Bronchoalveolar lavage in sarcoidosis.

Bronchoalveolar lavage (BAL) was performed in 1,188 patients suffering from sarcoidosis. After technical considerations, the authors analyze the results of BAL from a practical point of view concerning its value for the diagnosis of sarcoidosis and its prognostic value and its value for the selection of therapy, particularly for the decision as to steroid treatment. BAL helps in the diagnosis of sarcoidosis, but is not specific enough to provide this diagnosis on its own. The persistence of high alveolar lymphocytosis within the first year of evolution is strongly correlated with nonrecovery from pulmonary sarcoidosis at 2 years and thus with the evolution towards a chronic phase of the disease. On the other hand, BAL can provide basic information for a better understanding of the disease and permits immunocompetent cells and soluble factors to be recovered from the lung, which is useful for immunological studies.

Marked reduction of DNA antibody production and glomerulopathy in thymulin (FTS-Zn) or cyclosporin A treated (NZB X NZW) F1 mice.

In this study we have evaluated the action of two compounds, cyclosporin A (Cy A) and thymulin (FTS-Zn) in old (NZB X NZW) F1 male mice which develop severe autoimmune disease. Compared with control mice, thymulin and Cy A treated mice already showed by 2 weeks a dramatic diminution of their anti-DNA antibody level which remained low throughout the treatment period and persisted for 7 weeks afterwards. Renal specimens were studied by direct immunofluorescence in Cy A and thymulin treated mice. Glomerular deposits were decreased compared with control mice; these differences were particularly clearcut for IgG1 and IgG2 deposits. Finally, thymulin (FTS-Zn) and Cy A appeared to induce the same improvement of DNA titres and glomerulopathy in (NZB X BW) F1 mice.

[Angio-immunoblastic lymphadenopathy and neurological manifestations (author's transl)].

Cases of associated angio-immunoblastic lymphadenopathy (AIL) and peripheral neuropathies have been rarely reported in the literature. Three such cases are described, in two men and one woman aged 79, 59, and 45 years respectively. Diagnosis of AIL was confirmed by lymph node biopsy in the two latter patients during their lifetime, and in the first case from examination of cervical nodes at autopsy. All three patients presented neurological disorders, of the polyradiculoneuritis type in the first case, multiple neuritis of the lower limbs followed by radiculalgia in the second, and myalgia and neuralgia with neurogenic signs in the EMG in the third case. No evidence of a toxic, metabolic, or infections aetiology was found, histological examination of nerve and muscle specimens demonstrated localised AIL lesions in one case, and discrete lymphoplasmocytic infiltration of the peripheral nervous system in the other two patients.
