The topography and neuroanatomy of fibers connecting the deutocerebrum to the protocerebrum in the brain of the American cockroach Periplaneta americana were investigated by staining single or multiple deutocerebral neurons with cobalt, Lucifer Yellow, or biocytin. Five tracts are distinguished on the basis of their routes from origins in the antennal lobe to the protocerebral neuropil: the inner antenno-cerebral tract (IACT); antenno-cerebral tracts II, III, and IV (ACT II, III, IV), and the outer antenno-cerebral tract (OACT). These tracts are largely composed of the axons of four classes of deutocerebral projection neurons, which have been identified morphologically; the neuronal arborizations in the glomeruli of the antennal lobe and in the protocerebral projection regions have been examined. Projection neurons with processes in the inner antenno-cerebral tract and in the antenno-cerebral tract II each innervate a single glomerulus in the antennal lobe, and both types have terminals in the calyces of the mushroom bodies and in the lateral lobe of the protocerebrum. The axons of pheromone-sensitive projection neurons with dendritic trees in the male-specific macroglomerulus seem to run exclusively in the inner antenno-cerebral tract. Subgroups of these pheromone sensitive neurons differ in relative sensitivity to the two female attractant components as well as in the arborization pattern of their dendrites in the macroglomerulus. The projection neurons of two other classes each innervate many glomeruli in the antennal lobe, those of one class sending their axons into the protocerebrum in the antenno-cerebral tract IV and the other, in the outer antenno-cerebral tract. The neurons of antenno-cerebral tract IV innervate not only the mushroom body calyces and the lateral lobe but also neuropil regions not previously described in the cockroach. Neurons with axons in the outer antenno-cerebral tract have no terminals in the calyces but innervate the lateral lobe and the neuropil surrounding the tract. The morphological findings presented here show that, in addition to the tracts previously documented in the cockroach brain, there are other, presumably olfactory, connections between the deutocerebrum and the protocerebrum.

ACCORD is a general methodology of knowledge-based systems, and MACCORD its application to medicine. MACCORD represents the problem solving behavior of the medical expert in terms of various types of medical reasoning and at various levels of abstraction. With MACCORD the epistemic and cognitive processes in clinical medicine can be described in formal terminology, covering the entire diversity of medical reasoning. MACCORD is close enough to formalization to make a significant contribution to the fields of medical knowledge acquisition, medical didactics and the analysis and application of medical problem solving methods.

On the experience of 153 cases of rectal bladder after cystectomy for cancer, the indications, technique and results of this operations are reported. Earlier, it was a method of choice as a bladder substitute. Today the indication is limited to female cysto-urethrectomy. Technique consists in rectum isolation and ureterorectal antireflux anastomosis; the sigmoid colon is pulled through behind rectum and between outer and inner layer of anal sphincter fixed on the perineum. The overall postoperative mortality is 7.8%, but in the last 5 years there was no mortality. The most frequent complications are sigmoid retraction and its partial necrosis. Micturition mostly is under control, but 14% of patients have a relative defecation incontinence.

Necrosis of the colon is a rare but often fatal complication of necrotizing pancreatitis. Among 159 patients with necrotizing pancreatitis who were treated between 1 January 1986 and 31 December 1988, a colonic lesion developed in 10 patients (6.3%). All 10 patients underwent surgery. With regard to the time of occurrence and the extent of the lesion two types could be distinguished: first, extensive, probably ischemic colonic necrosis developed early in the course of the disease, i.e., within the 1st week; second, localized lesions mainly in the region of the left colonic flexure occurred much later and were considered to be a complication related to persistent infection in the remaining pancreatic necroses. The colonic lesions were treated by resection and fecal diversion in 9 patients and by local excision with direct closure and additional diverting colostomy in one patient. Even though the colonic complications could be controlled, only 1 of the 10 patients survived. This can be explained by the fact that all these patients had a severe form of the disease with extensive pancreatic necroses and were in a septic-toxic state with multiple organ failure at the time the colonic lesion occurred.

Based on a prospective trial on 91 patients diagnostic and therapeutic evaluation of ERCP in the early phase of acute pancreatitis was carried out. Detection of biliary stone disease and visualization of tumours of the papilla or main bile duct leads to therapeutic consequences (Papillotomy, exstirpation of the tumour). In addition to that, ERCP yields radiomorphologic informations, which seem to be typical for necrotizing pancreatitis. Showing a correlation to severity and prognosis of necrotizing pancreatitis these informations can be used for planning surgical tactics in the course of the disease.

A total of 155 high yielding dairy cows were used to study the effect of dietary beta-carotene (carotene) on fertility. The diet of group 1 was supplemented with 69 mg retinyl acetate per cow daily before calving and 96 mg retinyl acetate after calving: diet of group 2 was supplemented with 500 mg carotene per cow before calving and 96 mg retinyl acetate after calving; and that of group 3 with 500 mg carotene before and 700 mg after calving. Conception rates were 53.9, 42.2, and 27.9% for cows of groups 1, 2, and 3. Days open were 110 and 112 for young (second and third lactation) and old (fourth and later lactations) cows of the group 1; 130 and 129 for young and old cows of group 2; and 133 and 166 for young and old cows of group 3. Number of inseminations per conception was 1.8 and 1.8, 2.2 and 2.2, and 2.5 and 3.4 for the same groups, respectively. Within group 3 the percent of cows pregnant 120 d after calving was 57 and 20 for the young and old cows. Correlation coefficients between plasma carotene of 53 old cows of all groups 4 and 8 wk after calving, and the number of open days and the number of inseminations per conception were significant. The intake of high carotene may have adverse effects on the fertility of dairy cows.

The purpose of this report is to present recent data on two modulating factors of carcinogenesis that are found in Western-type diets: a beef-derived mutagenesis modulator that has been shown to inhibit the initiation of epidermal carcinogenesis in mice, and the possible role of net energy rather than dietary fat per se in the enhancement of rat mammary carcinogenesis.
Five cases of the dog-bite syndrome--palsy of the superior oblique muscle and a paradoxic inability to elevate the eye in adduction--are described. It is suggested that no treatment be given in the acute phase unless there are symptoms in the primary position, in which case peritrochlear injection of a depot steroid within 2 weeks of the injury may be of benefit. If after several months the ocular motility is still significantly restricted an attempt can be made to remove local scar tissue, with insertion of anchor sutures; recession of the inferior rectus of the fellow eye with adjustable sutures should correct residual symptomatic hypertropia.

CT staging of bladder neoplasms is a highly accurate procedure with a proven accuracy rate of 81% and a potential accuracy rate probably above 90%. At this institution, CT staging has become a routine procedure for all patients with bladder neoplasms prior to surgery or radiotherapy as well as to follow the progress of therapy. Staging of bladder carcinoma is performed by cystoscopy with biopsy, intravenous urography, and CT of the gas-filled bladder. Of 21 patients with surgical confirmation, 17 were correctly staged solely from the CT image. Overstaging in the four errors resulted from confusion produced by normal contiguous extravesical structures and by tangential viewing of the normal bladder wall. Negative contrast allowed precise demonstration of the morphology of the intravesical portion of the tumor.
