Thinking about the threat of nuclear war: relevance to mental health.

Ontario students in grades 7-13 responded to open-ended and multiple-choice questions about future concerns, particularly unemployment, job/career plans, and the nuclear threat. Although worries about nuclear war were more frequent than those in other areas, their expression was associated with a sense of social efficacy rather than with feelings of helplessness.

Reversible hypercapnic respiratory insufficiency in scleroderma caused by respiratory muscle weakness.

A patient with scleroderma presented with hypercapnic respiratory failure. Evaluation of pulmonary mechanics revealed severe restriction caused in part by respiratory muscle weakness. Treatment with prednisone corrected hypoventilation, improved symptoms, increased lung volumes, returned respiratory muscle strength to normal range, but did not change the degree of lung stiffness. This case demonstrates that restrictive patterns in scleroderma can be due to either lung or chest wall disease and that the latter may be reversible. If respiratory muscle weakness is present with restrictive ventilatory patterns in patients with scleroderma, a therapeutic trial of corticosteroid is warranted.

Current status of the field: an anthropological perspective on the behavior modification treatment of alcoholism.

This chapter discusses behavior modification treatment for alcoholism in terms of the potential influence of cultural factors on the treatment process. It is suggested that cultural factors may be an important determinant of treatment involvement and treatment success. Four categories of cultural factors are identified and discussed: (1) folk beliefs about the cause and treatment of illness; (2) beliefs about control of one's behavior: (3) socialization techniques; and (4) acculturation. It is also suggested that behavior modification treatments, compared to other general treatments, do less harm to an individual's cultural identity while, at the same time, they provide an opportunity for cultural factors to be used in the treatment selection process.

Eosinophilic fasciitis.

Shulman's syndrome includes fasciitis of rapid onset, sclerodermoid skin changes, absence of Raynaud's phenomenon, absence of visceral manifestations of progressive systemic sclerosis, hypergammaglobulinemia, transient eosinophilia, and a dramatic response to systemic corticosteroid therapy. Although this syndrome may be a variant of scleroderma, its dramatic responsiveness to corticosteroid therapy is a particularly distinguishing feature.

The nurse practitioner as a teacher of physical examination skills.

The service role of nurse practitioners has been widely recognized (1). However, their potential as clinical instructors of physical examination skills has not been appreciated. This article details a new first-year medical school course in physical examination in which nurse practitioners are key members of the instructional team.

Information management in clinical practice.

Information management is essential for optimal delivery of health care services to individuals and the community. Current information techniques--largely dependent on the individual patient record--cannot effectively store, process, retrieve, and communicate the vast amount of data and information which is integral to the comprehensive health care process. Computerized techniques similar to those which are an indispensable part of almost all scientific, commercial, and administrative sectors of society are urgently needed to support a fully effective health care system.
