Diffuse neurofibroma of scalp.

Diffuse neurofibroma is an uncommon but distinct variety of neurofibroma, usually affecting trunk, head and neck regions of adolescents and young adults. The clinical features, gross macroscopic and histopathological findings are enunciated and the criteria for instituting the preferred modality of treatment for such lesions has been reviewed, stressing upon the need to exclude the neurofibromatoses preoperatively.

Nitric oxide-related inhibition of carotid chemosensory nerve activity in the cat.

The hypothesis that endogenous nitric oxide may play a physiological role in the regulation of carotid chemosensory activity was tested in this study. The nitric oxide synthase (NOS) inhibitors, L-nitro-arginine-methyl ester (L-NAME, 25-200 microM) and NG-monomethyl-L-arginine acetate (L-NMMA, 50 and 100 microM) were used to study its effects on the chemosensory activity of perfused and superfused cat carotid bodies (n = 21) in vitro at 37-37 degrees C. L-NAME elicited slow excitation of the sensory activity as did L-NMMA. The peak-response was dose-dependent, and approached saturation around 200 microM. The excitation by L-NAME showed the following characteristics (mean +/- SEM): latency of response, 2.2 min +/- 0.3 min; time to peak response, 5.5 min +/- 1.0 min and the peak response increased to 407 +/- 42 imp/sec from 88 +/- 13 imp/sec. The peak response was significantly different (P < 0.05) from the baseline activity. L-arginine (50-500 microM) only briefly reversed the stimulation. Hypoxia enhanced the excitation by L-NAME. On the other hand, sodium nitroprusside (SNP, 0.5-10 microM) which supplies NO, terminated the excitatory effect of L-NAME. The results provide evidence in favor of an inhibitory role of endogenous NO in the carotid body, and exogenous application of NO confirms the inhibitory effect.

The Klippel-Trenaunay syndrome: a rare cause of hematuria requiring nephrectomy.

The Klippel-Trenaunay syndrome is a rare disorder in which the congenital vascular anomalies can affect the urogenital tract. Several cases of hemorrhage from the urogenital system have been reported in children with this condition. We report an upper renal tract hemorrhage in an adult, which required nephrectomy. To our knowledge this condition has not been reported previously in an adult with the Klippel-Trenaunay syndrome. We discuss the condition, and its diagnosis, treatment and relevance to the urogenital tract.

CO reveals dual mechanisms of O2 chemoreception in the cat carotid body.

The hypothesis that CO-binding pigments in the carotid body participate in O2 chemoreception was tested. The chemosensory nerve discharges of cat carotid body perfused and superfused in vitro at 36-37 degrees C with cell-free solution containing CO2-HCO3- (pH approximately equal to 7.39) were recorded to monitor O2 chemoreception. Several levels of PCO (60-550 Torr) at two levels of PO2 (50 Torr-140 Torr) were used. With high PCO of 500-550 Torr at any PO2 the discharge rate peaked promptly but the effect was significantly less than that to hypoxia. At any stage of the CO effect, exposure to light promptly attenuated or eliminated the response, as if the stimulatory effect of hypoxia was absent. Lower PCO of 60-70 Torr attenuated the response to hypoxia which was not suppressed by light. PCO of 140 Torr also attenuated the response to hypoxia and made the activity partially photolabile. During high PCO exposure the excitatory response to cyanide but not to nicotine was attenuated, consistent with the idea that the effects of nicotine are downstream from those of CO. Both inhibitory and excitatory effects of CO were promptly reversible. The results indicate that two types of CO-binding chromophores participate in O2 chemoreception in the carotid body.

A metronidazole-resistant strain of Trichomonas vaginalis and its sensitivity to Go 10213.

In the present study the comparative in-vitro and in-vivo efficacy of Go 10213 was compared with those of metronidazole, secnidazole, tinidazole, ornidazole and nimorazole against a metronidazole-resistant strain of T. vaginalis. Go 10213 was found to be superior in activity. It appears more promising than the drugs mentioned above on the basis of the evidence presented.

Comparative studies on the amoebicidal activity of known 5-nitroimidazole derivatives and CG 10213-Go in golden hamsters, Mesocricetus auratus, infected in the liver or caecum or both with trophozoites of Entamoeba histolytica.

CG 10213-Go, a 5-nitroimidazole derivative synthesized at the CIBA-Geigy Research Centre, Bombay, proved superior to such currently used 5-nitroimidazole derivatives as metronidazole, ornidazole, secnidazole, tinidazole and nimorazole, against experimental amoebic infections of the liver and caecum in a golden hamster model. CG 10213-Go is now undergoing clinical trials.

Phase 1 tolerability and antiamoebic activity studies with 1-methylsulphonyl-3-(1-methyl-5-nitro-2-imidazolyl)-2-imidazolidinone (Go.10213): a new antiprotozoal agent.

Go.10213, a new nitroimidazole, was studied in 12 male volunteers for tolerability and in 20 patients with intestinal amoebiasis for antiamoebic activity. Go.10213 was well-tolerated by volunteers up to a dose of 400 mg X 3. Patients also tolerated well the dose of 100-150 mg X 3 for 7 days. In two patients, there were mild and transient side-effects like headache, dizziness, fatigue, etc. No neurological side-effects were observed. There were no significant changes in blood pressure, pulse rate and ECG. The organ function tests did not show any adverse effects of Go.10213 on bone-marrow, kidney, liver, etc. Go.10213, at a dose of 150 mg X 3, showed potent antiamoebic activity in 10 patients with intestinal amoebiasis, as judged by the clinical relief, the eradication of the trophozoites and cysts of Entamoeba histolytica from stools and the healing of colonic ulcers. Go.10213, a novel nitroimidazole, may prove to be the most potent and safe agent against the protozoal infections, e.g. amoebiasis, giardiasis and trichomoniasis.

The infectivity of ingested adult hookworms.

Fourth-stage larvae and adults of Ancylostoma ceylanicum (dog strain) produced patent infections, with fertile ova, in hamsters to which they had been given in a drench. Similarly, 36-day-old (immature adult) Necator americanus given in a drench to hamsters also produced patent infections with fertile ova. The epidemiological significance of these experiments is discussed.

Efficacy of seven anthelmintics against Ancylostoma ceylanicum in the golden hamster, Mesocricetus auratus.

In an experimental patent infection of Ancylostoma ceylanicum in golden hamsters the efficacy of seven anthelmintics was tested; bephenium hydroxynaphthoate, tetramisole, tetrachlorethylene, phenylene 1,4-diisothiocyanate, thiabendazole, parbendazole and mebendazole were studied. Mebendazole was the most effective, with parbendazole, phenylene 1,4-diisothiocyanate, thiabendazole and bephenium hydroxynaphthoate also satisfactory. Tetrachlorethylene and tetramisole were the least effective in our screen.
