The Cockayne-Touraine type of dominant dystrophic epidermolysis bullosa--ultrastructural similarities to the Pasini variant.

Biopsies from the uninvolved skin of 5 patients with the Cockayne-Touraine (CT) type of dystrophic epidermolysis bullosa (DEB) were studied with the electron microscope. Dermal fibrillar bodies were noted in 2 patients and 3 showed basal lamina (BL) duplication or splitting. Discontinuity of the BL with herniation of keratinocyte cytoplasm was present in one patient. These changes, thought previously to be characteristic of the Pasini variant, indicate that abnormalities of the BL may be involved in blister formation in both disease subgroups.

Marfan's syndrome presenting as bilateral spontaneous pneumothorax.

A case of bilateral spontaneous pneumothorax in a 14-year-old girl with previously undiagnosed Marfan's syndrome is described. The pulmonary abnormalities of Marfan's syndrome are not commonly encountered and bilateral pneumothorax is itself a rare event which, in most instances, has been reported following invasive procedures.

The influence of extracellular matrix proteins on cutaneous and uveal melanocytes.

Cutaneous and ocular melanocytes are routinely cultured in complex mitogen-rich media. The physiological regulation of melanocyte proliferation and differentiation is not yet fully defined and this study summarises several separate lines of evidence which suggest that, in vivo, some of the signals required for melanocyte proliferation and differentiation may derive from extracellular matrix (ECM) proteins adjacent to these cells. Culture of cutaneous and uveal melanocytes on cell-derived and individual ECM proteins was found to influence cell morphology with such effects being most noticeable in mitogen-deficient media. Similarly, cell-derived and individual ECM proteins increased tyrosinase activity in normal cutaneous melanocytes and effects of these ECM proteins were seen most consistently in mitogen-deficient media. Uveal melanocytes (as has been reported for cutaneous melanocytes) showed preferential attachment to fibronectin over other ECM substrates. This attachment was particularly sensitive to drugs which affected intracellular calcium or calmodulin activity. Acute addition of fibronectin to coverslips of uveal melanocytes loaded with Fura-2 produced an acute and transient increase in intracellular calcium which was more prevalent in low density than higher density cells. We conclude that ECM proteins in vitro are capable of influencing melanocyte morphology, tyrosinase activity, and proliferation and that an ECM-induced elevation in intracellular calcium may be part of the signalling system that transmits ECM information into the cell.

A prospective study of the responsiveness of depression and suicidal ideation in acne patients to different phases of isotretinoin therapy.

The degree and manner to which isotretinoin affects the mental health of acne patients has not been extensively researched, despite reports of possible associations between isotretinoin and depression. In this study, 33 patients with acne were prescribed a standard 16-week treatment regime with isotretinoin. At the initiation of isotretinoin treatment, week 8 review and termination of treatment, patients completed validated measures of depression, hopelessness and self-rated dermatological severity and were also assessed dermatologically using an acne grading protocol. When the first phase of isotretinoin treatment (week 1 to week 8) was compared to the second phase (week 9 to week 16), patients reported significant improvements in the cognitive-affective features of depression during the first phase of treatment, but not during the second phase. Corresponding improvements in the somatic symptoms of depression and hopelessness were not found. The implications of the research are discussed in relation to methodological design issues in this area.
