Rollerball endometrial ablation: a histological study in danazol pretreated patients.

Endomyometrial biopsies were taken from danazol pretreated patients about to undergo rollerball endometrial ablation for menorrhagia. The biopsies were taken with a loop electrode at hysteroscopy before and after 1, 2, 3 passes of a rollerball electrode over the endometrium using powers of electrocoagulation diathermy varying from 30 to 100 watts. The biopsies were then assessed histologically. Nineteen biopsies from the perceived endocervical canal, 25 posttreatment curettings and 3 hysterectomy specimens were also assessed. Biopsies taken after danazol pretreatment but before diathermy showed a thinned inactive endometrium. A histological classification of the effect of electrocoagulation diathermy on the endometrium was developed describing increasing endometrial damage. The degree of endometrial damage was directly related to the number of rolls of the ball over the endometrium and to the power of electrocoagulation diathermy. Endometrium was found in 84% of biopsies from the perceived endocervical canal and endometrium was found in curettings from 3 of 11 patients who were amenorrhoeic following ablation.

Submucous fibroids: clinical profile and hysteroscopic management.

Fifty submucous fibroids were removed by hysteroscopic resection from 37 patients with abnormal uterine bleeding. Thirty-seven fibroids were completely resected and 13 incompletely resected. Nine patients also had rollerball endometrial ablations. The average follow-up period was 14 months (4 to 51 months) in 33 patients available for follow-up. All of these patients had cure of their symptoms, 3 after repeat operations. The 1 immediate complication was a postoperative infection. A normal uterine cavity was found in 13 of 14 patients examined by office hysteroscopy at follow-up.

Uterine perforation associated with endometrial ablation.

Three cases of uterine perforation associated with endometrial ablation are presented. Possible predisposing factors in 2 cases were previous Caesarean section and difficulty in cervical dilatation. Adequate training for gynaecologists in hysteroscopy is essential before undertaking any type of operative hysteroscopy.

Hysteroscopy. Its place in modern gynaecology.

Hysteroscopy has rapidly become important for the diagnosis and treatment of gynaecological problems. General practitioners are being inundated with inquiries from patients about this advance, particularly as media reports give general exposure to the subject. This article outlines the place of these new advances.

A survey of 59 triplet pregnancies.

The outcome of 59 triplet pregnancies from four hospitals was analyzed. The combined stillbirth and neonatal mortality rate was 232/1000. Preterm labour and birth order were the most important identifiable factors in relation to neonatal death. Birth weight within a triplet set and malpresentation were not related to neonatal death. Elective cervical suture appeared to be of no value in prolonging gestation.

Electroencephalographic changes after intra-amniotic prostaglandin F2alpha and hypertonic saline.

Six major convulsions have been seen among 555 patients who had therapeutic second trimester abortion by intra-amniotic prostaglandin F2alpha (PGF2alpha). Five of 21 patients receiving intra-amniotic hypertonic saline and 5 of 16 patients receiving PGF2alpha showed abnormalities in electroencephalograms after treatment. These changes were relatively minor in all saline patients and in three receiving PGF2alpha, but two patients receiving PGF2alpha showed epileptiform spike activity after infusion. There was no correlation between the electroencephalographic (EEG) changes and peripheral venous levels of PGF2alpha or its metabolite, 13, 14 dihydro-15 keto PGF2alpha (15 keto-h2-f2alpha).
