An episode of acute pustular psoriasis in a middle-aged man was associated with cholestatic jaundice and followed by acute tubular necrosis. It is suggested that renal failure was due to oligaemia after the loss of albumin into and from the skin. Fluid balance, central venous pressure, and arterial blood pressure should be monitored in patients with acute pustular psoriasis.
Treatment of hypertension with beta-blocking agents in three patients with moderately severe chronic renal failure was followed by rapid deterioration of renal function. In two of the patients the need for maintenance haemodialysis was accelerated but renal function in the third reverted to pretreatment levels after the drug was stopped. These findings suggest that until more is known about the effects of beta-blocking drugs they should not be given to patients with moderately severe renal failure.

To determine the possible effects of beta-adrenergic blocking drugs on blood flow in arteriosclerotic peripheral vascular disease, we have measured muscle blood flow in eight patients with intermittent claudication who were taking beta-blocking drugs for mild uncomplicated hypertension. Patients completed daily, pindolol 5 mg three times daily, atenolol 100 mg daily, or placebo. Mean exercise muscle blood flow fell by approximately 30% after each active drug when compared to placebo. beta-Blocking drugs may be contraindicated in the presence of peripheral vascular disease. Cardioselectivity and partial agonist activity appear to confer no advantages in such patients.
CA 125, a high-molecular-weight mucin, was first defined in 1981 by the monoclonal antibody OC125. Until recently, it has defied many attempts to purify it from a variety of sources, although many research groups have successfully raised antibodies that bind to CA 125. Nevertheless, CA 125 has demonstrated its considerable value as a marker in monitoring patients with ovarian cancer. This year, two research groups have succeeded in cloning the high-molecular-weight mucin CA 125. Their findings are summarized and the significance discussed in light of existing data from the human genome.
In this article we describe the successful management of pregnancy and delivery in a 26-yr-old patient with advanced diabetic nephropathy and chronic renal failure. Targets for control of blood urea and hemoglobin were achieved with the aid of continuous ambulatory peritoneal dialysis (CAPD). Peritoneal dialysis did not interfere with normal recovery from cesarean section. With CAPD, successful pregnancy is now possible in this group of patients, among whom fetal loss would otherwise be high.
Subclones of the two well-characterized myeloid cell lines HL-60 and KG1a were selected for doxorubicin resistance by systematic exposure to increased concentrations of the drug in vitro. Both subclones demonstrated a threefold increased resistance to the drug as evident from cell growth in liquid culture and clonogenicity in a semisolid matrix. Both resistant subclones displayed a similar degree of reduced total and nuclear doxorubicin levels. The HL-60 and the KG1a cells differed qualitatively and quantitatively with respect to glutathione (GSH) levels during culture, with markedly elevated concentrations in the resistant HL-60 subclone during 1 week of culture. Total GSH pools in resistant and sensitive KG1a cells were similar, but maximum GSH levels were reached earlier in the resistant KG1a clones than in the parental cells. Northern blot analysis suggests that resistance was accompanied by increased mdr1 expression in the KG1a but not in the HL-60 cells, whereas alterations in the glutathione S-transferase P1-1 and topoisomerase II message was evident in the latter. The results demonstrate the complex, multifactorial mechanisms behind the in vitro induction of even moderate resistance in anthracyclines.
BACKGROUND AND PURPOSE: Conventional catheter angiography (CCA) is the current reference standard for the diagnosis, assessment, and management of pial brain arteriovenous malformations (AVMs). The purpose of this study was to develop an MR angiographic technique that produces dynamic images comparable to those provided by CCA and to apply the technique to the investigation of pial brain AVMs. METHODS: Twenty patients with brain AVMs referred for stereotactic radiosurgery were recruited. All patients had CCA performed on a 1.5-T superconducting system. Sixty images were obtained at a rate of one image per second. Slices were orientated to produce Towne, lateral, and anteroposterior projections. A set of mask images was taken and then a series during the passage of a bolus of contrast material. MR examinations were assessed independently by neuroradiologists blinded to the conventional catheter angiographic findings. RESULTS: The nidus of the AVMs was depicted in 19 of the 20 patients, and correlation with CCA was excellent for measurements of maximum diameter. Venous drainage was correctly assessed in 18 of 19 cases. CONCLUSION: MR digital subtraction angiography shows promise as a noninvasive, dynamic angiographic tool for planning stereotactic radiosurgery of AVMs already delineated by catheter angiography. At present, it suffers from temporal and spatial resolution, which impede the assessment of some brain AVMs.
