[The contribution of sliding skin flaps in the follow-up treatment of facial burns].

Treatment of panfacial burns is a difficult challenge for the plastic surgeon. Although excision-grafting procedures can be used to provide sufficient skin of sufficient quality, inevitable skin retraction will, in the long run, produce the typical "burned face" aspect (retraction of the periorificial zones, smoothing out of the main skin folds). The grafted face then looks like a flat, rigid and unexpressive mask unable to perform its social function as an organ of expression and mimic. In order to reestablish the main facial features, we used sliding skin flaps which were either grafted immediately or during a second operation. We report here our experience and give details on the technical points and indications as well as the advantages and disadvantages of this method.

The surgical management of cranio-orbital neurofibromatosis.

Experience with the surgical management of cranio-orbital neurofibromatosis in 14 patients is reported (age range, 6-40 years). The skeletal abnormality of the orbit that occurs in a small proportion (less than 1%) of patients with neurofibromatosis is, in essence, the absence of the membranous portions of the sphenoid and the adjacent bone forming the boundaries between the cranium and the orbit. The goal of surgery is tumor resection and reconstruction of the posterior bony defect by bone graft. Two additional procedures are also described that better enhance aesthetically-the mask lift and facial tissue expansion.

Mask lift and facial aesthetic sculpturing.

Paul Tessier started the great adventure of sculpturing the malformed face more than 30 years ago. Key procedures such as subperiosteal dissection and lifting of the facial mask, bone grafting from a cranial site of harvest, and lateral canthopexies have been utilized and have withstood the trial of time in thousands of patients. The mask lift with facial aesthetic sculpturing, a newcomer in aesthetic surgery, is an extrapolation of these craniofacial procedures. It is a different and revolutionary approach to the aging face that seeks to normalize, rejuvenate, and embellish the face through a subperiosteal lift of the facial mask and transformation of the underlying structures. Facial aesthetic sculpturing refers to the aesthetic sculpturing of the facial skeleton. It allows us to equilibrate the components of a malformed face and normalize it by osteotomies, displacements in three dimensions, and bone overgrafting; to normalize the forms of an unattractive face and to embellish it by harmonization of the skeletal volume; and to restore the forms of an aging face and to rejuvenate it by augmenting resorbed bone, reducing hypertrophic bone, and lifting the facial mask. My experience includes 350 patients operated on from 1981 to 1992, of these, 200 cases were purely aesthetic, 150 functional. The complications encountered have been few. The technique described offers an appropriate response to the different problems of aging and embellishment of a face, whether for reequilibration of osseous volumes or for sagging of soft tissues of the superior two-thirds of the face. Facial aesthetic sculpturing finds application also in some malformations and acquired deformities and in young faces with unsightly features.

[Secondary surgical restoration of contours and volumes of facial burns].

Recent progress in plastic surgery techniques have clearly contributed to the improvement of the aesthetic prognosis of facial burns. However, although current excision-graft techniques achieve good quality skin, often in sufficient quantity, these grafts give the face a fixed, inexpressive, flat mask appearance, du to the inevitable phenomena of scar retraction. To palliate this unsightly appearance of the grafted face, the authors restore the principal facial relief (eyebrows, cheekbones, nostrils, philtrum, lips, chin) by using, depending on the particular case, subperiosteal parietal bone grafts, or flaps of scar tissue grafted immediately or secondarily. The authors report their experience and describe the techniques, indications, as well as the advantages and disadvantages of these various procedures.

"Mask lifting" in orthomorphic facial correction of bilateral cleft palate.

In bilateral cleft palate patients, abnormal obliquity and narrowness of orbits are noted on radiographs, computed tomographic scans, and magnetic resonance imaging investigations. Deformity of bony structures (increase in transverse length) is sometimes associated with malposition of eyelids, which results in a typical antimongoloid facies. Five patients were treated, and we report our series to illustrate the problem and its correction using the "mask-lift" procedure, initially described by Tessier and Krastinova.

[Surgical treatment of orbito-palpebral hemangioma].

The authors propose an original surgical procedure for the treatment of the orbito-palpebral hemangiomas. Preoperative evaluation include a CT-scan of the orbito-palpebral regions as well as a selective arteriogram. Embolizing was performed after supra-selective cannulation of the feeding vessel. Access to the orbital contents is obtained by an intracranial, superior eyelid crease and lymbic exposure. Complete bone reconstruction and functional reconstruction of the eyelid lead to a highly aesthetic and functional result.

[Orthomorphic facial treatment of patients with sequelae of bilateral lip-palate cleft by the "mask-lifting" procedure].

The clinical study of the major cranio-facial malformations such as Apert syndrome, Treacher-Collins syndrome, Blepharophimosis and Bilateral Cleft Palate patients, lead us to note a similarity of the orbito-palpebral region. A characteristic obliquity and narrowness of the orbits associated with an antimongoloid eyelids disposal was noted after X-rays, CT-scan and clinical evaluation. Authors suggest a facial orthomorphic restoration of bilateral cleft palate patients by the "mask-lifting" procedure associated to the conventional maxillo-facial surgery.

An intracranial/extracranial approach for the surgical management of haemangiomas of the orbit and eyelid.

The authors propose a combined approach for the treatment of orbito-palpebral haemangiomas. Two cases are presented and discussed. Pre-operative evaluation included coronal, axial, and sagittal CT scans as well as a selective arteriogram. These studies assisted the planning of the surgical approach. An intracranial/extracranial surgical approach was used with ocular-palpebral dissection. Pre-operative arterial embolization was helpful in controlling perioperative blood loss. Precise excision of the masses was thus accomplished and functional and aesthetic reconstruction aided by the conservation of normal anatomical structures.

[Subperiosteal face-lift].

The "mask lift" is a new surgical technique based on a different and revolutionary approach to the "aging face". The author presents her experience with 130 cases since 1981.
