[X-ray computed tomography coupled with cisternography in chronic pericerebral effusions in young children].

The authors study 26 children with macrocranium and pericerebral effusion as diagnosed by C.T. Three groups are described: 10 with the classical C.T. features of subdural effusion, 12 with features of external hydrocephalus and 4 with massive pericerebral effusion. C.T. performed after lumbar subarachnoid injection of contrast medium, shows that all the effusions are subdural. In only one case, opacification of the subdural space occurred. The authors conclude that plain C.T. alone is unable to demonstrate the precise localization of pericerebral effusion in children and that all these effusions are, in their experience, subdural in localization.

MRI of hypothalamic hamartomas in children.

Eight cases of hypothalamic hamartomas (HH) diagnosed in children by clinical, biological and CT studies have been explored by MRI and compared with a control group of twenty children without hypothalamohypophyseal signs. MRI revealed the hamartoma in all cases. Its location and position relative to the floor of the third ventricle was precisely determined. The signal intensity of the lesion was identical to that of normal grey matter on T1-weighted images (300/600.28), but an hyperintense signal was observed over T2-weighted images (1800.60/120) in seven of the eight cases. These data suggest that HH are somewhat different in structure from normal brain tissue. MRI variations in signal intensity should be taken into account in the diagnostic work-up of these lesions.

Facial vascular malformations in children. Conventional and digital, diagnostic and therapeutic angiography.

The authors present their experience with conventional and digital angiography of vascular malformations of the head and neck in children. 22 hemangioendotheliomas, 8 venous angiomas, and 3 arteriovenous fistula were studied. 22 patients were embolised. DSA offers many advantages during the diagnostic as well during the therapeutic phase of angiography. Embolization appears to have a major role in treatment of such vascular malformations.

Separation of diffusion and perfusion in intravoxel incoherent motion MR imaging.

Intravoxel incoherent motion (IVIM) imaging is a method the authors developed to visualize microscopic motions of water. In biologic tissues, these motions include molecular diffusion and microcirculation of blood in the capillary network. IVIM images are quantified by an apparent diffusion coefficient (ADC), which integrates the effects of both diffusion and perfusion. The aim of this work was to demonstrate how much perfusion contributes to the ADC and to present a method for obtaining separate images of diffusion and perfusion. Images were obtained at 0.5 T with high-resolution multisection sequences and without the use of contrast material. Results in a phantom made of resin microspheres demonstrated the ability of the method to separately evaluate diffusion and perfusion. The method was then applied in patients with brain and bone tumors and brain ischemia. Clinical results showed significant promise of the method for tissue characterization by perfusion patterns and for functional studies in the evaluation of the microcirculation in physiologic and pathologic conditions, as, for instance, in brain ischemia.

[MRI of aortic arch anomalies in children. Initial results].

A prospective study by nuclear magnetic resonance in 7 infants with respiratory disorders and with a barium swallow suggestive of anomalies of the aortic arch was conducted. It allowed the definition of an investigation protocol including sedation, RF coil adapted to infants size, naso-gastric tube placement, thin MR sections. Sequences are T1 weighted with ECG gating in two perpendicular frontal oblique and transverse oblique planes determined on a scout sagittal MR acquisition. Precise determination of the anatomy of the vascular malformation and its relationship with the trachea and esophagus were obtained. After definition of the investigation protocol, MRI was found to be a very effective evaluation technique for aortic arch anomalies. It can be proposed as an alternative to preoperative digital angiography. Its indications remain to be defined in relation to the clinical features and the results of the chest X-ray and barium swallow.

[Cystic or pseudocystic ovarian masses in young girls. The role of ultrasonic diagnosis].

Twenty adnexal tumors have been discovered between 7 and 16 years old girls. With few exceptions, the sonographic appearance in nonspecific. They were grouped in four categories: teratomas, cystadenomas, adnexal torsions, polycystic diseases. Surgery with be discussed upon consideration of age, clinical signs and sonography, and diagnostic emergency will be emphasized in case of acute adnexal torsion.

[Neonatal ovarian cysts. The role of ultrasonic diagnosis].

Nine cases of adnexal cysts diagnosed by antenatal ultrasonography, have been followed after birth and operated upon. Simple ovarian cysts as well as old adnexal torsions appear as sonolucent masses. Acute adnexal torsion appears as mixed or solid masse. Sonographic appearance can help the post natal management of these masses.

Primary lung cancer staging: prospective comparative study of MR imaging with CT.

Forty-four patients with bronchogenic carcinoma were studied prospectively by both computed tomography (CT) and magnetic resonance (MR) imaging of the thorax during the week preceding thoracotomy. Transaxial MR imaging sequences included T1- and T2-weighted sequences. Coronal and sagittal T1-weighted sequences were added according to tumor location. CT and MR studies were reviewed separately, and the results were compared with surgical and pathologic findings on the basis of TNM classification. No statistically significant differences were found between the two imaging methods for the evaluation of tumor extent or node involvement. T2-weighted sequences did not yield further information on tumor extent or node involvement. Additional imaging planes (coronal or sagittal) appeared useful to study chest wall invasion. Analysis of concordances and discordances did not indicate whether one modality could be substituted for the other, or which diagnostic strategy might be more useful.

MR imaging of intravoxel incoherent motions: application to diffusion and perfusion in neurologic disorders.

Molecular diffusion and microcirculation in the capillary network result in a distribution of phases in a single voxel in the presence of magnetic field gradients. This distribution produces a spin-echo attenuation. The authors have developed a magnetic resonance (MR) method to image such intravoxel incoherent motions (IVIMs) by using appropriate gradient pulses. Images were generated at 0.5 T in a high-resolution, multisection mode. Diffusion coefficients measured on images of water and acetone phantoms were consistent with published values. Images obtained in the neurologic area from healthy subjects and patients were analyzed in terms of an apparent diffusion coefficient (ADC) incorporating the effect of all IVIMs. Differences were found between various normal and pathologic tissues. The ADC of in vivo water differed from the diffusion coefficient of pure water. Results were assessed in relation to water compartmentation in biologic tissues (restricted diffusion) and tissue perfusion. Nonuniform slow flow of cerebrospinal fluid appeared as a useful feature on IVIM images. Observation of these motions may significantly extend the diagnostic capabilities of MR imaging.

[Unusual echographic aspects of hepatic distomiasis].

The authors report, a pediatric case of hepatic fascioliasis with an unusual ultrasonographic aspect; that is a hypoechoic intrahepatic nodule with an extrahepatic prolongation through a thickened capsule. This ultrasonographic appearance, which has not been reported in the literature to our knowledge, seems to reflect the parasite migration.

The vitamin status of low birth weight infants and their mothers.

The levels of vitamins A, E, B1, B2, folate, and B12 were measured in maternal blood, cord blood, and in 5-day neonate capillary blood samples. Twenty-six infants and their mothers served as controls and were compared with 32 low birth weight (LBW) infants. LBW infants showed significantly lower levels of vitamins A, E, B2, and folate than the controls. The folate status of mothers of LBW infants was significantly lower (7 +/- 1.8 ng/ml) than that of control mothers (21 +/- 0.8 ng/ml).
