Hemophilus influenzae b pericarditis in children.

Recent experience in the diagnosis and management of Hemophilus influenzae b pericarditis is described in five children. Anterior pericardectomy appears to be the preferred method of surgical drainage because it was associated with a shorter hospitalization than pericardiocentesis or closed or open pericardotomy and removed the risk of recurrent cardiac tamponade and constrictive pericarditis. Countercurrent immunoelectrophoresis of sera and pericardial fluid was used to rapidly identify the etiology of pericarditis in four of four patients tested. The observation that three children appeared to develop pericarditis in the absence of a contiguous infectious focus suggests that bacteremic seeding of the pericardium may be important in the pathogenesis of this disease.

Generator program for computer-assisted instruction: MACGEN. A software tool for generating computer-assisted instructional texts.

This publication describes MACGEN, an interactive development tool to assist teachers to create, modify and extend case simulations, tutorial exercises and multiple-choice question tests designed for computer-aided instruction. The menu-driven software provides full authoring facilities for text files in MACAID format by means of interactive editing. Authors are prompted for items which they might want to change whereas all user-independent items are provided automatically. Optional default values and explanatory messages are available with every prompt. Errors are corrected automatically or commented upon. Thus the program eliminates the need to familiarize with a new language or details of the text file structure. The options for modification of existing text files include display, renumbering of frames and a line-oriented editor. The resulting text files can be interpreted by the MACAID driver without further changes. The text file is held as ASCII records and as such is also accessible with many standard word-processing systems if desired.

HIV-positive mothers and stigma.

Our purpose in this paper is to demonstrate how stigma pervades the lives of human immunodeficiency virus (HIV)-positive mothers and their children. Data from a grounded theory study on HIV-positive mothers are used to illustrate Goffman's theory of stigma. This research is an example of "emergent fit," where extant theory is discovered by the interpretive researchers to fit much of the data. The sample included 18 HIV-positive mothers who participated in in-depth interviews. The HIV-positive mothers valued being perceived as normal but acknowledged that normalcy was lost for them because of the stigma of HIV. Consequently, they tried to pass as normal by managing information and manipulating their environment. They attempted to cover up their illness by lying and pretending. Health care professionals can provide quality, client-centered care when they understand the power that stigma holds over these women and the strategies that effectively mitigate the stigma.

Low-molecular-weight peptides derived from extracellular matrix as chemoattractants for primary endothelial cells.

The development of synthetic and naturally occurring scaffolds for tissue engineering applications has included strategies to promote attachment of specific cell types, control the rate of scaffold degradation, encourage angiogenesis, or otherwise modulate the host response. We have reported that bioscaffolds developed from porcine small intestinal submucosa (SIS) facilitate the constructive remodeling of tissues and recruit marrow-derived cells that persist long after the acute inflammatory stages have resolved. We have not yet determined which cells are recruited, the eventual fate of these cells, or via what mechanisms the events occur. We now have analyzed various molecular weight fractions of acid-hydrolyzed SIS by both functional and morphologic methods and have determined that fraction 4 (5 to 16 kDa) possesses chemoattractant activity for primary murine adult liver, heart, and kidney endothelial cells in vitro. Addition of fraction 4 to Matrigel plugs promoted in vivo vascularization when the plugs were implanted subcutaneously in mice. These results indicate that small-molecular-weight peptides derived from the degradation of porcine SIS are biologically active in the recruitment of murine endothelial cells in vitro and in vivo.

The development of a clinical skills centre.

Guidelines produced by the General Medical Council of Great Britain have emphasised the importance of the development of the skills and attitudes appropriate for a junior doctor. Medical schools are in the process of reforming their curricula accordingly. The development of these skills is made increasingly difficult by changes such as short admissions to hospital, increased care in the community, and reduced resources. This article outlines the development of a clinical skills centre as a multidisciplinary unit to improve clinical skills teaching with the aid of up-to-date technology and educational practices. By sharing our experience we aim to provide a practical guide for the development of such units in other medical and nursing colleges.
