Transmyocardial laser revascularization.

Transmyocardial laser revascularization is a new treatment modality for patients with chronic angina that is refractory to traditional management. A laser is used to create full-thickness channels through areas of ischemic myocardium. Transmyocardial laser revascularization is performed in an effort to improve myocardial oxygenation, eliminate or reduce angina, and improve functional status. Transmyocardial laser revascularization currently is undergoing clinical trials. Early results have demonstrated significant reduction in anginal symptoms. Long-term efficacy of transmyocardial laser revascularization remains to be determined.

Comprehensive assessment of families of the critically ill.

The family enters the critical care unit under a significant amount of stress with many diverse and complex needs. During this crisis period, dysfunctional behavior may lead to ineffective coping. This article will present a comprehensive family assessment tool based on Myra Levine's Principles of Conservation. The assessment tool not only will enhance the comprehensiveness of patient and family care, but will provide a method for reassuring families that they are part of their relatives' care and recovery. This tool will also enable the critical care nurse to individually assess and meet the holistic needs of families.

Need satisfaction levels of family members of critical care patients and accuracy of nurses' perceptions.

The purpose of this research was to gather information about the level of need satisfaction as perceived by family members and the extent to which critical care nurses are able to accurately identify those areas of high and low family member satisfaction. A descriptive-comparative study was completed. Participants were 92 critical care nurses and 52 family members of patients in the critical care units of two large general hospitals. Family members reported being more satisfied than dissatisfied for 43 of the 46 needs. They were relatively well satisfied with those needs that pertained to personal support systems, visitation, and information. They were less satisfied with those that related to psychologic aspects, the environment, and institutional support services. Critical care nurses were moderately accurate at identifying the extent to which family members perceive their needs as being met. However, numerous items were identified for which marked disagreement was found.
