[Observation on inhibitory effect of tetrandrine on the growth of human skin fibroblast in vitro].

The effect of tetrandrine on the growth of human skin fibroblasts was observed in vitro culture. The result revealed that tetrandrine significantly inhibited the growth of fibroblast. The higher the concentration of tetrandrine, the higher the inhibition rate, forming a time-dependent relationship. The possible value of tetrandrine in the treatment of hypertrophic scar is discussed.

Report from the Chinese Bone Marrow Transplant Registry with special regard to autologous transplant.

OBJECTIVE: To study the current status of autotransplantation in the People's Republic of China and facilitate national and international exchange of relevant experiences. METHODS: On the basis of the materials collected from the member units around our country, various statistical methods, such as Kaplan-Meier and chi 2 tests, were used to estimate the probabilities of leukemia free survival (LFS). RESULTS: Up to July 31, 1996, a total of 1213 cases of stem cell transplantation (SCT) were performed in 61 medical units in the People's Republic of China, and 22 of the units are able to perform both allogeneic SCT (Allo-SCT) and autologous-SCT (ASCT). The remaining 41 units had the capacity only to perform autologous-SCT. There were 772 cases of autologous SCT performed. The three year probabilities of leukemia free survival for acute mylogeneous leukemia complete remission (AML-CR1) and acute lymphocyte leukemia complete remission (ALL-CR1) recipients were 56% and 42.8%, respectively. The three year probabilities of relapse were 44.8% and 47%, respectively. Among the patients with acute leukemia who attained CR1 within 40 days of diagnosis and who subsequently underwent autotransplantation within three to six months, the three year probabilities of LFS for acute mylogenous leukemia (AML) and acute lymphocyte leukemia (ALL) were 72% and 50%, respectively. CONCLUSIONS: Patients who attained complete remission (CR1) within 40 days after diagnosis and underwent subsequent autologous-SCT within three to six months of CR1 enjoyed a significantly increased LFS.

[Quantitative and objective evaluation of vocal function after vertical hemilaryngectomy].

The lately invented instrument, phono laryngograph, which can evaluate objectively vocal function, was used to determine separately in 30 normal adults and 30 patients with laryngeal carcinoma treated primarily by vertical hemilaryngectomy from 1984 to 1994. The results showed that mean air flow rate, intensity, MPT, have significant difference between surgical groups and normal group (P < 0.01). There is pitch difference between surgical groups and normal group (P < 0.05). We also found that the determined values have significant difference between postoperative two years and postoperative ten years groups (P < 0.01).

[Subjective and objective of evaluation of UPPP in the treatment of obstructive sleep apnea syndrome in short-term follow-up].

Uvulopalatopharyngoplasty (UPPP) has become a widely accepted method for treating obstructive sleep apnea syndrome (OSAS). The authors studied 52 OSAS patients treated with UPPP and assessed pre- and postoperative parameters including subjective clinical analysis and objective evaluation of computer test. 34(65.38%) OSAS patients felt satisfied of improving snoring sound 3-6 months later following UPPP. 46 patients reported an improvement in their symptoms of daytime somnolence and morning headache. 41 patients underwent follow-up sleep analysis computer (SAC) study; 19 of them (46.34%) had a reduction in AHI more than 50%; meeting our criteria for surgical success. All 41 patients had a significant reduction of total duration of apnea at sleep stage. The study suggests that the effectiveness of UPPP in treating OSAS is limited and subjective clinical symptoms following UPPP do not correlate with the objective finding of SAC; so it is important to stress the significance of follow-up study and necessity to further therapy.

[Blocking action of tetrandrine on the scar-collagen matrix contraction stimulated by transforming growth factor beta].

OBJECTIVE: To investigate the effect of Tetrandrine on the scar-collagen matrix contraction stimulated by transforming growth factor beta (TGF-beta). METHODS: The scar-derived fibroblasts were suspended in a solution of collagen matrix, and TGF-beta, they were associated to form a scar model in vitro. The influence of tetrandrine on the contraction indexes was observed. RESULTS: Tetrandrine plays a blocking action in the scar-collagen matrix contraction stimulated by TGF-beta, which was in a dose-dependent manner. CONCLUSION: This function of tetrandrine could be responsible for the anti-scar mechanism.

[Effect of dauricine on redistribution of glycoprotein IV in platelet membrane of patients with mitral stenosis].

OBJECTIVE: To investigate the effect of dauricine on the irreversible platelet aggregability of patients with mitral stenosis (MS). METHODS: Glycoprotein IV (GP IV) and thrombospondin (TSP) levels on the membrane surface of the stationary platelet or platelet activated by thrombin (0.05 U/ml, 0.1 U/ml, 0.5 U/ml, 1.0 U/ml) in 16 patients with MS were measured with flow cytometric method and compared with those of the healthy (14 cases). RESULTS: The GP IV level of stationary platelet, the GP IV and TSP levels of activated platelet in MS patients were higher than those in the healthy significantly (P < 0.01), while the TSP level of stationary platelet was not different between the patients and the healthy (P > 0.05). The GP IV redistribution on the activated platelet surface was apparently inhibited by dauricine (50 mumol/L, P < 0.05, P < 0.01) and the release of TSP from intracellular alpha-granules was inhibited by dauricine only in the activated platelets induced by thrombin of low concentration (0.05 U/ml and 0.1 U/ml, P < 0.05, P < 0.01), inhibiting effect was not found in those activated with high concentration of thrombin. CONCLUSION: The activity and reactivity to thrombin of platelets increased in MS patients, and dauricine was able to reduce the occurrence of the irreversible platelet aggregation in MS patients.

[Clinical study on Shuanghuanglian powder in treating children viral myocarditis].

OBJECTIVE: To study immune function of children viral myocarditis and to evaluate the clinical effect of Shuanghuanglian Powder (SHLP). METHODS: The authors determined serum COXB-IgM and the numbers of lymphocyte subsets CD3+, CD4+, CD8+, CD4+/CD8+ with viral myocarditis patients by ELISA and indirect immunofluorescent assay. Sixty-two cases were divided randomly into two groups. Thirty-two cases treated by conventional therapy with SHLP and the other 30 cases treated with conventional therapy alone were taken as control group. RESULTS: COXB-IgM was positive in 39 of 62 patients, which were significantly different with those of normal controls (P < 0.001). In addition, the level of CD4+ cells and the ratio of CD4+/CD8+ were decreased while CD8+ increased. After treatment with SHLP, the recovery of symptoms, signs and immune function in patients were better than that of controls. These changes were significantly different (P < 0.01). CONCLUSIONS: There were disturbance of immune regulatory function with children viral myocarditis patients and SHLP is an effective drug in treating children viral myocarditis.

[The value of cephalometric analysis in diagnosis and treatment of obstructive sleep apnea syndrome].

OBJECTIVES: To study the value of cephalometric analysis in the diagnosis, treatment option and evaluation of prognosis. METHODS: A detailed cephalometric analysis was performed using lateral X-ray films from 50 cases with obstructive sleep apnea syndrome (OSAS) and 30 age- and sex-matched controls. RESULTS: Statistical results showed that OSAS patients were different from controls in the following aspects: 1. Their uvula, soft palate and tongue were significantly enlarged. 2. The hyoid bone was displaced inferiorly and posteriorly. 3. The posterior airway space (palatopharyngeal plane) was narrow. 4. The hard palate was elongated. 5. The mandibular body was retropositioned. Fifty unselected consecutive OSAS patients(mean AHI 51.42 +/- 3.96, Medilog SAC 847, Oxford UK) underwent LAUPP using CO2 laser. Twenty-five cases were followed-up for 4-8 months. 10/25 had obtained a reduction in AHI > 50%, 15/25 had AI < 20 times. The success rate was compared with cephalomentric findings. CONCLUSION: Cephalometry has an important diagnostic and predictive value in the clinical investigation of patients suspected of having OSAS.

[Pathologic changes of palatopharyngeal soft tissue in obstructive sleep apnea syndrome].

OBJECTIVE: To study the pathologic changes of palatopharyngeal soft tissues in patients with OSAS. METHODS: Different analyses have been done on uvular muscles (UM) and/or palatopharyngeal muscles (PPM) in patients with OSAS(n = 27), SS(n = 17) and controls (n = 15). The transverse sections were sequentially stained for HE, myofibrillar ATPase, NADH-TR, Lillie Oil Red-O. Ten specimens were evaluated by transmission electron microscopy(TEM). RESULTS: Patients with OSAS have a significantly smaller percentage of muscle in the uvula than that of SS patients (15.0 +/- 3.0 vs 20.3 +/- 5.5), The percentage of muscle is inversely related to the percentage of fibrous tissue in OSAS group (r = -0.75, Y = 42.6 - 0.64X); The total number and area of muscle fibers, the number and area of type I muscle fibers are significantly lower in OSAS than those in SS (800 +/- 345 vs 1200 +/- 360n; 1.10 +/- 0.56 vs 1.70 +/- 0.60 micron 2; 156 +/- 95 vs 360 +/- 225 n; 920 +/- 350 vs 1365 +/- 540 micron 2 respectively); PPM: The percentage of type I muscle fibers within total muscles and its area are significantly lower in OSAS than those in SS and normal controls(20.5 +/- 4.0 vs 34.5 +/- 4.5 n; 20.5 +/- 4.0 vs 35.5 +/- 4.7; 860 +/- 360 vs 1260 +/- 340 micron 2; 860 +/- 360 vs 1350 +/- 370 micron 2; respectively); the muscles from OSAS patients seemed to show neurogenic alterations. Structurally altered fibers shows myofibrillar loss, increased number of vesicles, and rough Z line and so on. CONCLUSION: Our finding suggest that the neurogenic functional disturbance of pharyngeal dilator muscles may play an important role in causing the airway collapse in OSAS.

[Study on cis-acting elements of rat glutathione S-tranferase P1 gene].

OBJECTIVE: To investigate the cis-acting regulatory DNA elements in 3.0 kb 5' flanking upstream region of rat glutathione S-tranferase (rGSTP1) gene. METHODS: Using exonuclease III (Exo III) and S1 nuclease, a series of deletions were made in the 3.0 kb region, then fused into highly sensitive luciferase reporter vector. The deleted mutants were transfected into HeLa cells. Luciferase activities of the transfected cell lysates were assayed. RESULTS: Deleted from -2.9 kb to -2.3 kb, luciferase activity had dropped about 67% (P < 0.05). The element between -2.5 kb and -2.2 kb could increase gene expression activity regardless of orientation and position. Further deleted from -1.8 kb to -1.3 kb, luciferase activity increased about three times (P < 0.05). CONCLUSIONS: There are enhancer element between -2.5 kb and -2.2 kb and a negative regulatory sequence between -1.8 kb and -1.3 kb in the upstream region of rGSTP1 gene.

[Coexisting pheochromocytoma and renal artery stenosis].

OBJECTIVE: To improve the diagnosis of coexisting pheochromocytoma and renal artery stenosis. METHOD: Four patients with coexisting pheochromocytoma and renal artery stenosis were treated. RESULT: After removal of the pheochromocytoma, the blood pressure became normal in two patients combined with resection of kidney with renal artery stenosis, and one patient with local adhesion loosened. The hypertension, however, persisted in another patient after the resection of pheochromocytoma even though the long segment of renal artery stenosis was relieved. CONCLUSION: After pheochromocytoma was diagnosed with its known position, coexisting renal artery stenosis should be suspected according to one of the following conditions: (1) Vascular murmurs are noted at the area of the upper abdomen or back; (2) One of the kidneys becomes less sized; (3) Using the inhibitor of receptor alpha only has no effect lowering blood pressure and it is necessary to add the inhibitor of angiotension; (4) Postoperative hypertension does not resolve when no remaining pheochromocytoma is present. the renal artery stenosis can be reversed after both removal of pheochromocytoma and loosening of adhesion in case of stenosis due to the pressure of the tumor and adhesion caused renal artery angling and local stricture. Extensive stenosis or closure, however, can not be corrected with severe atrophy of renal parenchyma due to pheochromocytoma, and the resection of kidney should be indicated.

[Expression of bFGF in primary human lens epithelial cells].

OBJECTIVE: To study the mechanism of after cataract formation and investigate the expression of basic fibroblast growth factor (bFGF) in primary human lens epithelial cells (PHLECs). METHODS: Human lens epithelial cells were cultured in vitro, and the expression of bFGF was detected with immunohistochemistry and in situ hybridization. RESULTS: PHLECs express bFGF in protein and mRNA levels. CONCLUSION: Based on bFGF promoting PHLEC growth and differentiation, the results suggest that bFGF from PHLECs take part in promoting such cell growth and differentiation.

[Experimental observation of foreign gene transfer with lipofectin to the primary human lens epithelial cells].

OBJECTIVE: To determine whether an exogenous gene carried by lipofectin can be introduced into the primary human lens epithelial cells. METHODS: Plasmid DNA with beta-galactosidase gene carried by lipofectin was applied to primary cultured human lens epithelial cells. Gene expression was detected by enzymatic color reaction using X-gal as a substrate in the 2 and 6 days of expression after 12, 24, 36 hours of transfection respectively. The gene transfer positive rate of the lens epithelial cells was counted. RESULTS: The foreign gene could be transferred into primary human lens epithelial cells by lipofectin. In the expression of 2 days, transfer positive rate was up to 48% after 24 hours of transfection. CONCLUSIONS: Efficient and stable transfer of the functional gene can be achieved by lipofectin into the lens epithelial cells. Lipofectin is an available and a promising vehicle for delivering aim gene and studying on the mechanism of physiology and pathology of lens epithelial cells.

[Determination of 6 isoflavonoids in the hairy root cultures of Astragalus membranaceus by HPLC].

An accurate RP-HPLC method for the determination of 10-hydroxy-3, 9-dimethoxypeterocarpan, (3R) 8,2'-dihydroxy-7,4'-dimethoxyisoflavan, formonenetin (7-hydroxy-4'-methoxyisoflavone), 8, 3'-dihydroxy-7, 4'-dimethoxy-isoflavone, 2'-hydroxy-3', 4'-dimethoxyisoflavan-7-O-glucopyranoside and alycosin (7,3'-dihydroxy-4'-methoxyisoflavone) in the hairy root cultures of Astragalus membranaceus was developed. Among them, 10-hydroxy-3, 9-dimethoxypeterocarpan, (3R) 8,2'-dihydroxy-7,4'-dimethoxyisoflavan and 8,3'-dihydroxy-7,4'-dimethoxyisoflavone were new compounds in Astragalus membranaceus. The analytical column was Nucleosil C18. The mobile phases were methanol-water = 3:2 and 1:1 (v/v). The detection wavelengths were 254 nm and 280 nm. Calibration graphs were rectilinear between 2.5 micrograms and 12.5 micrograms. The recoveries were between 96.47% and 103.33%. The relative standard deviations (RSDs) of measurement precision test were 2.57%-6.52%. The flavonoid contents in the hairy root cultures were between 0.0005% and 0.0065% (dry weight).

[Longitudinal surveillance of sensitivity of Plasmodium falciparum to pyronaridine in south Yunnan].

AIM: To understand the changes in the sensitivity of Plasmodium falciparum to pyronaridine in the south of Yunnan Province. METHODS: WHO standard in vivo and in vitro microtest was used. RESULTS: In 1984-1985 and 1995, 36 and 39 cases of acute falciparum malaria were observed, the average defervescence time being 32.7 +/- 16.0 h and 56.2 +/- 27.4 h, the mean asexual parasite clearance time, 64.2 +/- 22.9 h and 55.3 +/- 11.8 h, and the recrudescence rate of patients followed up for 28 days, 15.2%(5/33) and 37.5%(9/24), respectively. The resistance rates of resistant isolates by in vitro microtest were 0(0/24), 8.8%(3/34), 13.8%(4/29) and 36.4%(4/11) in 1988, 1990, 1992 and 1995, respectively, while the corresponding ID50 were 13.0, 20.0, 32.0 and 40.0 nmol/L, ID95 were 48.0, 136.0, 325.0 and 330.0 nmol/L, the mean concentrations for complete inhibition of schizont formation were 32.6, 99.5, 148.2 and 190.1 nmol/L, respectively. CONCLUSION: The sensitivity of Plasmodium falciparum to pyronaridine was found to have a tendency of continuous dropping in south Yunnan.

[Detection of TT virus infection and clinical investigation in patients with liver diseases].

104 serum samples from patients with liver diseases were detected for TTV DNA by nested-PCR method. The results showed that 25 of 104(24%) were positive for TTV DNA. Among 104 samples, 48%(12/42) were positive for nonA-E and G 19% (4/12) were positive for hepatitis A 25% (8/32) were positive hepatitis B 11.1% (1/9) was positive hepatitis C 4 of 6(66.7%) fulminant hepatitis cases were detected positive for TTV DNA. The partial gene of two TTV isolates from the patients were cloned and sequenced. The sequence analysis shows that there is 98.4% nucleotide identity between AB008394(from Japan) and one isolate from the patient (TTVEC023). The results suggest that TTV is associated with acute, chronic and fulminant hepatitis. TTV infection would be the important cause of non A-E and G hepatitis.
