The Cardiff Cervical Cytology Study. Prevalence and epidemiology of cervical neoplasia.

The Cardiff Cervical Cytology Study showed a prevalence of carcinoma-in-situ that rose to a peak of 6.1/1000 in age group 35-44 and then decreased. Prevalence of microinvasive and occult invasive carcinoma rose to peaks of 1.8/1000 and 1.1/1000 respectively in age group 45-54 and then declined. Epidemiological analysis was based on comparison to three groups-dysplasia, carcinoma-in-situ and microinvasive carcinoma combined, and occult and clinical invasive carcinoma combined. For all groups prevalence increased with lower social class, was higher in widowed, divorced and separated women than in married women, and increased with decreasing age at first marriage and at first pregnancy and with increasing number of pregnancies. The magnitude of these association was remarkably similar for all three histological groups. Screening for cervical neoplasia is based on the belief that the various histological categories are part of a continuum, a spectrum of disease, and the existence of a common epidemiological pattern for the three histological groups is consistent with such a hypothesis.

Abnormal blood vessels in the gastric antrum: a cause of upper-gastrointestinal bleeding.

Three patients who bled from curious vascular lesions of the gastric antrum are described. Each presented with an iron-deficiency anemia. Histological examination in two of the patients demonstrated numerous ectatic submucosal vessels in the antrum, the appearances being somewhat similar to angiodysplasia of the colon. The gastric lesions were not shown by barium meal examination or angiography but gave characteristic appearances on endoscopy. We believe that Billroth I partial gastrectomy is the treatment of choice for this condition.

Effect of splenectomy alone and in combination with antilymphocyte serum on renal allograft rejection in the rat.

The effect of splenectomy alone and in combination with suboptimal doses of antilymphocyte serum (ALS) have been studied in the relatively weak (DA X Lewis)F1 to DA and the strong DA to Lewis renal allograft models. In the F1 to DA model, splenectomy alone produced a slight but significant prolongation of graft survival and in combination with a suboptimal dose of ALS gave excellent suppression of rejection. In the strong DA to Lewis model, splenectomy alone had no effect on graft survival and there were no additive effects with suboptimal doses of ALS. However, splenectomy alone produced a striking suppression of the lymphocytotoxic response to the kidney grafts in this combination. To elucidate the mechansim whereby splenectomy weakens the immune response, (DA X Lewis)F1 to DNA renal allografts were performed with the renal vein anastomosed to the recipient's portal vein. This procedure had no effect on graft survival.

Pharmacy practice in an operating room complex.

The steps involved in establishing a comprehensive pharmaceutical service in an operating and recovery room complex is described. Objectives of the operating room pharmaceutical satellite were to: (1) improve control of distribution, storage and charging for all drugs, especially Schedule II controlled substances; (2) reduce inventory costs and loss of revenue; (3) improve compliance with the drug formulary; and (4) establish patient-oriented pharmaceutical services. The pharmacy satellite improved inventory control and patient charging, assured continual access to all drugs and appropriate security for controlled substances, and expanded the pharmacy department's clinical, drug information and research activities.

Comparison of "early gastric cancer" in Britain and Japan.

Before the introduction of endoscopy, four out of 720 cases of gastric cancer were diagnosed before the cancer had breached the muscularis propia, an incidence of 0.5%. Using endoscopy and endoscopic biopsy, 10 out of 101 cases of gastric cancer were diagnosed at this "early" stage, an incidence of 10%. Their clinical, morphological, and histological characteristics are compared with those of Japanese "early gastric cancers" and reveal a remarkable similarity. The results of this study suggest that a higher proportion of British gastric cancers could be diagnosed at an "early" stage by more intensive investigation of dyspeptic patients using up to date radiological techniques, fibreoptic endoscopy, and endoscopic biopsy.
