[Obsessional-type compulsive behavior caused by bilateral circumscribed pallidostriatal necrosis. Encephalopathy caused by a wasp sting].

A previously healthy young man developed encephalopathy after a wasp sting in 1968. Neurological complications included chorea, currently of a minor degree, replaced by buccofacial dyskinesias and compulsive movements. The most disabling consequences were of psychic nature, the patient presenting an unusually marked compulsive activity of an obsessional type. The whole psychic picture however was quite different from that of the usual neurosis, since there was neither anxiety nor doubt, but a mental vacuum contrasting with almost normal intellectual and affective capacities. It was as though there was a loss of the auto-activation system of psychic life, postulated by dynamic psychologists. C.A.T. demonstrated bilateral pallidostriatal necrosis. This suggests that the striatum plays a motor and psychic role. In the field of motor control, its hypoactivity appears to determine not only chorea but dyskinesias similar to tics and compulsive movements. In the psychic field, the striatum appears to play the very general role of an auto-activation system of intellectual and affective life, its deficiency leading to mental activity of a compulsive nature. Improvement in knowledge of striatal biology should clarify not only the basic mechanisms of chorea, but those of tics and obsessions.

[Solitary intracranial plasmacytoma : report of a case and review of the literature (author's transl)].

The authors report the case of a 71 year old woman affected by a solitary intracranial plasmacytoma. Computed tomography statement and monoclonal gammapathy found in the serum and in the cerebrospinal fluid permitted the diagnosis. Brain tumor and gammapathy improved after radiotherapy. A review of the 17 cases reported in the available literature showed some characteristics of these tumors : epidemiology, localization, evolution.

[Acute non-traumatic pancreatitis in childhood. Report of 9 cases (author's transl)].

The authors report 9 cases of acute non traumatic pancreatitis in children. The most common symptom in their patients is atypical abdominal pain often associated with vomiting (5 cases) and shock (4 cases). Among laboratory investigations: --High serum amylase level (average: 1 045 UI/l) is constant, associated with hypocalcemia and hyperglycemia in 3 patients. --Radiographic findings on plain film of the abdomen are diagnostic in 4 cases. --Abdominal ultrasound is the most reliable test and positive in the only patient on which performed. Among etiologies, drug induced pancreatitis is the most common (5 cases) due to combined Prednisone-L-Asparaginase (4 patients): --A duodenal ulcer and a case of choletithiasis are reported. --In 2 patients no determinant factors are found. A good response to parenteral nutrition, gastric suction and antisecretory agents is observed in 7 cases. 2 leukemic patients died shortly after the acute episode.

[The treatment of organic pain of the peripheral nervous system using clomipramine. 30 cases (author's transl)].

Clomipramine used alone is an affective analgesic against organic pain due to a lesion of the peripheral nervous system, in particular post-herpetic pain. The quality of the result is related to the dose, and hence to tolerance. Fluohydrocortisone is the most powerful drug against orthostatic hypotension, the chief cause of limitation of treatment. No complications were seen, even in very elderly patients.

Prolactin-secreting pituitary adenoma in a man with gigantism: a case report.

A prolactin-secreting pituitary adenoma was removed trans-sphenoidally from a 37 years old man with gigantism (218 cm). Serum levels of prolactin (PRL) were elevated pre-operatively and decreased after administration of L-Dopa with no increase after TRH as is usually observed in PRL-secreting adenomas. Growth hormone (GH) and somatomedin serum levels were normal with no modification of GH after insulin hypoglycemia, oral glucose loading or L-Dopa. Morphological examination of the tumour demonstrated the presence of lactotrophs by light and electron microscopy and by immunofluorescense staining. No somatotrophs were found. In this unique case, the relationship between a PRL-secreting adenoma and gigantism is discussed.

[Contribution to anatomical and clinical studies of avoidance phenomena (author's transl)].

The authors report 5 cases of avoiding phenomenon of the hand from parietal lesions. Four of these patients were operated for intractable epilepsy, from a parietal cortectomy that implicated a certain amount of white matter. The avoiding phenomena observed are analyzed as elementary motor perturbations rather than a disturbance of motor comportment. The authors distinguish two types of avoiding phenomena: 1--Avoiding phenomenon of dystonic type occurs during any motor activity and persists during all its development. It is probably related to the removal of the cortex of the post-central gyrus and of the anterior part of the superior parietal lobule; 2--The initial avoiding phenomenon is brief and occurs only at the beginning of the movements which implicate solely the upper limb; it seems related to the removal of the internal part of the superior parietal lobule that corresponds approximately to areas 5 and 7 from Brodmann. Some hypotheses are discussed concerning the possible physio-pathological mechanisms of these disturbances.
