Psychiatric day hospitalization: multiple perspectives on outcome.

Patients admitted over a three year period to a psychiatric day hospital were followed in order to: 1) substantiate the findings of other studies supportive of the short to intermediate term efficacy of psychiatric day hospitalization; 2) examine outcome on a number of dimensions and from a variety of perspectives; 3) examine issues relating to the differential efficacy of psychiatric day hospitalization; 4) discuss implications of the above factors in regards to planning effective day hospital programs.

The patient with peripheral arterial occulsive disease.

Peripheral arterial occlusive disease predominantly involves the lower extremities. The greatest incidence occurs in males over 50 years of age. Manifestations include intermittent claudication, diminished or absent pulsations, pallor, and trophic changes. Patients are particularly vulnerable to infection, ulcer formation, and gangrene. The treatment program aims to promote circulation, and to prevent tissue damage and complications. Some patients have reconstructive surgery, which includes prosthetic grafts as well as bypass procedures.

Detecting vascular problems in patients with diabetes treated with an intra-aortic balloon pump.

Individuals with diabetes are at increased risk for both peripheral vascular disease and coronary artery disease. In patients with severe coronary artery disease, a cardiac assist device called an intra-aortic balloon pump (IABP) often is used to aid the failing heart and prevent further cardiac ischemia. Because this device is inserted via the femoral artery, patients are at risk of limb ischemia distal to the insertion site. Patients with diabetes are particularly prone to this complication. Detecting the early signs and symptoms of ischemia is crucial to preventing serious sequelae. Standard vascular examination techniques, in addition to being subjective and not easily reproducible, may be misleading in patients with diabetes. This article provides a review of the signs and symptoms of lower limb ischemia and noninvasive vascular tests that clinicians can use to evaluate lower extremity circulation. Also included are protocols for patient care during and after hospitalization, and two case studies of cardiac patients with diabetes who were treated with an IABP.

A retrospective investigation of the relationship between aerobic exercise and quality of life in women with breast cancer.

Aerobic exercise rarely has been considered in the rehabilitation of patients with cancer. This retrospective study was undertaken to determine if women with breast cancer who exercise experience a higher quality of life (QOL) than women with breast cancer who do not exercise. A group of 42 women who exercised was compared to a group of 29 women who did not exercise. Compared to the nonexercisers, the women who exercised had a significantly higher QOL (p = 0.03) as measured by the Quality of Life Index for Patients With Cancer (QOL Index). The groups did not differ significantly in age, education, employment, socioeconomic status, stage of disease, time since diagnosis, type of surgery, type of treatment, or presence of pre-existing health conditions precluding exercise. However, the women who exercised perceived significantly fewer barriers to exercise (p = 0.0001) as measured by the Perceived Barriers to Exercise Scale (PBE Scale) and were more likely to be married or living with a significant other (p = 0.02).
