Plasmacytoma of the head and neck.

Fourteen cases of plasmacytoma arising in the head and neck are described. Good local tumour control was achieved with minimal surgery and moderate doses of radiation. Subsequent myelomatosis did occur but this was unusual and no more frequent with tumours arising in the maxilla and mandible than at extramedullary sites. It is recommended that all apparently solitary plasma cell tumours of the head and neck are treated by radiotherapy with curative intent and then followed up carefully for life.

Wandering fixed drug eruption: a mucocutaneous reaction to acetaminophen.

Two patients are presented with a fixed drug eruption to acetaminophen characterized by recurring erythematous, circular plaques of the skin and oral mucosa along with multiple sites of deep (postinflammatory) hyperpigmentation clinically characteristic of fixed drug eruption. The involved sites did not necessarily flare with each exposure, nor did activity always appear in the same sites with each flare. Eventually some sites apparently became completely refractory. This caused the eruption to seem to "wander." Patch testing with 1% acetaminophen in alcohol (300 mg/30 ml alcohol) confirmed the causative agent in one patient. Although patch testing was negative in the other patient, oral reexposure caused a flare. Acetaminophen appears to be an uncommon cause of fixed drug eruption, with a tendency for lesions to appear at the same or at different sites in flares, perhaps because of a prolonged refractory period and the tendency to become completely refractory in some locations.

Effect of dopamine-depleting brain lesions in rat pups: role of striatal serotonergic neurons in behavior.

Previous results from our laboratory have demonstrated that 3-day-old rats given dopamine (DA)-depleting brain lesions are spared the severe behavioral dysfunctions seen after comparable brain damage in adults. This behavioral sparing is accompanied by a sprouting of serotonin (5-HT)-containing neurons in the striatum. The present results extend these observations by demonstrating that rats given the brain lesions as 15- or 27-day-olds continue to suckle, wean, and grow into adulthood without exhibiting any obvious behavioral dysfunctions, yet striatal 5-HT levels do not increase. Moreover, combined destruction of DA- and 5-HT-containing neurons in 3-day-old rat pups also produced no obvious behavioral dysfunctions. These and other results indicate that increases in striatal 5-HT are not necessary for the behavioral sparing observed after DA-depleting brain lesions in neonatal rats.

Antibody to human immunodeficiency virus in factor-deficient plasma. Effect of heat treatment on lyophilized plasma.

The prevalence of antibody to human immunodeficiency virus (HIV) was determined in various commercial substrate plasmas used in clotting factor assays, and viral isolation was attempted from both seropositive and seronegative samples. Antibody to HIV was detected in 13 of 13 plasma substrates used for Factor VIII assays and in 2 of 3 plasma substrates used for Factor IX assays. Antibodies were not detected in any of the other factor-specific substrates. Virus could not be isolated from any of the seropositive samples after 28 days in culture. Heat treatment of the samples under conditions known to inactivate HIV in plasma products indicated that heating the lyophilized substrate plasmas at 60 degrees C for up to eight hours had little effect on factor substrates and factor assays. Progressive loss of Factor V in the deficient plasmas was the most serious effect produced by heat treatment.

Halogenated pyrimidines as radiosensitizers in the treatment of glioblastoma multiforme.

Sixty patients with high-grade gliomas (including 50 patients with glioblastoma multiforme) were entered on four sequential Phase I trials combining continuous intravenous infusions of halogenated pyrimidines and high-dose brain irradiation. Patients received two 14-day infusions of bromodeoxyuridine (BUdR) or iododeoxyuridine (IUdR) during the initial wide field and later reduced field radiation treatment (total radiation dose 65-70 Gy). All patients were followed a minimum of 6 months or until death. The actuarial median survival was 13 months for the entire group, with an 18-month survival of 24%. No significant survival differences were observed based on BUdR versus IUdR, 12-h versus 24-h infusion schedule, degree of surgical resection, or sex. Good performance status and age under 50 years were significant favorable prognostic factors. Of interest, the 48 patients who completed planned treatment had a 14-month median survival, with a 30% 18-month survival. These survival observations are at least comparable to other combined modality trials in patients with glioblastoma multiforme. Ongoing and planned clinical trials using the halogenated pyrimidine analogs as radiosensitizers in patients with glioblastoma multiforme are discussed.

Clinical characteristics and resource utilization of ICU patients: implications for organization of intensive care.

We reviewed the clinical characteristics and resource utilization of 391 medical (M) and 315 surgical (S) ICU patients. In general, MICU patients had more physiologic derangement, as determined by the admission, maximal, and average acute physiology scores (APS). SICU patients had more frequent therapeutic interventions as measured by admission, maximal, and average therapeutic intervention scoring system values. Notably, 40% of MICU and 30% of SICU patients never received any active interventions and were admitted strictly for monitoring purposes. Patients on admission with APS less than or equal to 10 had markedly shorter ICU stays, with almost 50% less treatment than patients with APS over 10. Fifty-six percent of patients with APS less than or equal to 10 did not require any active intervention. In contrast, 83% of patients with APS greater than 10 had considerable intensive interventions. These patients required mechanical ventilation, invasive monitoring, and vasoactive drugs more than twice as often as patients with lower APS scores. Consideration should be given, therefore, to the organization of ICUs according to the patient's severity of illness.

Summary of early clinical experience with anisoylated plasminogen streptokinase activator complex in the treatment of acute myocardial infarction.

Preliminary investigations using a single intracoronary dose of APSAC (up to 30U) revealed dissolution of intracoronary thrombi in 59 of 83 patients (71%) with acute myocardial infarction, as indicated by reperfusion of coronary arteries. Reocclusion of arteries occurred in 20.5% of patients. Based on these findings, a subsequent study was undertaken in 302 patients with evidence of recent acute myocardial infarction. Single intravenous bolus doses of APSAC (5 to 30U) produced reperfusion in 79% of patients, with reocclusion occurring in only 9% of patients receiving the higher doses. Adverse effects included an initial hypotension/bradycardia reaction, a later syndrome featuring pyrexia, nausea and vomiting, and bleeding complications, including 4 patients with cerebrovascular accidents. In these early studies APSAC appeared to be as effective as streptokinase, as reported in the literature, and to produce a lower incidence of reocclusion than tissue plasminogen activator.

Isolated tricuspid valve prolapse diagnosed by echocardiography.

Isolated tricuspid valve prolapse in the absence of mitral valve prolapse or other cardiac defects has not been previously noted. This report describes a patient who on both M-mode and two-dimensional echocardiography demonstrated tricuspid prolapse without other associated abnormalities. The implications of this finding are discussed.

Intimacy maturity and its correlates in young married couples.

Intimate relationships, like the individuals who participate in them, are conceived of as following developmental processes. Five dimensions of intimacy--that is, relationship orientation, caring-concern, commitment, sexuality, and communication--are identified, and an approach to assessing relationship maturity on each of these dimensions is described. In a study of 31 young married couples, scores on each dimension of intimacy maturity were analyzed in relation to gender, gender role (as assessed through an adaptation of the Bem Sex Role Inventory; Bem, 1974), and marital adjustment. There were no significant differences between husbands and wives on any of the intimacy or marital adjustment scales and only a marginal difference on one gender role scale (agency). Patterns of correlations among intimacy, marital adjustment, and gender role scores varied by gender. Not only do there appear to be two marriages (his and hers) in every marriage, but the correlates of these marriages also vary.

Safety of ticarcillin disodium/potassium clavulanate.

Clavulanate potentiated ticarcillin contains two components with an established safety record. Ticarcillin used clinically alone and with clavulanate, a component of clavulanate potentiated amoxycillin, given orally or intravenously. No pharmacokinetic interaction occurs between the two components when given together in man or animals in which metabolism is qualitatively similar to man. Safety evaluation studies carried out in the animals established as suitable for studying the toxicology of ticarcillin have shown no unexpected synergistic or antagonistic toxic effects of Timentin not predicted from the toxicological evaluation of clavulanate alone. Reproductive mutagenic, cardiovascular and general pharmacological studies have shown no significant hazard from Timentin. Clinical experience with ticarcillin has been reviewed, and impairment of platelet function, seen at supratherapeutic doses, has been shown in an animal model not to be influenced by clavulanate. The assessment of the safety of ticarcillin and clavulanate for therapeutic use in man is thus comprised of the clinical experience with ticarcillin and parenteral clavulanate in clavulanate potentiated amoxycillin, animal safety evaluation studies, and the clinical experience with Timentin reported in this symposium.

Early treated hypothyroidism: development at 3 years.

Eighty children from the North West and North East Thames Regional Health Authorities who had been identified as having congenital hypothyroidism either by neonatal screening (76 cases) or by clinical symptoms (four) were seen for psychological assessments at 1 (60 cases), 3 (58), and/or 5 (20) years of age. Almost all the children's scores were in the normal range, and comparisons with matched controls suggested that they were doing only slightly less well than normal children in overall development. They were significantly slower, however, on a motor skills task. Moreover, a low initial serum thyroxine and triiodothyronine value, considerable delay in initial bone age, and an absence of thyroid tissue on isotope scan seemed to be associated with a somewhat poorer prognosis.

Acute bacterial conjunctivitis. Bacteriology and clinical implications.

We report the results of eye culture specimens, obtained from patients under 20 years of age, submitted to the Bacteriology Department of our institution from January 1 through April 30, 1983. A total of 72 specimens were positive for one or more strains of bacteria. The most commonly isolated bacteria was Hemophilus influenzae (34 strains, 42%), followed by Staphylococcus epidermidis (11 strains, 13.75%) and Streptococcus pneumoniae (9 strains, 11.25%). Mean age of patients with H. influenzae (excluding a 20-year-old patient) was 15 months with standard deviation of 13 months. Chloramphenicol and tetracycline showed excellent in vitro activity against bacteria of all age groups. Tetracycline may prove to be the drug of choice for the treatment of acute conjunctivitis if comparative clinical data support its in vitro superiority.

Children of alcoholic fathers and recovered alcoholic fathers: personal and family functioning.

Twenty-one adolescent children of recovered alcoholic fathers and 14 children of alcoholic fathers were compared with 35 sociodemographically matched children on aspects of family and personal adjustment, the parent-child relationship and perceptions of alcoholism. Children of recovered alcoholics and controls rated their families as happier and more trusting, cohesive, secure and affectionate than children of families where father still drank alcohol. Adolescents scored similarly on measures of self-esteem and locus of control, but children of alcoholics were less happy with their lives. The three groups did not differ in their relationships with either parent. Children of alcoholic or recovered alcoholic fathers were less likely to attribute alcoholism to internal causes than controls, however, and were more positive about alcoholics and their recovery.

Passage of lanthanum through the intercellular spaces of the sebaceous gland.

Lanthanum introduced intradermally into cattle, sheep, goats, ponies and rats penetrated into the sebum through the intercellular spaces of the sebaceous gland. It is concluded that the sebaceous gland is permeable to the passage of small molecules outwards and probably in some circumstances inwards. Since the constituents of sebum are not all produced by necrosis it is likely that the sebaceous gland is not a simple holocrine gland.

Route of passage of cypermethrin across the surface of sheep skin.

14C cypermethrin applied topically to the dorsal surface of sheep moved radially across the skin within the stratum corneum of the epidermis at a rate which exceeded 11 cm h-1. This spread was accompanied by some dermal infiltration which was most marked at the site of application.

Metabolic cooperation between mouse mammary tumor subpopulations in three-dimensional collagen gel cultures.

A series of subpopulation lines derived from a single mouse mammary tumor were tested for their ability to interact with each other in metabolic cooperation assays in three-dimensional collagen gel cultures. Inhibition of growth in the presence of selective drug (6-thioguanine or 2-fluoroadenine) of two 6-thioguanine-resistant cell lines, 66cl4 and 44FTO, as well as a 2-fluoroadenine-resistant line, 168FAR, occurred in the presence of other sensitive cells, demonstrating the transfer of drug sensitivity to drug-resistant cells. Transfer of drug resistance was shown by using selective medium containing hypoxanthine, aminopterin, and thymidine plus ouabain in which mutual metabolic cooperation between line 66cl4 (also ouabain resistant) and "wild-type" lines is required for growth. Effective communication leading to the transfer of drug resistance and ultimate survival of both cell types occurred in mixtures containing as little as 10% of either cell type. The requirement for mutual metabolic cooperation in hypoxanthine, aminopterin, and thymidine plus ouabain medium prevented line 66 from overgrowing line 66cl4 to the extent that it did in control medium. The ranked abilities of lines 66, 410.4, and 168 to promote growth when mixed with line 66cl4 in collagen cultures in hypoxanthine, aminopterin, and thymidine plus ouabain medium were correlated with the ranked cell densities required for these same cell lines to inhibit growth of line 66cl4 by metabolic cooperation in the presence of 6-thioguanine in monolayer cultures. These ranked abilities may be related to the effectiveness with which each of these cell lines is able to form communicating junctions with line 66cl4.
