Psychopharmacology and mental retardation: a 10 year review (1990-1999).

We conducted a 10 year review of the literature pertaining to psychopharmacology and mental retardation. Studies were included or excluded from the review based on meeting one or more of the methodological criteria normally considered fundamental for sound scientific research. The vast majority of studies conducted in the last 10 years in this area had major methodological flaws. While a large number of medications were prescribed for various psychological disorders and behavior problems, most drug administrations were not based in science, were not evaluated appropriately, and generally did not follow best practices for treatment of persons with mental retardation. Very few medications prescribed were behavior or psychiatric symptom specific; that is, most medications were given to suppress a myriad of aberrant behaviors thus chemically restraining the individual in question. Practices such as these present serious problems for service providers due to the deleterious side effects of many psychotropic medications and the federal government's intervention into the care-provision practices of developmental centers, community homes, and other living arrangements for persons with mental retardation. Implications of our review are discussed.

Does memory development belong on an endangered topic list?

Although memory has long been regarded as a central and well-defined topic within the field of cognitive development, developments in and related to the study of memory increasingly suggest that the study of memory needs to be situated in a number of broader conceptual and research contexts. Four of these contexts are identified here and the ways they accommodate memory phenomena are examined: (1) knowledge (what is remembered?); (2) comprehension (what does it mean?); (3) context/function (why remember?); and (4) strategy (how to remember?). Memories, it is suggested, are best examined as knowledge structures that are the product of efforts to understand and to know, and memorizing is a socially situated activity undertaken in the service of individual or social goals.

A specialized hospice for dementia patients and their families.

This article reports the development of a small pilot program and briefly describes its activities. The growing number of dementia patients in American communities indicates a need to extend hospice services to these patients and their families. It is difficult to determine at present whether this need could be met more effectively by encouraging existing hospices to accept these patients or by establishing specialized programs such as this one. Research in this area is indicated, as well as the sharing of information among hospices providing care to other specialized groups--people with AIDS or terminally ill children for example. The Alzheimer's Family Care Hospice has provided services to eight terminally ill dementia patients and their families in a pilot project sponsored jointly by the Rush Alzheimer's Disease Center and the Chicago Area Chapter of the Alzheimer's Association. Another six patients did not meet admission criteria.

Long-term visual outcome of choroidal neovascularization in pathologic myopia: natural history and laser treatment.

UNLABELLED: This retrospective study was designed to help clarify the visual prognosis during long-term follow-up (5-10 years) of myopic choroidal neovascularization (CNV) with and without laser treatment in a referral population. PATIENTS AND METHODS: A group of 50 consecutive non-treated eyes and a group of 50 consecutive treated eyes were selected retrospectively. Inclusion criteria were: fluorescein angiographic documentation of CNV not involving the center of the fovea, visual acuity (VA) > or = 20/200, age less than 60, onset of symptoms < or = 6 months and at least five years of follow-up. The mean decrease in VA in non-treated and treated eyes during the follow-up was analysed on the basis of subgroups with the same initial VA. RESULTS: Considering both groups (100 eyes) at presentation, 89% of CNV spared the center of the fovea but were located in the foveolar area (< 200 microns) and only 11% were extrafoveal (> or = 200 microns). In the natural history group, after five years all CNV involved the center of the fovea and mean VA was 20/160. In the treated group, at the end of the five year follow-up and after one or more laser session, 64% of eyes had a dry scar and mean VA was 20/74. Nevertheless there was a very high rate of recurrences (72%) and at the end of follow-up 36% of treated eyes had a subfoveal recurrence with a mean decrease in VA to 20/154. The difference between the mean decrease in VA of the treated and non-treated groups was statistically significant at two years. At five years (100 eyes), the difference persisted only for eyes with initial VA better than > or = 20/40 (P < 0.05). At eight years (76 eyes) and ten years (24 eyes), this difference was no longer significant. COMMENTS: This study confirms that CNV accompanying degenerative myopia has an extremely poor prognosis. Nevertheless analysis of final VA in both groups suggests that eyes with high initial VA gain more from treatment.

[Prescription of herbal teas for children under six months of age: the opinion of physicians from a middle size city in Southern Brazil].

A cross sectional study survey was carried out among physicians who work at children's clinics in Pelotas. The doctors were interviewed by medical students about the prescription of tea for children under six months, risk factors and breastfeeding. About half of the interviewed physicians prescribed tea. The main reasons for that were: intestinal colic (35%), diet supplementation (21%), and oral rehydration (18%). The relationship between tea and diarrhoea, tea and malnutrition, tea and weanning was acknowledged by 19%, 47% and 83% of them, respectively. These association remained significant (p<0.05) even after adjustments were allowed for graduation time and pos-graduation courses. The conclusion is that some common aspects of children's diet are not well managed by most of these doctors.

[Unilateral hypertrophy of the coronoid process associated with facial asymmetry. Case report].

A rare case of unilateral coronoid process hypertrophy associated with limitation of mouth opening, deviation of the mandible to the affected side and facial asymmetry is reported. The importance of a thorough preoperative clinical and radiological examination is stressed. The resection of the coronoid process via an intraoral approach and the subsequent physiotherapy led to a good functional and aesthetic result.
