Survivorship analysis of the Mayo total ankle arthroplasty.

From 1974 until the end of 1988, 204 primary Mayo total ankle arthroplasties were performed at the Mayo Clinic. By means of actuarial analysis, we determined the cumulative rates of survival with failure (defined as removal of the implant) as the end point. The average duration of follow-up was nine years (range, two to seventeen years). By applying the Cox proportional-hazards general linear model, we identified two independent variables that were associated with a significantly higher risk of failure: a previous operative procedure on the ipsilateral foot or ankle and an age of fifty-seven years or less. The overall cumulative rate of survival at five, ten, and fifteen years was 79, 65, and 61 per cent, respectively. The probability of an implant being in situ at ten years was 42 per cent for patients who were fifty-seven years old or less and who had had previous operative treatment of the ipsilateral ankle or foot and 73 per cent for those who were more than fifty-seven years old and who had had no such previous operative treatment. We do not recommend the use of the Mayo total ankle arthroplasty, particularly in younger patients who have had a previous operative procedure on the ipsilateral ankle or foot.

Nonmeckelian jejunal or ileal diverticulosis: an analysis of 112 cases.

BACKGROUND: Incidentally found asymptomatic disease, chronic pain or malabsorption, emergent disease with complications are the three distinct modes of presentation of nonmeckelian jejunal or ileal diverticulosis (JID). We analyzed our experience with each of these three subsets of cases to determine both the role of operative intervention and an overall management strategy. METHODS: Retrospective analysis was done of 112 clinically diagnosed cases of nonmeckelian JID between 1975 and 1990. RESULTS: JID was an incidental finding in 47 patients (42%), and 82% of those who were followed up for a mean of 4.8 years remained asymptomatic. Forty-five patients (40%) had malabsorption or chronic pain, and only 46% of those who were followed up for a mean of 6.5 years were cured with conservative therapy. Of the 20 patients (18%) who had acute complications, 11 (10%) required operation, which was associated with minimal morbidity and one death. Ninety percent remained asymptomatic for a mean of 3.9 years. CONCLUSIONS: Incidentally found JID is usually asymptomatic and should be observed. Resection for complicated JID is safe and effective. Although malabsorption or chronic pain should be treated medically initially, the significant incidence of failure warrants further study of the role of resection in this group.

Parathyroid imaging with technetium-99m-sestamibi: an initial institutional experience.

BACKGROUND: The ideal method for preoperative localization of abnormal parathyroid glands has yet to be determined. Technetium-99m-sestamibi, previously used for myocardial perfusion studies, has recently been introduced for parathyroid imaging. METHODS: From August 1991 to September 1993, 44 patients underwent Tc-99m-sestamibi scanning (45 scans) and surgical exploration for hyperparathyroidism at our institution. These 44 patients form the database for this retrospective study. Twenty-eight patients had persistent hyperparathyroidism, six had recurrent disease, three had prior thyroid operation, and seven underwent first time neck operations. The nature of disease was complex and varied: single gland, 26; primary hyperplasia, 5; multiple endocrine neoplasia type 1, 5; familial, 3; secondary or tertiary, 5. One patient with single gland disease and one patient with multiple endocrine neoplasia type 1 had parathyroid carcinoma. All patients had biochemical confirmation of hyperparathyroidism. RESULTS: Twenty-six (58%) of 45 scans accurately predicted the location(s) of all abnormal gland(s) involved (true positive). Surgical removal of these glands was curative. Sixteen (36%) of 45 scans were false negative because they did not show all abnormal glands involved; however, 7 of these 16 scans did localize at least one abnormal gland. The overall sensitivity of this test was 62% when all abnormal glands were considered but increased to 79% with the demonstration of at least one abnormal gland. These values increased to 80% and 90% (p = 0.03), respectively, in the last 20 patients when the injected dose of Tc-99m-sestamibi was increased from 10 to 15 mCi. Thirty-seven of 44 patients were cured after operation. Six of the seven patients who experienced surgical failure had multigland disease, one of which was malignant. Sestamibi scans correctly identified abnormal cervical glands in 18 (58%) of 31 patients and abnormal mediastinal glands in six (75%) of eight patients. CONCLUSIONS: Tc-99m-sestamibi scanning is helpful in the reoperative setting as an adjunct to localizing abnormal parathyroid tissue. A higher percentage of positive tests occurs in patients with mediastinal and single gland disease. A higher dose of Tc-99m-sestamibi (15 mCi) significantly improves test sensitivity.

Total hip arthroplasty after failed intertrochanteric osteotomy.

From 1969 through 1982, 305 hips in 290 patients had total hip arthroplasty for failed femoral intertrochanteric osteotomy. Of these, we reviewed 215 hips (70.5%) with a minimum follow-up of five years. The results were good or excellent in 79%, but there were technical problems at operation in 23% and a total perioperative complication rate of 11.8%. Late complications occurred in 13.1% including seven late infections (3.2%). At a mean follow-up of ten years, 39 hips had been revised (18.1%), there was probable loosening in 19.5% of stems and 12.6% of cups and possible loosening in 11.4% of stems and 7.2% of cups. The cumulative probability of failure at ten years was 20.6%. We recommend that intertrochanteric osteotomy be advised, planned and executed bearing in mind the possible need for a subsequent total hip arthroplasty, and that screws and plates should be routinely removed soon after union of the osteotomy.

Survivorship analysis of implant arthroplasty for the first metatarsophalangeal joint.

During a 21-year period at the authors' institution, 93 primary implants of the first metatarsophalangeal joint were performed on 79 patients (12 men and 67 women). Their mean age was 56 years (range, 19 to 75 years). The average duration of follow-up evaluation in 75 patients who were alive and without reoperation was 12 years (range, two to 17 years). With the proportional hazards general linear model, young age was identified as a significant risk factor for poor survival: at ten years, implant survival was 82% in patients 57 years of age or younger, and 90% in patients older than 57 years. The overall probability that an implant would not have been removed was 86% at ten years after arthroplasty and 82% at 15 years.

The pain drawing and Waddell's nonorganic physical signs in chronic low-back pain.

Pain drawings were obtained from a group of 651 patients who had chronic low-back pain. Pain drawings were rated in four grades according to the degree of organic and nonorganic pain. Experienced and inexperienced evaluators were used. The reliability was excellent with an interevaluator reliability between 73% and 78%. A correlation between pain drawings and Waddell's nonorganic physical signs demonstrated that a large proportion of patients with high Waddell scores had nonorganic pain drawings. No significant differences were noted in the distribution of Waddell scores and pain drawings based on patient gender or payment status (i.e., medicolegal or workers' compensation). Pain drawings afford an important adjunct in the physician's assessment of chronic low-back pain.

Effect of electrical stimulation on lumbar paraspinal muscles.

Forty healthy, sedentary, premenopausal women were entered into a randomized, single-blind, controlled study to determine the effects of capacitively coupled electrical stimulation on the strength of the lumbar paraspinal muscles and the bone mineral in the lumbar spine. All were between 35 and 45 years of age and had normal physiologic estrogen. The study group received electrical stimulation over the lower lumbar paraspinal muscles for 30 minutes twice a day. Isometric strength of the lumbar paraspinal muscles was assessed with a strain-gauge dynamometer at entry and again after 3, 6, 9, and 12 months. Bone mineral was measured in the lumbar spine by dual-energy x-ray absorptiometry at entry and at 6 and 12 months. After 3 months, the median increase in isometric strength was 8.1% in the study group and 1.6% in the control group (P < 0.03). This initial difference was maintained during the remainder of the study. No further changes were seen between the two groups at 6, 9, and 12 months. It was concluded that capacitively coupled electrical muscle stimulation can, throughout a 1-year period, improve and maintain isometric strength of the lumbar paraspinal muscles independent of exercise, but it has no measurable effect on bone mass in the lumbar spine.

Long-term outcome in patients undergoing surgical repair of tetralogy of Fallot.

BACKGROUND: Although corrective surgery for tetralogy of Fallot has been available for more than 30 years, the occurrence of late sudden death in patients in whom surgery was apparently successful remains worrisome. METHODS: We studied long-term survival among 163 patients who survived 30 days after complete repair of tetralogy of Fallot, examining follow-up hospital records and death certificates when relevant. RESULTS: The overall 32-year actuarial survival rate among all patients who survived surgery was 86 percent, as compared with an expected rate of 96 percent in a control population matched for age and sex (P < 0.01). Thirty-year actuarial survival rates were calculated for the patient subgroups. The survival rates among patients less than 5 years old, 5 to 7 years old, and 8 to 11 years old were 90, 93, and 91 percent, respectively--slightly less than the expected rates (P < 0.001, P = 0.06, and P = 0.02). Among patients 12 years old or older at the time of surgery, the survival rate was 76 percent, as compared with an expected rate of 93 percent (P < 0.001). The performance of a palliative Blalock-Taussig shunt procedure before repair, unlike the performance of a Waterston or Potts shunt procedure, was not associated with reduced long-term survival, nor was the need for a trans-annular patch at the time of surgery. Independent predictors of long-term survival were older age at operation (P = 0.02) and a higher ratio of right ventricular to left ventricular systolic pressure after surgery (P = 0.008). Late sudden death from cardiac causes occurred in 10 patients during the 32-year period. CONCLUSIONS: Among patients with surgically repaired tetralogy of Fallot, the rate of long-term survival after the postoperative period is excellent but remains lower than that in the general population. The risk of late sudden death is small.

Outcomes after curative resections of cholangiocarcinoma.

OBJECTIVES: To elucidate the clinical and pathologic features of extrahepatic cholangiocarcinomas and to identify prognostic variables in patients treated surgically. DESIGN: Retrospective review of clinical records of patients undergoing surgical exploration for cholangiocarcinoma, with univariate and multivariate analyses of the clinical and pathologic factors that influenced patient survival. SETTING: Mayo Clinic, Rochester, Minn. STUDY PARTICIPANTS: One hundred seventy-one patients undergoing operative intervention for diagnostic, palliative, or curative reasons between 1976 and 1985. Follow-up was complete until death or for a minimum of 5 years for surviving patients. INTERVENTION: A curative surgical resection was performed in 29% of patients, while the remainder underwent tumor biopsy or a palliative procedure. MAIN OUTCOME MEASURE: Patient survival following operative treatment. RESULTS: The operative mortality in this patient cohort was 5% and median survival was 13 months. Overall 5-year survival was 16%, with 44% of patients having a curative resection still alive at 5 years. Using univariate analysis, curative resection, tumor stage, Eastern Cooperative Oncology Group performance status, total bilirubin concentration, lymph node status, liver invasion, tumor morphology, tumor grade, and site of tumor origin were significant determinants of prognosis. Using the Cox proportional hazards model for multivariate analysis, curative resection, Eastern Cooperative Oncology Group performance status, total bilirubin concentration, and tumor grade were the only variables predictive of patient outcome. A curative resection of a proximal cholangiocarcinoma had a similar chance of providing long-term survival as a curative distal ductal resection. CONCLUSIONS: Although the tumor extent and the patient's overall health will affect outcome, curative resection for cholangiocarcinoma at all sites should be undertaken since this treatment offers the best chance for long-term survival.

The histological pattern and pathological involvement of the anal transition zone in patients with ulcerative colitis.

BACKGROUND: The aim was to determine whether the transitional epithelium (TE) of the anal transition zone (ATZ) was involved by chronic ulcerative colitis (CUC) and whether preserving the ATZ preserves the disease. METHODS: Proctocolectomy specimens from 50 CUC patients and 50 patients with rectal cancer serving as controls were stained with alcian-blue to map the ATZ, and biopsy specimens containing adjacent TE and rectal mucosa were examined. RESULTS: The mean inflammation score (0, none; 4, severe) in TE of controls was 0.4, whereas in CUC it was 0.5. However, the mean inflammation score of the rectal mucosa within the ATZ was 0.2 in controls and 2.6 in CUC (P < 0.001). Rectal columnar epithelium extended past half of the maximum length of TE in 75% of patients (65-83%; 95% CI) and was within 1 cm of the dentate line in 89% (81%-94%). CONCLUSIONS: Although the TE of the ATZ was not inflamed, the rectal mucosa within the ATZ was. Moreover, rectal mucosa traversed half of the length of the ATZ in 75% of patients and was within 1 cm of the dentate line in fully 89%. Preserving the ATZ may preserve the disease in the majority of patients with CUC.

Low-versus high-energy photon beams in radiotherapy for lung cancer.

This retrospective study analyzed the outcome of lung cancer patients who were treated with either 4-MV or 10-MV photons. From October 1979 through December 1982, 126 patients with locally advanced, unresectable or medically inoperable, nonmetastatic non-small cell lung cancer were treated in a prospective trial in which they were randomly assigned to one of three chemotherapy combinations and thoracic radiotherapy. The patients were stratified by cell type, extent of operation, age, sex, and status of supraclavicular lymph nodes. All patients were followed until death or for a minimum of 4.8 years. Of the 102 evaluable patients, 98 were treated with either 4-MV or 10-MV photons (49 patients in each group). Outcomes examined included best primary tumor response, time to first local (in-field) recurrence, disease-free survival, and overall survival. No significant differences were detected between the patients treated with 4-MV or 10-MV photons for several important prognostic and treatment factors or for any of the study outcomes, including first local (in-field) recurrence, disease-free survival, and overall survival. For the group of 98 patients treated with either 4-MV or 10-MV photons, the estimated 2-year freedom from first local (in-field) recurrence was 47.7%. The estimated 2-year disease-free and overall survivals were 21.6% and 28.6%, respectively.

Trends in the utilization of primary total hip arthroplasty, 1969 through 1990: a population-based study in Olmsted County, Minnesota.

Using the unique data resources of the Rochester Epidemiology Project, we identified the residents of Olmsted County, Minnesota, who underwent total hip arthroplasty (THA) between 1969, when this technique was introduced, and 1990. During this period, 735 residents underwent 859 primary THAs. An initial dramatic upsurge in utilization of THA soon after its introduction was followed by a lower rate of increase, to an age- and sex-adjusted rate of 59.9 per 100,000 person-years in 1987 through 1990. In Olmsted County, the procedure was used more often in women than in men, but men underwent THA at a younger age than did women. THAs were performed for various conditions of the hip, but the increased utilization during the last decade of the study was mainly attributable to its application in primary degenerative hip disease among patients (male more often than female) from a wider age range; utilization of THA for hip fractures decreased during the study period. Most THAs were of the cemented variety (86.1%); however, the rate of use of noncemented THAs, first performed in 1984, is increasing. The overall upward trend in Olmsted County was consistent with European data but less than the rate of increase observed for the United States as a whole between 1983 and 1987. Olmsted County utilization rates, if projected nationally, suggest a need for more than 139,000 primary THAs each year at an annual direct cost of more than $2.7 billion. An urgent need exists for detailed examination of national utilization patterns and clarification of the role of cementless implants and of THA for hip fractures.

Aortopulmonary window: factors associated with early and late success after surgical treatment.

Between 1953 and 1990, 19 patients, who were from 7 weeks to 27 years old, underwent surgical correction of aortopulmonary window at the Mayo Clinic. Associated cardiac anomalies were present in nine patients (47%). At operation, extracorporeal circulation was used in all except one patient. In seven patients, division and primary closure were done. In four patients, the defect was closed by direct suture through a transpulmonary approach. In eight patients, the defect was closed with a patch through a transpulmonary or transaortic approach. Four deaths (21%) occurred intraoperatively or immediately postoperatively. All four patients had undergone division of the aortopulmonary window before 1962, and three of them had a pulmonary vascular resistance (Rp) that was 7.3 U.m2 or more and a ratio of Rp to systemic vascular resistance (Rp/Rs) that exceeded 0.6. One patient with an Rp of 11.8 U.m2 and an Rp/Rs of 0.72 died 16 years postoperatively. Statistical analysis of risk factors indicated that early year of operation (P = 0.022), division of the aortopulmonary window versus transaortic or transpulmonary closure (P = 0.009), and a high Rp/Rs (P = 0.021) were significantly associated with perioperative death. All patients with a preoperative Rp of 8 U.m2 or less, an Rp/Rs of less than 0.4, and no major associated cardiac anomalies were in functional class I (New York Heart Association) postoperatively. Our study confirms that infants with aortopulmonary window should undergo operation early, before irreversible pulmonary vascular changes have developed. Patients with an Rp/Rs that exceeds 0.4 should be thoroughly assessed to determine their operability.

Risk factors for cytomegalovirus and severe bacterial infections following liver transplantation: a prospective multivariate time-dependent analysis.

Risk factors for cytomegalovirus and severe bacterial infections were studied prospectively by univariate, multivariate and time-dependent Cox model analysis in 79 consecutive liver transplant patients treated with selective bowel decontamination. Cytomegalovirus infection occurred in 39 patients (49%) and was symptomatic in 22 patients. Twenty-six patients (33%) developed at least one of 43 documented severe bacterial infections. In a multivariate analysis of risk factors for all cytomegalovirus infections, the cytomegalovirus seronegative recipient-cytomegalovirus seropositive donor group was the highest risk group (P < 0.001). Using the same analysis for risk factors for symptomatic cytomegalovirus infections, a prolonged prothrombin time (P < 0.005), a diagnosis of acute fulminant hepatitis as the underlying liver disease (P < 0.01) and a cytomegalovirus seronegative patient receiving a liver from a seropositive donor (P < 0.001) were significant. The treatment with OKT3 therapy (P < 0.008) and hepatic artery thrombosis (P < 0.02) were found to be significant risk factors in a time-dependent univariate analysis but were not independent risk factors when multivariate analysis was utilized. Significant risk factors for major bacterial infections (P < 0.03) using univariate analysis included a prolonged anesthesia, anhepatic and surgical times, as well as the transfusion of large amounts of fresh frozen plasma or autologous blood. In a multivariate analysis, only the transfusion of large amounts of fresh frozen plasma (P < 0.04) was a significant independent risk factor. Cytomegalovirus infection was a risk factor for the development of severe bacterial infections (P < 0.03) in a multivariate time-dependent analysis.

Autologous platelet-rich plasma does not reduce transfusion of homologous blood products in patients undergoing repeat valvular surgery.

BACKGROUND: Patients undergoing cardiac surgery employing cardiopulmonary bypass frequently require transfusion of homologous blood products and, therefore, are exposed to the risk of transfusions. Autologous platelet-rich plasma administration may reduce homologous transfusion and attendant risks. METHODS: In a blinded, randomized fashion, patients undergoing repeat sternotomy and valvular surgery received either a sham product (n = 28) or autologous platelet-rich plasma (n = 28) at the conclusion of cardiopulmonary bypass. Perioperative blood loss, coagulation profiles, and transfusion requirements were compared between the two groups. RESULTS: In the first 24 h postoperatively, both the platelet-rich plasma and sham groups received a median of 10.5 units of homologous blood products. Total median perioperative homologous transfusion requirements were 13 and 11.5 units for the platelet-rich plasma and sham groups, respectively. There was no significant difference in intraoperative or postoperative bleeding between the groups. CONCLUSIONS: Autologous platelet-rich plasma did not reduce perioperative bleeding or transfusion requirements in repeat valvular surgery.

Complications and risk factors after ileal pouch-anal anastomosis for ulcerative colitis associated with primary sclerosing cholangitis.

OBJECTIVE: This study determined predictive factors for postoperative complications and outcome after ileal pouch-anal anastomosis in patients with ulcerative colitis and primary sclerosing cholangitis. SUMMARY BACKGROUND DATA: Patients with ulcerative colitis and primary sclerosing cholangitis treated by colectomy and ileostomy are at high risk of troublesome bleeding from peristomal varices. METHODS: Postoperative complications and outcome were assessed in 40 patients with ulcerative colitis and sclerosing cholangitis who received an ileal pouch-anal anastomosis between January 1981 and February 1990. RESULTS: Immediate postoperative and remote ileoanal anastomosis-related complications were high but related directly to the severity of liver disease. No patient had perianastomotic anal bleeding. CONCLUSIONS: In patients with both ulcerative colitis and primary sclerosing cholangitis, ileal pouch-anal anastomosis is safe and is not associated with perianastomotic bleeding.

Congenital rubella syndrome: ophthalmic manifestations and associated systemic disorders.

Congenital rubella syndrome has a wide variety of severe ophthalmic and systemic complications. A worldwide rubella epidemic from 1963 to 1965 affected thousands of infants. This is a 20 year follow up study of patients with congenital rubella syndrome analysing the prevalence of ophthalmic disorders, associated systemic problems, and correlations among these defects. The authors statistically analysed 125 cases of congenital rubella seen in the Mayo clinic ophthalmology department over a 32 year interval. Most patients were young adults. Ocular disease was the most commonly noted disorder (78%), followed by sensorineural hearing deficits (66%), psychomotor retardation (62%), cardiac abnormalities (58%), and mental retardation (42%). Multiorgan disease was typical (88%). Ocular disease and hearing loss were frequently associated (53% had both) but not significantly correlated. A similar association existed between ocular and cardiac disease. Cataracts and microphthalmia were significantly correlated with poor visual acuity (each p < 0.0001). Glaucoma was significantly correlated with cataracts (p = 0.0002) and microphthalmia (p = 0.0024) but not poor visual acuity. Four patients with microphthalmia developed late onset glaucoma. No significant association was found between gestational age at time of maternal infection and the incidence of individual ocular conditions. However, several cardiac disorders were significantly associated with gestational age. Although new cases of congenital rubella are rare, surviving victims continue to challenge the ophthalmic and medical communities with a wide range of ocular and systemic disorders.

Evaluation of portal venous hypertension with cine phase-contrast MR flow measurements: high association of hyperdynamic portal flow with variceal hemorrhage.

Thirty-two patients with portal venous hypertension and endoscopically proved esophageal varices who were being evaluated for possible liver transplantation were studied with cine phase-contrast magnetic resonance (MR) imaging. Flow measurements in the main portal vein were obtained and associated with the presence of variceal hemorrhage within 2 years before the MR examination. Low (hypodynamic) flow was present in 22 patients, while high (hyperdynamic) flow was present in 10 patients. The presence of variceal hemorrhage was significantly associated with a high portal venous flow rate (P = .001), high variceal grade (P = .030), and Child class A or B (P = .003); however, only portal venous flow (P = .006) and variceal grade (P = .044) were found to be associated with variceal hemorrhage in a multiple logistic regression analysis. The portal venous flow rate was significantly higher among patients with Child class A or B disease compared with those with class C disease (median, 24.6 vs 8.0 mL/min.kg; P = .004).
