[A clinical trial of pefloxacin (Abaktal) in the treatment of acute pyelonephritis].

The therapeutic effect of Pefloxacin in the treatment of acute pyelonephritis was examined in a randomized clinical trial. The patients in the control group have been treated by gentamycin. With a success rate of 80% in the treatment of acute pyelonephritis caused by various bacteria (E. coli, Pseudomonas, Klebsiella, Staphylococcus aureus, Proteus mirab.), a side frequency of side effects and a good compliance Pefloxacin is recommended as an effective antimicrobiell drug.

Localized injections of botulinum toxin for the treatment of focal dystonia and hemifacial spasm.

Medical treatment of dystonia usually results in an incomplete response and is frequently unsuccessful. Peripheral surgical therapy is available for some focal dystonias, but may only offer temporary relief and may have unacceptable complications. We have used local injections of botulinum toxin into the appropriate muscles for treatment of disabling focal or segmental dystonia in 93 patients with torticollis, blepharospasm, oromandibular dystonia (OMD), limb dystonia, lingual dystonia, and dystonia adductor dysphonia, in addition to four patients with hemifacial spasm. Significant relief of motor symptoms was seen in 69% of the patients with blepharospasm and 64% of patients with torticollis; 74% of the latter group with pain experience relief. Relief of symptoms was noted in most patients with OMD and limb dystonia, and all with lingual dystonia, dystonic adductor spastic dysphonia, and those with hemifacial spasm. Benefit averaged 2 1/2-3 months initially; however some patients experienced longer relief with subsequent injections. Adverse effects were transient, although 2 patients developed antibodies against the toxin, and we documented evidence for distant effects in others. This approach of chemically weakening contracting muscles in focal dystonia offers many advantages over pharmacotherapy and surgical therapy. Additional experience is needed to explore the proper doses, and potential for long term adverse effects.

Restriction patterns of adult chicken globin genes at early and late stages of embryonic development.

Globin mRNA isolated from anemic chicken was transcribed into cDNA and integrated into the Pst I cleavage site of plasmid pBR 322. After cloning in E. coli strain HB 101 and colony hybridization with 125I-labelled globin mRNA the plasmids of individual clones were characterized by hybrid arrested cell-free translation. Thus we could isolate clones containing alpha or beta globin chain nucleotide sequences. DNA was isolated from chicken blastoderms incubated for 18-20 h and from 11 d chicken embryos. A comparison of the restriction maps of the DNA from the two developmental stages with labelled nick translated plasmids and labelled cDNA did not indicate any globin gene rearrangements between these two stages of embryonic development. We conclude, that the adult chicken globin genes show a constant genomic organization during embryonic development. However, the restriction patterns of the globin gene family of the chicken strain investigated revealed some differences after 2 generations of propagation.

Noninvasive assessment of myocardial contractility in asymptomatic patients with sever aortic regurgitation and normal left ventricular ejection fraction at rest.

In 14 asymptomatic patients with isolated aortic insufficiency the slope k of the end-systolic pressure-volume relation was determined noninvasively with equilibrium radionuclide angiography. The results were compared with changes in left ventricular ejection fraction during maximal physical stress. Nine normal volunteers served as a control group. Patients with aortic insufficiency did not differ significantly from the control group with respect to left ventricular ejection fraction at rest (aortic insufficiency 62 + 8 percent, control 65 +/- 6; probability [p] = not significant [NS]), physical work capacity (aortic insufficiency 113 +/- 32 watts, control 117 +/- 25; p = NS) or age (aortic insufficiency 40 +/- 10 years, control 47 +/- 7; p = NS). The slope (k) of the end-systolic pressure-volume relation was found to be significantly lower in the group with aortic insufficiency (3.1 +/- 1.1) than in the control group (4.1 +/- 0.5; p less than 0.05). Patients with aortic insufficiency could be classified into two subgroups with respect to the slope k. In subgroup A (n = 7) the slope fell within the normal range (4.0 +/- 0.6) as defined by the control group, and the left ventricular exercise reserve was normal (6 percent +/- 1). In subgroup B (n = 7) the slope was significantly lower (2.2 +/- 0.6, p less than 0.01), indicating depressed myocardial contractility, and all patients experienced left ventricular dysfunction during exercise (left ventricular exercise reserve -5 +/- 5 percent). Thus, noninvasive determination of the end-systolic pressure-volume relation identified two subsets of asymptomatic patients with aortic insufficiency, one with impaired myocardial contractility and normal left ventricular exercise reserve and a second group with depressed myocardial contractility and left ventricular dysfunction during exercise. Therefore, an abnormal baseline contractile state in asymptomatic patients with aortic insufficiency may be uncovered by noninvasive determination of the end-systolic pressure-volume relation or by assessing the left ventricular exercise reserve. Serial studies in a larger group of patients undergoing surgical correction of the valve lesion are indicated to determine whether this information will be helpful in evaluating when to operate on asymptomatic patients with aortic insufficiency.

[Urinary incontinence in women, without urodynamic studies].

From a series of 212 cases of urinary stress incontinence in women, the author analyses the results of 41 operations by retro-pubic vaginal fixation and 110 operations by aponeurotic support of the bladder neck. From this second group of 110 operations, 100 cases were reviewed with 93 successes, 3 improvements and 4 failures. The results obtained after one year were definitive. The author stresses the importance of the pre-operative assessment of the clinical signs and symptoms. Out of 90 cases of pure stress incontinence, with no other associated disturbance of micturition, there was a 95,5% success rate. 16,6% of cases had post-operative retention which was easily treated by simple measures. However, out of the 10 cases of mixed stress incontinence, with associated symptoms of urinary urgency, the results were favorable in only 70%. The author believes that urodynamic studies have a certain role in the investigation of the cause for a failed operation. They may even be useful in the investigation of the urodynamics of the vesicosphincteric apparatus of the woman with stress incontinence associated with other disturbances of micturition. However, clinically pure urinary stress incontinence does not require urodynamic investigation and can be corrected by lifting up the anterior vaginal wall (Bonney's manoeuvre). According to the author, the aponeurotic sling is the best way of treating these patients. The results are better when there is not a simultaneous cure of cystocele. Retro-pubic vaginal fixation is only used in cases of minor, discrete stress incontinence in elderly women and to complete surgery for prolapse.

Chicken ribosomal DNA: gene frequency and purification by R-loop hybridization.

The genes for ribosomal RNA of chicken are present in about 200--240 copies per haploid genome. The ribosomal genes were separated from the bulk of DNA by different methods. In CsCl-actinomycin D gradients the GC-rich ribosomal DNA is shifted to the lower density side of the bulk of DNA. After hybridization with iodinated ribosomal RNA under conditions where R-loops are formed ribosomal DNA has an increased density. The R-loops can be separated from the bulk of DNA in Cs2-SO4 gradients.

Radiation dose and labeling of hippuran.

Renal morphology and renal function were studied with iodine-123-hippurate in 98 patients with various renal diseases. Sequential scintigraphy with iodine-123-hippurate demonstrates much better renal blood flow, morphology and function than iodine-131-hippurate. A method of labeling hippurate in a short time (2 h) with high efficiency (95--100%) is described. Radiation exposure to various organs was calculated and significant differences in radiation exposure to the thyroid are demonstrated.

[Estimation of regional cerebral blood flow with the gamma-camera (author's transl)].

Using a computerized gamma camera regional cerebral blood flow and regional cerebral blood transit time are measured after injection of Xenon 133 and Technetium 99m respectively into the internal carotid artery. Equipment, technical procedures and methodology are described. Typical cases are presented to describe the usefullness of this procedure.
