It is now recognized that a previously normal fold of synovium on the medial side of the knee (a medial patellar plica) may, as a result of trauma, become pathologically inflamed and/or fibrosed, producing symptoms that may be mistakenly diagnosed as a torn meniscus or chondromalacia patellae. Clinical awareness, aided by arthroscopy, can establish the diagnosis. The correct treatment is simple and, if instituted early before irreversible changes have occurred in the femoral condyle articular cartilage, is extremely effective.
The effects of intrarectal metronidazole and intraincisional povidone iodine on sepsis after emergency appendicectomy were compared in a double-blind randomised controlled trial in 496 patients. Wound sepsis occurred in 12.3% of metronidazole-treated patients compared with 24% in the povidone-iodine group and 23.5% in an untreated control group. The metronidazole-treated patients left hospital approximately 2 days earlier than patients in the other two groups. They returned to work sooner and receiver fewer visits from the district nursing service. A short six-dose course of metronidazole significantly reduces the wound-infection rate in patients over the age of 12 undergoing emergency appendicectomy. If the clinical and economic benefits of metronidazole shown by this study are confirmed, the drug should be considered for routine use in emergency appendicectomy.

Mass spectrometric sensitivity data for the compounds cholesterol, cholestane, DDT, decafluorotriphenylphosphine, morphine, LSD and methyl stearate have been developed over a period of time and under a variety of conditions. This information is presented on both a relative and an absolute basis. With the exception of morphine, relative sensitivies were reproducible to within a factor of three over a six month period. This reproducibility facilitates quantitation and a single standard can be used for calibration of organic compounds from various classes. Detection limits of 50 femtograms for LSD are realized for samples introduced via the direct inlet probe.
A series of 7alpha,8alpha-expoxyestradiol derivatives with ethynyl, 2- or 3-furyl, or 2-thienyl substituents in the 17alpha position was prepared. The products were highly active orally in the Allen-Doisy test in rats, but most of them were only weakly active in the uterotrophic assay in the mouse. A 7alpha,8alpha-methylene analog and a 7alpha,8alpha-difluormethylene analog were less active than the corresponding epoxides.
Nocardia species aromatized 19-hydroxyprogesterone, 3beta, 19-dihydroxy-pregn-5-en-20-one 3-acetate and pregn-5-ene-3beta, 19, 20beta-triol 3-acetate, without cleavage of the side chain, into 3-hydroxy-19-norpregna-1,3,5 (10)-trien-20-one. Septomyxa affinis aromatized the ring A and cleaved the side chain of 19-hydroxyprogesterone to yield estrone. With 19-hydroxypregna-4, 7-diene-3, 20-dione as substrate, the transformation was more complex and many products were formed.
