Clinical laboratory evaluation of the Perkin-Elmer KA-150 Enzyme Analyzer.

We evaluated the Perkin-Elmer KA-150 Enzyme Analyzer in a clinical laboratory situation according to a rigid evaluation protocol. The factors evaluated for five commonly assayed enzymes were as follows: (a) rate of analysis of patient samples, (b) analytical linearity, (c) overall precision (within-run and between-day), (d) carry-over from a sample with activity near 1000 U/liter, (e) error detection at low and high concentrations, (f) correlation of values with those obtained by methods used in our laboratory and (g) normal-value ranges. We find the KA-150 to be an effective and rapid enzyme analyzer.

Employment of MEDEX graduates and trainees. Five-year progress report for the United States.

The MEDEX approach to the training and deployment of physician extenders is described and contrasted with other physician-extender training models, and results of a survey of 277 MEDEX graduates and 207 trainees are presented. Practically all MEDEX graduates and trainees are employed in full-time practice and are working with fee-for-service family physicians in the rural areas of the United States, which suggests that the strategy used to place Medex practitioners into communities requiring additional health services has been successful.

Treatment of trichuriasis with a new drug, mebendazole.

Effectiveness of mebendazole, a new anthelminthic drug recommended for treatment of Trichuris trichiura infection, was evaluated in 50 children attending a pediatric outpatient clinic in New York City. Mebendazole, 100 mg. administered orally twice daily for three days resulted in an apparent cure of 48 children. Posttreatment stools of the remaining two children had a substantial reduction in the number of eggs. A second course of treatment, identical to the first, led to the elimination of eggs in the stool specimens of these two children. Mebendazole appears to be an effective drug against Trichuris trichiura.

Measurement and projection of the demand for health care: the Chilean experience.

Projection of the probable demand for health services over time is one of the most important-and difficult-aspects of the health planning process. Not only must the planner contend with many variables external to the health sector, but also with the difficult to predict correlations between the supply of health, services and the resultant demand. This article briefly reviews some of the principal demand projection methods in use and then describes in detail the one adopted in the Chilean health manpower study. The demand portion of the study in Chile involved three main elements: 1) a sample survey which measured the met and unmet demand for medical, dental, and hospital services as a function of six population variables (age, sex, location, income, educational level, and medical insurance status); 2) a baseline demand projection which takes into account the probable effects of changes in these six variables over ten and 20 years on the utilization of services; and 3) an alternative projection which postulates the fulfillment of certain targets for the improvement of health care. The approach offers the planner a number of important analytical and programmatic advantages compared with other methods now available and are discussed along with their limitations.

Clinical management of blunt trauma patients with unilateral rib fractures: a randomized trial.

Optimal pain management is essential in blunt trauma patients sustaining significant chest trauma. The purpose of this randomized prospective trial was to measure the difference in pulmonary function in nonintubated patients with unilateral multiple rib fractures receiving two modalities of pain relief: systemic narcotic medications alone or local anesthetics given by intrapleural catheter (IPCs). Forty-two patients were randomized to receive systemic narcotic medications or IPCs for pain control. The patients with IPCs statistically had more compromised pulmonary function as measured by forced vital capacity (FVC) on admission; however, they tended toward a greater objective improvement of FVC on discharge. When analyzing a cohort of severely impaired patients (initial FVC < 20%), half of the systemic medication patients compared to only 10% of the IPC group failed and required another mode of therapy. Catheter complications were minor and did not contribute to overall morbidity. The IPC patients had fewer failures than the systemic medication patients.

Purification and characterization of a factor which inhibits retrovirus replication.

Chick embryo cells (CEC) secrete a factor which inhibits HIV1 replication. Non-productive infection of CEC with vesicular stomatitis virus increases 5-10-fold the synthesis of this factor. After purification by FPLC the biological activity is associated with a heat-resistant protein of approximately 150 kDa, composed of two subunits of 70 and 58 kDa and charged negatively. Monoclonal antibodies raised against this protein block the inhibitory activity of the purified protein and react with the 150-kDa and 58-kDa proteins in Western blots.
