures. The simplicity and high precision of this cytotoxicity test make it an attractive adjunct to other in vitro bioassay methods.

The specificity of inhibin bioassay using cultured pituitary cells.

It has been established that the inhibition of the cellular content of FSH in cultured pituitary cells can be used as a sensitive and precise bioassay for inhibin. During studies on the inhibin content of human plasma, FSH suppression was noted to occur together with LH suppression. Further studies revealed that where combined FSH and LH suppression occurred, cytological changes in the pituitary cells suggestive of toxicity were found, indicating non-specific effects of these substances. Charcoal treatment or gel filtration of seminal plasma removed or separated the toxic substances from the inhibin activity, the latter characterized by FSH suppression parallel to the standard preparation, no LH suppression and no light-microscopic changes in pituitary cells. It is recommended that careful evaluation of all inhibin bioassay systems should be undertaken to detect substances producing non-specific effects and additional guidelines for the assessment of specificity are suggested.

Snowflake degeneration of the retina.

Ten patients in four families with white or yellow-white granular-like deposits of the peripheral retina, consistent with the description of snowflake degeneration are reported. The granular changes are observable near the equatorial fundus as numerous ophthalmoscopically discrete deposits commonly up to 100 to 200 microns in size that focally thicken the retina. They can be rather evenly distributed about the entire circumference of the eye, but sometimes they may show a predilection for the inferior quadrants. The lesions need not be associated with recognizable vitreous liquefaction, vitreoretinal traction, retinal breaks, or retinal pigmentation, and in some instances the condition appears relative stable for intervals as long as five years. The similar appearance in a mother and her 9-year-old son suggests further the relative stability of the condition. In its pure form, snowflake degeneration may be an innocent dominant familial finding but when associated with other conditions such as lattice degeneration, snowflake degeneration may be of clinical significance. Affected members may have co-existing extensive white with and without pressure changes and/or lattice degeneration.
