An O-antigen processing function for Wzx (RfbX): a promising candidate for O-unit flippase.

O antigen is the major cell surface antigen of gram-negative bacteria, and the genes responsible for its synthesis are located in a single gene cluster. The wzx (rbfX) gene, which is characteristic of the major class of O-antigen gene clusters, encodes a hydrophobic protein with 12 potential transmembrane segments. We demonstrate that a wzx mutant accumulates undecaprenol pyrophosphate-linked O units which appear to be on the cytoplasmic side of the cytoplasmic membrane, suggesting that the wzx gene encodes a flippase for O-unit translocation across that membrane.

C-terminal half of Salmonella enterica WbaP (RfbP) is the galactosyl-1-phosphate transferase domain catalyzing the first step of O-antigen synthesis.

We previously showed that the product of the wbaP gene of Salmonella enterica serovar Typhimurium has two functions: it is involved in the first step of O-antigen synthesis (the galactosyltransferase [GT] function) and in a later step (the T function), first thought to be the flipping of the O-antigen subunit on undecaprenyl pyrophosphate from the cytoplasmic face to the periplasmic face of the cytoplasmic membrane. We now locate two wbaP(T) mutations within the first half of the wbaP gene by sequencing. Both mutants retain GT activity, although one was a frameshift mutation resulting in a stop codon 10 codons after the frameshift to give an open reading frame containing only 138 of the 476 codons in WbaP. We also show that there is a secondary translation starting within the wbaP gene resulting in the synthesis of a polypeptide with GT activity. These results indicate that the N- and C-terminal halves of WbaP are the T and GT functional domains, respectively. We now propose that the T block operates prior to the flippase function, probably at the release of undecaprenyl pyrophosphate-linked galactose from WbaP.

Induction of chinook salmon growth hormone promoter activity by the adenosine 3',5'-monophosphate (cAMP)-dependent pathway involves two cAMP-response elements with the CGTCA motif and the pituitary-specific transcription factor Pit-1.

In this study, the functional role of two cAMP-response elements (CRE) in the promoter of the chinook salmon GH gene and their interactions with the transcription factor Pit-1 in regulating GH gene expression were examined. A chimeric construct of the chloramphenicol acetyltransferase (CAT) reporter gene with the CRE-containing GH promoter (pGH.CAT) was transiently transfected into primary cultures of rainbow trout pituitary cells. The expression of CAT activity was stimulated by an adenylate cyclase activator forskolin as well as a membrane-permeant cAMP analog 8-bromo-cAMP. Furthermore, these stimulatory responses were inhibited by a protein kinase A inhibitor H89, suggesting that these CREs are functionally coupled to the adenylate cyclase-cAMP-protein kinase A cascade. This hypothesis is supported by parallel studies using GH4ZR7 cells, a rat pituitary cell line stably transfected with dopamine D2 receptors. In this cell line, D2 receptor activation is known to inhibit adenylate cyclase activity and cAMP synthesis. Stimulation with a nonselective dopamine agonist, apomorphine, or a D2-specific agonist, Ly171555, suppressed the expression of pGH.CAT in GH4ZR7 cells, and this inhibition was blocked by simultaneous treatment with forskolin. These results indicate that inhibition of the cAMP-dependent pathway reduces the basal promoter activity of the CRE-containing pGH.CAT. The functionality of these CREs was further confirmed by deletion analysis and site-specific mutagenesis. In trout pituitary cells, the cAMP inducibility of pGH.CAT was inhibited after deleting the CRE-containing sequence from the GH promoter. When the CRE-containing sequence was cloned into a CAT construct with a viral thymidine kinase promoter, a significant elevation of cAMP inducibility was observed. This stimulatory response, however, was abolished by mutating the core sequence, CGTCA, in these CREs, suggesting that these cis-acting elements confer cAMP inducibility to the salmon GH gene. The interactions between CREs and the transcription factor Pit-1 in mediating GH gene expression were also examined. In HeLa cells, a human cervical cancer cell line deficient in Pit-1, both basal and cAMP-induced expression of pGH.CAT were apparent only with the cotransfection of a Pit-1 expression vector. These results taken together indicate that the two CREs in the chinook salmon GH gene are functionally associated with the cAMP-dependent pathway and that their promoter activity is dependent on the presence of Pit-1

Steroidogenic factor 1 and estradiol receptor act in synergism to regulate the expression of the salmon gonadotropin II beta subunit gene.

The orphan nuclear receptor steroidogenic factor-1 (SF-1) regulates the expression of several genes involved in the reproductive function and development of the adrenal, the gonads, and the pituitary gonadotropes. It also confers the gonadotrope-specific expression of the glycoprotein hormone a subunit gene by the binding to a gonadotrope-specific element (GSE). In this study, we have shown that SF-1 transactivates the salmon gonadotropin II beta subunit (sGTHII beta) gene expression. SF-1 alone offered a slight but significant enhancement on sGTHII beta promoter activity (7.2 +/- 0.6 fold). However, it stimulated sGTHII beta gene expression dramatically (127 +/- 37 fold) when combined with the estrogen receptor (ER). This synergistic interaction was specific for sGTHII beta promoter as well as for both SF-1 and ER and was estradiol-dose dependent. 5'-Deletion studies of the sGTHII beta promoter identified two putative SF-1 binding sites (GSE) and one previously identified proximal estrogen-responsive element (pERE) at -274 bp involved in this activation. The two GSE sequences located at -354 bp (sGSE(3) and -162 bp (sGSE(2) upstream of the transcription site, although imperfect as compared with the consensus GSE, bound specifically to the in vitro-translated mouse SF-1 protein. 5'-Deletion studies, competition experiments, and site-directed mutagenesis showed that binding to pERE and GSE(2) were necessary for the SF-1/ER synergistic effect. These studies suggest that the synergistic interaction of SF-1 and ER, possibly through cooperative binding or protein-protein interaction, is essential in conferring a cell type-specific expression of the GTHII beta subunit gene.

Cocaine concentration-effect relationship in the presence and absence of lidocaine: evidence of competitive binding between cocaine and lidocaine.

UNLABELLED: To better understand the interaction between cocaine and lidocaine, we studied the cocaine's concentration-effect relationship for action potential duration (APD) and the rate of rise of phase 0 of the action potential (Vmax) of canine papillary muscle in the presence and absence of lidocaine. We measured APD and Vmax during programmed stimulation and superfusion with normal Tyrode's, 30 microM cocaine and 30 microM cocaine + 30 microM lidocaine. Using two microelectrodes, we simultaneously recorded action potentials from two sites during programmed stimulation and measured the conduction velocity and effective refractory period during exposure to normal Tyrode's, cocaine and cocaine + lidocaine. Cocaine with or without lidocaine delayed the plateau of the APD restitution curve. At 1000 msec cycle length, the addition of 30 microM lidocaine to the superfusate containing 30 microM cocaine shortened the time constant for reactivation of Vmax from 514 +/- 63 to 234 +/- 28 msec (P < .01). Lidocaine also improved the conduction velocity decreased by cocaine, but did not significantly change the effective refractory period. The configuration of cocaine concentration-effect curve for APD was biphasic. For cocaine concentrations < 100 microM, APD progressively shortened prolonged with increasing concentrations. As cocaine concentrations increased > 100 microM, APD progressively shortened. The addition of lidocaine to the superfusate with cocaine > 100 microM tended to attenuate the progressive APD shortening due to cocaine. Lidocaine shifted the curve correlating cocaine concentration and reduction of Vmax rightward, but preserved Emax at cocaine concentration > 225 microM. These findings suggest competitive antagonism between cocaine and lidocaine at a single sodium channel receptor. CONCLUSION: lidocaine displaces cocaine from the sodium channel receptor through competitive binding. Lidocaine may prove to be beneficial in reversing cocaine-induced slowing of ventricular conduction.

Improved counter-regulatory hormonal and symptomatic responses to hypoglycemia in patients with insulin-dependent diabetes mellitus after 3 months of less strict glycemic control.

This study of patients with insulin-dependent diabetes mellitus (IDDM) and hypoglycemia unawareness examines the effect of a period of less strict glycemic control on the defective counter-regulatory hormonal responses to and impaired awareness of hypoglycemia. In seven patients with IDDM with strict glycemic control and hypoglycemia unawareness, responses were studied with the use of a hyperinsulinemic hypoglycemic clamp test before and after 3 months of test treatment, which aimed at increasing daily mean blood glucose levels to 8 to 10 mmol/L, based on self-monitoring four times per day. The 3 months of test treatment elevated the patients' levels of glycosylated hemoglobin HbAlc from a mean of 6.9% (standard error [SE] 0.3%) at baseline to 8.0% (SE 0.3%, p < 0.05) and decreased the number of episodes of hypoglycemia (blood glucose level less than 3.0 mmol/L) per patient from a mean of 4.7 (SE 1.3) per week to 1.9 (SE 0.5) per week (p < 0.05). Although the same nadir (2.2 mmol/L) and duration of hypoglycemia were reached during each of the two clamp tests, the scores for sweating and lack of concentration were improved (p < 0.05) at the second test. The release of growth hormone and epinephrine during hypoglycemia were significantly improved (p < 0.05) at the second test, whereas the glucagon, cortisol and norepinephrine responses were unchanged. There were no significant differences in the results of the autonomic and cognitive function tests between the two tests. The results suggest that impaired hormonal and symptomatic responses to hypoglycemia in IDDM can be partially reversed by a modest rise in glucose levels for 3 months.

[Thoraco-cranial operation for treatment of lung cancer with brain metastasis].

Ten cases of lung cancer with brain metastasis who received simultaneous surgery for the primary lung tumor and for the metastatic intracranial tumor between 1990 and 1994 are reported. There was no death due to operation, nor were serious complications. With follow up until Feb. 1995, four patients died and its mean survival period was 8.25 months. Six patients are still alive with a mean survival period of 16 months, the longest one being nearly 3 years. The authors hold that, if the primary tumor is located at the periphery of the lung, the brain metastasis is solitary and patient's general condition is good, simultaneous thoraco-cranial operation is indicated and feasible. Postoperative adjuvant chemotherapy is needed to achieve better treatment results.

[Nosocomial bacterial infection in comprehensive intensive care unit].

OBJECTIVE: To establish the prevalence of nosocomial bacterial infection and changes in resistance in a comprehensive intensive care unit. METHODS: We retrospectively analyzed all the bacterial isolates of ICU, PUMC Hospital from 1989 to 1994, such as the species and resistance pattern. RESULTS: Among all bacterial isolates, the percentage of Gram-positive cocci steadily increased (16% in 1989, 33% in 1994), while that of Gram-negative bacilli decreased (84% in 1989, 67% in 1994). The most prevalent isolates of Gram-negative pathogens were Pseudomonas aeruginosa, Acinetobacter baumanii, inducible Enterobacteriaceae and Xanthomonas maltophilia, with imipenem and amikacin as the most susceptible antibiotics. Enterococci, oxacillin-resistant Staphylococcus epidermidis and oxacillin-resistant Staphylococcus aureus accounted for 44%, 21% and 10%, respectively. The resistant rate of these three Gram-positive bacteria to vancomycin were 19%, 0% and 0%. CONCLUSION: Understanding of the epidemiological data and changes of bacterial resistance profile is prerequisite to rational use of antibiotics, which is crucial to reducing the resistance of nosocomial infection pathogens.

[The effect of nimodipine and papaverine on delayed cerebral vasospasm in dogs].

OBJECTIVE: To evaluate the effect of nimodipine and papaverine (Pap) on the spastic arteries with delayed cerebral vasospasm (DCVS) in dogs. METHODS: A model of DCVS in dogs was induced by the cisterna magna injections of fresh autologous blood twice. The diameter of the basilar arteries with and without DCVS was measured with the aid of a JVC operative microes cope in vivo after nimodipine and Pap were directly placed around the arteries. RESULTS: The dilation of the basilar arteries was obviously induced by application of nimodipine and Pap in the dogs without DCVS no dilation was induced by application of nimodipine and slight dilation by application of Pap but in those with DCVS. CONCLUSION: The cerebral blood flow is improved by the effect of nimodipine on non-spastic arteries. Pap has been found to reverse slightly the narrowing of the basilar arteries with DCVS and to dilate the arteries without DCVS, but the vasodilation effect is transient.

[The relationship between placental pathology and certain pathogens detected in paraffin-embedded tissues in cases of intrauterine growth retardation].

OBJECTIVE: To study the relationship between placental pathology and certain pathogens detected in paraffin-embedded placental tissues in cases of intrauterine growth retardation (IUGR). METHODS: The PCR-DNA techniques were used for determination of pathogens, including toxoplasma, cytomegalovirus and herps simplex type II in paraffin-embedded placental tissues from 53 cases of IUGR. RESULTS: Among 53 cases of IUGR, the pathogens were detected in 19 cases (35.85%). The relationships of pathogens detected to placental pathology were analyzed. Pathogens were found in 10 of 18 placentas with villitis, 7 of 11 placentas with villous maturation retarded and 2 of 3 intervillous space infections respectively. The incidence of infection in the pathological placentas was 59.38%. CONCLUSIONS: The study suggested that the haematogenous infections could play important roles in unknown causes of IUGR.

Overexpression of a reversion-related protein in the revertant RR cells.

The cDNA insert of the plasmid p14-6 is found to be the 3'-untranslated region (3'-UTR) of the transcription factor for human interleukin-6, NF-IL6. This 3'-UTR is actively transcribed in the revertant cell line RR, which contains the p14-6 plasmid integrated into its genomic DNA. Simultaneously a protein specifically bound to this 3'-UTR is expressed in significantly larger amounts. Its overexpression is apparently related to the reversion of the malignant cellular phenotype. The properties of this protein, named BNF, and possible reasons for its overexpression are discussed, and hypothesis on the mechanism of reversion of the RR cells is proposed.

A prospective randomized study of medications after silicone intubation.

BACKGROUND AND OBJECTIVE: Following silicone intubation, most surgeons prescribe either an antibiotic or an antibiotic-steroid combination medication. No known controlled study has been performed to determine whether steroids in addition to antibiotics improve the postoperative outcome. PATIENTS AND METHODS: The author performed a prospective randomized study of 108 patients (118 eyes) who underwent silicone intubation over a period of 4 1/2 years. Sixty-seven patients received a combination of neomycin, polymyxin B, and dexamethasone and 41 patients received gentamicin for 2 weeks after surgery. A standardized surgical technique and instruments were used. Follow-up ranged from 8 to 44 months (average 19 months). RESULTS: No statistically significant difference was found in the failure rates when patients treated with an antibiotic-steroid combination were compared with those treated with antibiotic alone. CONCLUSION: Following silicone intubation, there appears to be no additional benefit of an antibiotic-steroid combination medication over an antibiotic alone.

In utero haematopoietic stem cell transplantation: current perspectives and future potential.

In utero transplantation (IUT) of haematopoietic cells is a new therapeutic option for families with increased risk of having a child with an inherited disorder. Immunological naiveté and the rapidly expanding haematopoietic system in the first trimester human fetus, make therapeutic intervention by IUT a real possibility for those disorders which can be diagnosed early in gestation. Fewer cells are required than in postnatal BMT and therapy can be offered before the pathological sequelae of a disorder become manifested. However, only a few cases of IUT have been performed in humans and it is imperative that consensus is reached quickly on issues such as cell numbers/cell types so that the benefits of this approach to treatment can be realised. This review presents the current status of IUT, the cases thus far recorded and offers a prospective view of developments in this rapidly expanding area.

Galeal flap: clinical application in periorbital and orbital reconstruction.

We report the use of a galea flap in the reconstruction of the orbit and the periorbital in region three representative cases. Two patients, who had communication between the orbit, paranasal sinuses, and cranium following orbital and periorbital tumor resection, required a physical barrier to partition these compartments and an immunological barrier to prevent infection. A third patient, who had a contracted socket and had been on systemic steroids chronically, required a tissue with intrinsic blood supply to sustain an implant and promote healing. The outcome in all three patients was gratifying. The surgical anatomy is reviewed, the surgical technique detailed, and potential applications and complications discussed.

A study of platelet specific antibody (anti-Sib(a)).

OBJECTIVE: To screen and analyse the platelet-specific alloantibody form the sera of patients who received multiple platelet transfusions or experienced side effects after platelet transfusions. METHODS: 156 patients, who received multiple platelet transfusion or experienced side effects after platelet transfusion, were collected from hospitals in Shanghai. The simplified sensitized erythrocyte platelet serology assay (SEPSA) was used to screen the platelet antibodies and the identification of specificities of the platelet alloantibodies was performed with a platelet panel of known antigens and absorption tests. RESULTS: Seventy-four sera were found positive. Among them 59 sera (47.4%) contained HLA antibodies, two sera (2.7%) platelet-specific antibodies, and 13 sera both HLA and platelet-specific antibodies. One of 2 sera containing platelet-specific alloantibody, the serum Zhu, was confirmed to have a monospecific platelet-specific alloantibody, the anti-Sib(a) (HPA-2b). CONCLUSIONS: We report the first case of anti-Sib(a) in Chinese Han population. The antigen frequency of Sib(a) is considered as one of the most important antibodies which will cause the platelet transfusion refractoriness in China.

[The study of BL21(DE3)/PET-11 as an expression system of human IL-2].

The coding region of hIL-2 cDNA is specifically amplified by PCR. A Nde I site is introduced into its 5'-end and its 3'-non coding region is partly deleted. After filling-in by Klenow enzyme, the DNA fragment is cloned into Sma I site of PUC18. Eight white colonies are screened and one (C1) contains Nde I site. DNA sequencing shows that non-specific mutation has not been found in its coding region. The DNA fragment digested by BamH I, Nde I of C1 plasmid is recovered and subcloned into PET-11 and transfered into BL21 (DE3). After IPTG induction, the Bacteria lysate is run directly on SDS-PAGE. Western blot test shows a specific rhIL-2 McAb binding band. The molecular weight is about 17000. Laser scanning indicates that the absorption area of IL-2 is about 5% of the total area.

[The influence of surfactant on the alveolar morphology in premature rabbits].

OBJECTIVE: To study the effects of treatment by surfactant manufactured by our institute on premature rabbits through observation of alveolar morphologic changes. METHODS: The morphology of alveolar area in the premature rabbits were quantitatively studied by morphometry. RESULTS: In the surfactant treated group, there was a reduction of alveolar number, and increases in alveolus area and the ratio of alveolus to alveolar septum. These changes were better than that in the control group. CONCLUSION: Our surfactant was effective for respiratory distress syndrome in premature rabbits.
