Experimental studies in rats and bulls demonstrated the feasibility of using a silicone-Dacron prosthesis as a sperm reservoir in cases of excretory azoospermia. In the study of bulls, 50% of the cows inseminated with spermatozoa aspirated from the alloplastic spermatocele conceived and delivered healthy calves. The clinical results of a series of 14 humans suffering from excretory azoospermia not treatable by other means (e.g., epididymovasostomy or vasovasostomy) also are presented. THe aspirations from these spermatoceles produced motile spermatozoa; however, none of the patients'p spouses conceived after insemination.
PURPOSE: To compare ioxaglate and iobitridol for percutaneous transluminal renal angioplasty (PTRA) as regards thromboembolic complications, the quality of diagnosis, and renal and general safety. METHODS: One hundred and eighty-nine patients were prospectively studied, 98 of whom received ioxaglate, and 91, iobitridol. Twenty-two were secondarily excluded from the evaluation of thromboembolic complications as they did not undergo PTRA. RESULTS: Two hundred and two PTRAs were performed. The total volumes of contrast medium administered and the procedure durations were the same for each patient. In the ioxaglate group, four dissections (3 stents), one occlusive dissection, and two spasms occurred; in the iobitridol group, there were three dissections (all stented), one occlusive dissection (stented), and two spasms. The final angiograms showed four renal infarctions with ioxaglate (2 of which were in patients who were not anticoagulated), two with iobitridol. No significant difference was seen in the incidence of thromboembolic complications when the PTRA was performed after anticoagulation (n = 150; 3.9% vs 4%, p = 0.78); in the whole population, thromboembolic complications were more frequent in the ioxaglate group but the difference was not significant (5.7% vs 3.7%, p = 0.74). The quality of the diagnosis and the general and renal safety were the same in the two groups. CONCLUSION: Regarding the clotting phenomenon, we recorded as many thromboembolic complications with ioxaglate as with iobitridol.

UNLABELLED: No reliable data are available on cases of lethal child abuse (by active force) in the area of the former German Democratic Republic. In a multicenter study we therefore examined the police and court records for such cases occurring in the period 1 January 1985 to 2 October 1990 in the entire area of the former German Democratic Republic. RESULTS: The study center received information on 39 cases of lethal child abuse which correspond to approximately 7 cases per year. However, a low percentage of undetected crimes which cannot be determined more precisely has to be taken into consideration. Almost 40% of the victims were younger than 1 year, 73% of the victims showed indications of repeated ill-treatment. The effects caused by using direct blunt forces, against the head in particular, were by far the most frequent causes of death. The male contact person (the victim's father, brother or stepfather as well as the life companion of the child's mother in particular) killed the child in most of the cases. As far as it is known, 37% of the male/female offenders suffered from chronic alcoholism; 32% of the male/female offenders were under the influence of alcohol when the crime happened. 83% of the male/female offenders who were found guilty made a confession shortly after the crime had happened or during the interrogations. Almost all the male/female offenders were sentenced to prison (the duration of the imprisonment varied between one year and for life). Due to the considerably lower section rate compared to the one in the German Democratic Republic, it is to fear that each second fatal child abuse is not detected in the new federal states.
UNLABELLED: No reliable data are available on cases of lethal child neglect in the area of the former German Democratic Republic. In a multicenter study we therefore examined the police and court records for such cases occurring in the period 1 January 1985 to 2 October 1990 in the entire area of the former German Democratic Republic. RESULTS: A total of 9 cases were reported to the study center. This does not include undetected cases, whose number, though indeterminate, is probably small due to the general obligation to perform autopsies on all children dying before their 16 birthday. Just over half of all victims were less than one year old, the oldest was 3 1/2 years old. Starvation and severe dehydration were by far the most common causes of death; in half of cases these occurred in combination with hypothermia. Most often mother killed their children by neglect, either alone or together with the victim's father. In the majority of cases no close bond existed between the parents and the child. Seventy percent of the perpetrators were chronic alcoholics. All 10 of the perpetrators were sentenced to imprisonment for periods ranging from one year to life long. Mitigating circumstances were presented at the sentencing phase of the trial on behalf of 20% of those convicted. The experience of legal medical and pathological institutes in the former German Democratic Republic underscores the need to perform an autopsy on all deceased infants and young children. Only this can ensure that no cases of lethal child neglect are overlooked.

Regulations considering the inner-community-exchange of frozen bovine semen prescribe the use of lincomycin and spectinomycin (LINCOSPECTIN) and streptomycin-penicillin as antibiotics in the diluted semen. A susceptibility test with Tris-egg yolk-freezing medium, mostly used in Germany, demonstrated that lincospectin in a concentration of 450 micrograms/ml diluted semen had no detrimental spermatologic effect on sperm function.
Chordoma of base of skull is a rare tumor in children. The case reported was atypical by its slow course, the initial clinical signs resulting from the development of the tumoral process in the posterior fossa at the level of the petrous and pontocerebellar angle. Neuro-ophthalmologic manifestations appeared at a late stage only.

Secondary prevention is of importance when the patient is already suffering from a serious disease, e.g., from arterial obstruction causing a stroke or an amputation, from a hip fracture or other diseases that might threaten his independence. Secondary prevention covers a wide field of topics. First of all, the patient must recover from his acute disease. It is important to avoid complications which are not specific for the disease, but are typical for a bedridden old person (decubital ulcer, dehydration and others). Prevention also means to avoid recurrence of the same disease as well as complications that frequently occur during the clinical course and may influence the outcome (spasticity in stroke patients, muscular calcification following hip replacement). Frequently, old persons do not completely recover following serious disease, they are limited in their daily activities and their capability to leave home. Secondary prevention tries to fight isolation; the patient should live a meaningful life.
Aids for mobility and for activities of daily living (ADL) are frequently necessary in geriatric patients, but they are costly. We were interested to find out whether prescribed aids were really used by elderly patients. 177 patients were visited at home 6-8 weeks following discharge from a geriatric hospital and the use of their aids was evaluated. Mobility aids had been delivered with few exceptions; they actually were in frequent use. Aids for ADL were not delivered in 8% of cases, and 10% of those delivered had never been used. Hand-rails had been delivered but not been fixed at times. The reasons for these unexpected findings are discussed.
According to the German law salute-fire guns altered weapons with a barrel length of more than 60 cm. They have inside the barrel special constructions, which are guiding the gunshot residues under high pressure to the muzzle. Therefore they own an high potential of injury, like blank-cartridge guns with short barrels.
