The shuttle walk exercise test in idiopathic pulmonary fibrosis.

The shuttle walk test (SWT) is a validated, incremental walking test for chronic obstructive pulmonary disease, but not for idiopathic pulmonary fibrosis (IPF). The measurement of maximal oxygen consumption (VO2 max) is considered to be the gold standard measurement of functional capacity. This study examines the relationship between IPF patients' performance on the SWT and VO2 max. Twenty patients were recruited for the study, which consisted of two separate experiments. Firstly, the relationship between SWT performance on a conventional corridor SWT, with that on a programmable treadmill SWT designed to reproduce the corridor SWT was examined (n=10). In the second experiment, the relationship between performance on the treadmill equivalent SWT and VO2 max measurements was studied (n=10). There was a significant correlation between distance walked on the corridor SWT, and that walked on the treadmill equivalent SWT without VO2 max measurements (367 m vs. 410 m) (r=0.91, P=0.0003). There was a significant correlation between distance walked on the treadmill equivalent SWT (277 m), and the directly determined VO2 max (14.87 ml/kg/min) (r=0.74, P=0.01). During both experiments, a significant correlation was also observed between baseline PaO2 and SWT performance, and between DLCO and SWT performance. The shuttle walk test is a simple objective measure of functional capacity in IPF patients, which should facilitate the evaluation of new therapeutic compounds for IPF.

Granulomatous vasculitis--a clinical presentation.

A case of granulomatous vasculitis affecting vessels in the left temporal area of the skull, and also vessels in the right and left axillary areas, is described. Initially, the patient responded favourably to antibiotic therapy, raising the possibility of a microbiologic agent as the causative factor of this puzzling disorder. Finally she has responded very favourably to steroids, thereby indicating a possible immunologic basis for the condition, as has already been postulated by several authors.

Esthesioneuroblastoma: report of two cases in Nigerians.

Two cases of esthesioneuroblastoma occurring in African Nigerians are reported, with a review of the literature. Though the tumour has highly suggestive clinical features, histologic diagnosis may be difficult. The name 'esthesioneuroblastoma' is considered to be preferable to 'olfactory neuroblastoma' in order to avoid consideration of such a tumour as a secondary from the adrenal gland. Early diagnosis and treatment by surgery gives the best chance of cure.

Neurilemmoma of the vagus nerve: a case report.

A CASE of an unusually big neurilemmoma arising from the vagus nerve in the neck is presented in this paper. The tumour was so big that it could not be removed until its size had been reduced by the removal of about 15 cc of its fluid contents. The patient had a cardiac arrest on removal of the tumour, denoting probable stimulation of the vagus nerve leading to vagal arrest. His heart started, however, after resuscitative measures and he has not shown any residual cardiac abnormalities, thus indicating that the main vagal trunk was undamaged.

Chloroquine ototoxicity--a reversible phenomenon?

The case of a six-year-old girl suffering from severe cochleo-vestibular dysfunction following a series of chloroquine phosphate injections is presented in this paper. Prompt institution of therapy with steroids and vaso-dilators has helped to restore the hearing to a serviceable (socially acceptable) level. A surmise is made about the pathogenesis of chloroquine ototoxicity and how prompt appropriate therapy may be able to reverse the process. A plea is made for early referral of these cases so that prompt attention may be paid to them.

Arterio-venous fistula of the ear.

A case of congenital arterio-venous fistula involving the external ear in a 33-year-old female patient is described in this paper. She presented to the hospital with severe haemorrhage from the swelling over the ear necessitating immediate admission into hospital and transfusion of whole blood. The treatment of the case is described in detail. She is alive and well 3 years after her definitive treatment.

Solitary tuberculoma of the larynx: a case report.

A case of solitary tuberculoma of the larynx in a 56-year-old man from Calabar, Nigeria, is presented. He had upper respiratory tract obstruction severe enough to necessitate a tracheostomy. It is suggested that tuberculoma of the larynx may not be as rare as a review of the currently available medical literature would suggest. More may be discovered if all cases of polypoid masses in the larynx, however innocent they look, are histologically examined.

Penetrating injury of the chest and coronary arteriovenous fistula.

An unusual case of coronary artery fistula, due to a stab wound in the chest in a 17-year-old male patient, with a resultant acute inferior myocardial infarction is presented. At surgery, complete transection of the right coronary artery was found, which formed a fistulous communication with both the right atrium and the right ventricle. The development of a continuous murmur in the period immediately after the injury, the absence of hemopericardium, and multiple cardiac-chamber involvements were unique features of the case. The inferior myocardial wall was akinetic due to infarction and did not improve following revascularization surgery. A brief review of the literature is also presented.

Immunity and allergy--a report of skin-testing (for allergic diatheses) in Nigeria.

The importance of Allergy in the causation of several important diseases e.g. Asthma, is well known (Voohorst 1962; Voorhorst, Spieska-Beezeman and Spieska 1964-). In this article, the value of finding out the causative allergen by a detailed case-history and later by a skin-testing against the most probable allergens (as suggested by the case-history)--is stressed. The major allergens responsible for the Allergic immunopathies in Nigeria--as found by testing in 50 cases are mentioned. It is hoped that more hospitals will be stimulated to perform skin-testing in view of the encouraging results and responses to de-sensitization so far obtained in different centres practising this procedure.
